Spinal cord evoked injury potentials in patients with acute spinal cord injury.

Six patients were examined in the acute stage of spinal cord injury, between 11 h and 12 days posttrauma. Quadripolar epidural electrodes were positioned either percutaneously using a Tuohy needle or directly into the epidural space during surgical intervention. These electrodes were combined with a common reference to obtain monopolar recordings of spinal cord evoked potentials resulting from either median nerve stimulation at the wrist or tibial nerve stimulation at the popliteal fossa. Spinal cord evoked injury potentials (SCEIPs), stationary potentials with positive polarity on the distal …

Spinal cord vascularity. I. Extraspinal sources of spinal cord arteries in man.

The arrangement of extraspinal sources of the spinal cord arterial supply in man is more complicated than previously described, especially with regard to the origin and branching of the aortic segmental arteries. The fact that other arteries in the neck than the vertebral artery, such as the costo-cervical trunk and the ascending cervical artery may contribute to the supply of the cervical cord is confirmed, and also the occurrence of two or more spinal branches from different sources entering the same intervertebral foramen. Frequent occurrence of two or more segmental arteries arising from a…

Spinal Cord Infarction as an Immediate Complication of Spinal Cord Stimulator Placement.

BACKGROUND: Spinal cord stimulators (SCSs) are a widely used intervention for managing chronic neuropathic conditions. SCSs are considered safe, but rare complications can arise, typically attributed to spinal cord compression or contusion. Spinal cord infarction as a complication of SCS placement has not been previously reported in the literature. CASE REPORT: A 76-year-old woman developed spinal cord infarction resulting in right leg paralysis following SCS placement. Immediately postprocedure, there was bilateral lower extremity paralysis with partial recovery of the left leg but persisten…

Improved Sensation With Transcutaneous Spinal Cord Stimulation Following Chronic Spinal Cord Injury.

INTRODUCTION: Following spinal cord injury (SCI), recovery of sensation is often neglected despite its critical roles in motor task execution and injury prevention. The Up-LIFT trial recently found targeted transcutaneous spinal cord stimulation (tSCS) combined with rehabilitation significantly improved sensation in addition to upper extremity (UE) strength and function. Here, we explore the clinical and functional significance of these sensory improvements. METHODS: Sixty participants with chronic, incomplete cervical SCI completed a two-month rehabilitation run-in phase followed by two mont…

Transplantation of engineered spinal cord organoids restores functions after spinal cord injury.

Generating functional maturation neural organoids to model degenerative disease or replace large damaged central nervous tissue remains an enormous challenge. Here, we developed a novel blood vessel-mimicking nanomaterial system by combining carboxylated cellulose nanofibres (CCN) with Matrigel to create biosafety scaffolds. These engineered scaffolds demonstrated a remarkable capacity to support the long-term growth over 300 days and functional maturation of neural organoids, enabling the development of centimetre-scale organoids without necrotic cores. CCN-engineered human spinal cord organo…

Cardiovascular safety of transcutaneous spinal cord stimulation in cervical spinal cord injury.

This study evaluated whether cervical transcutaneous spinal cord stimulation (tSCS) in conjunction with rehabilitation on upper extremity function alters blood pressure regulation in individuals with cervical spinal cord injury. This study is a secondary analysis of the Up-LIFT trial, a prospective single-arm multicenter trial designed to evaluate the safety and efficacy of tSCS in conjunction with rehabilitation (tSCS + rehab) on upper extremity function in individuals with chronic cervical spinal cord injury. Utilizing this large data set obtained from 60 individuals across 14 internationa…

Ascending Spinal Cord Infarction Secondary to Recurrent Spinal Cord Cavernous Malformation Hemorrhage.

We report a case of a 58-year-old Hispanic man who developed ascending paraparesis over several weeks secondary to recurrent hemorrhages and resulting in spinal cord ischemia from a low thoracic spinal cord cavernous malformation. The patient's deterioration was attributed to recurrent hemorrhage of a thoracic intramedullary cavernous malformation at T11 resulting in vascular congestion and spinal cord ischemia. The patient was found to have a heterozygous mutation on exon 13 of gene KRIT1, which was consistent with autosomal dominant familial cerebral cavernous malformations. Expedited surgic…

Imaging of Spinal Cord Injury: Acute Cervical Spinal Cord Injury, Cervical Spondylotic Myelopathy, and Cord Herniation.

We review the pathophysiology and imaging findings of acute traumatic spinal cord injury (SCI), cervical spondylotic myelopathy, and briefly review the much less common cord herniation as a unique cause of myelopathy. Acute traumatic SCI is devastating to the patient and the costs to society are staggering. There are currently no "cures" for SCI and the only accepted pharmacologic treatment regimen for traumatic SCI is currently being questioned. Evaluation and prognostication of SCI is a demanding area with significant deficiencies, including lack of biomarkers. Accurate classification of SCI…

[A study of spinal cord ischemia during aortic cross-clamp--evoked spinal cord potential and histological analysis of the spinal cord].

The relationship between the evoked spinal cord potential (ESP) and the histological findings of the spinal cord after thoracic aortic cross-clamp was studied. Thoracic aorta was cross-clamped in 23 dogs and ESP was monitored before, during, and after cross-clamping. Incidence of paraplegia and histological findings were studied after the dogs recovered from the procedure. Aortic cross-clamp was maintained for 60 minutes in 20 dogs (Group A). And cross-clamp was released 10 minutes after the amplitude of ESP became lower than 20% of control in 3 dogs. (Group B). In group A, three types of ESP …

Monitoring of spinal cord perfusion pressure in acute spinal cord injury: initial findings of the injured spinal cord pressure evaluation study*.

OBJECTIVES: To develop a technique for continuously monitoring intraspinal pressure at the injury site (intraspinal pressure) after traumatic spinal cord injury. DESIGN: A pressure probe was placed subdurally at the injury site in 18 patients who had isolated severe traumatic spinal cord injury (American Spinal Injuries Association grades A-C). Intraspinal pressure monitoring started within 72 hours of the injury and continued for up to a week. In four patients, additional probes were inserted to simultaneously monitor subdural pressure below the injury and extradural pressure. Blood pressure…

Intramedullary Spinal Cord Metastasis in Spinal Ependymomas.

Drop metastasis, or intramedullary spinal cord metastasis (ISCM), is a rare condition accounting 0.9%-5% of CNS or spinal metastases. It arises from tumour cell dissemination through cerebrospinal fluid (CSF), often affecting the thoracic and lumbar spine. Clinical features include rapidly progressive neurological deficits, limb weakness, sensory loss, and pain. Diagnosing drop metastasis is challenging and MRI with gadolinium remains the gold standard modality for investigation. Surgical resection followed by radiotherapy remains the primary treatment option, whereas chemotherapy serves a pal…

Avoiding Spinal Cord Conundrums.

Spinal cord maladies occupy a uniquely perilous corner of emergency medicine. The challenge is that spinal cord pathologies often present subtly, with nonspecific and evolving symptoms such as atraumatic back pain, urinary retention, minimal leg weakness, or imbalance, all of which can easily be dismissed or attributed to an alternative diagnosis at initial presentation. Because early symptoms are often not pronounced or yet definitive, it is essential to recognize deviations from expected patterns of more benign disease, perform detailed history and physical examination, and act on atypical f…

Neonatal Spinal Cord Injury.

Neonatal spinal cord injury is rare, and diagnosis is often delayed. The causes of spinal cord injury are quite broad including mechanical trauma, ischemia in the prenatal or perinatal period, iatrogenic causes, or other etiologies such as spinal abnormalities or neoplasms. The perinatal history and specific physical examination findings can suggest a diagnosis of neonatal spinal cord injury, which warrants confirmation by magnetic resonance imaging. Management involves supportive care and physical and occupational therapy. Early recognition is paramount to determine the specific cause, manage…

Metastatic spinal cord compression.

Metastatic spinal cord compression is compression of the spinal cord or cauda equina as a result of metastatic deposits in the spinal column. It affects approximately 4000 cases per year in England and Wales. Prompt identification and treatment of metastatic spinal cord compression is necessary to prevent irreversible neurological injury, treat pain and maintain patients' mobility, function and independence. Survival of patients with common malignancies has improved significantly with the ongoing development of radiotherapy and chemotherapy, as well as improved surgical treatment of resectable…

Malignant spinal cord compression.

Malignant spinal cord compression (MSCC) is a potentially devastating consequence of cancer. Early recognition of the signs and symptoms of MSCC can allow diagnosis prior to the development of irreversible complications. Information provision to patients and doctors regarding the risk of MSCC and a streamlined pathway for further investigation are both key to improving the outcome for patients developing this condition. Described in this paper is the development of such a pathway at Aberdeen Royal Infirmary.
