Spironolactone in adolescent acne vulgaris.

Acne vulgaris (AV) is the most common skin condition affecting adolescents, most likely due to elevated androgen levels during puberty. Androgens stimulate and enlarge the sebaceous glands and keratinocytes, resulting in increased production of sebum and abnormal hyperproliferation of keratinocytes which lead to the formation of acne lesions. Current standard of care for AV includes topical therapies for mild cases and antibiotics or oral retinoids for severe cases. In recent years, spironolactone, an aldosterone antagonist and diuretic, has been applied to the treatment of AV due to its anti-…

Spironolactone for
Adult Female Acne.

Many cases of acne are hormonal in nature, meaning that
they occur in adolescent girls and women and are aggravated
by hormonal fluctuations such as those that occur during the menstrual cycle or in the setting of underlying hormonal imbalances as seen in polycystic ovary syndrome. For these patients, antihormonal therapy such as spironolactone is a valid and efficacious option. Herein, initiation and utilization of this medication is reviewed.

Upper gastrointestinal bleeding and spironolactone.

Spironolactone is a potassium-sparing diuretic marketed since the 1960s that is known to cause the same adverse effects as other diuretics as well as some effects of its own, such as hyperkalaemia, gynaecomastia and menstrual disturbances. A cohort study and 2 case-control studies concurred to show an approximately twofold, dose-dependent increase in the incidence of upper gastrointestinal bleeding in patients receiving spironolactone. The postulated mechanism is that spironolactone's aldosterone antagonist activity delays gastroduodenal healing. However, gastrointestinal bleeding is not a com…

[Bullous pemphigoid induced by spironolactone].

INTRODUCTION: The role of spironolactone as pemphigoid-inducing agent has recently been suggested. We report a new case of pemphigoid probably induced by spironolactone. The disease resolved after withdrawal of the diuretic drug containing aldactone. OBSERVATION: A 76 year-old patient was hospitalized for eczematiform lesions and severe pruritus. Histological and immunological investigations led to the diagnosis of pemphigoid. For several years, the patient had been treated with acarbose, amlodipine, fluvastatine, buflomedil, lysine acetylsalicylate and a spironolactone-furosemide association…

Use of spironolactone in treatment of hirsutism.

Spironolactone (Aldactone), 100 mg to 200 mg daily, has antiandrogenic effects that may enhance treatment of androgen-excess syndromes, particularly severe hirsutism. Combination therapy with an oral contraceptive or with dexamethasone appears to have a beneficial effect. Side effects are transient. The drug should be avoided during pregnancy and in women who have a family history of breast cancer, although there is no proven association between spironolactone and breast malignancy.

The effect of spironolactone on aromatase activity.

Spironolactone, an aldosterone antagonist, causes decreased plasma testosterone (T) levels and gynecomastia in men and is clinically useful in the treatment of hirsutism in women. Many mechanisms of action for spironolactone have been proposed. It has been suggested that one cause for low plasma T levels may result from an increased metabolic clearance rate of T due to increased extraglandular aromatization to 17 beta-estradiol. The purpose of this investigation was to determine the effect of spironolactone on the rate of activity of aromatase in human fetal liver (hFL) cells. The activity of …

Spironolactone bodies in an adrenal adenoma.

Spironolactone bodies were observed in an adrenal cortical adenoma that was removed from a patient with primary hyperaldosteronism (Conn syndrome) treated preoperatively with spironolactone. The electron microscopical evaluation of this adrenal cortical adenoma shows origin of spironolactone bodies from whorls of endoplasmic reticulum in cells with the cytoplasmic features of those from the zona glomerulosa of the adrenal gland. There was no evidence that the bodies were derived from mitochondria, which confirms recent ultrastructural findings in patients treated with spironolactone. These bod…
