Background: Percutaneous nephrolithotomy under ultrasound-guided access (US-PCNL) has multiple advantages compared with fluoroscopy. No prospective data currently examine the efficacy of this technique for the management of staghorn stones. Our objective was to determine the safety and effectiveness of US-PCNL for the management of staghorn calculi through a prospective cohort study, using strict definitions of stone free based on postoperative computed tomography (CT) imaging. Methods: Utilizing a prospective cohort of US-PCNLs performed by a single surgeon between October 2020 and September …

Less-invasive therapy for pediatric urolithiasis is available due to the miniaturization of equipment and improved optics; however, surgical treatment strategies, especially for large calculi, remain controversial. We describe here our experience of treating a 2-year-old boy with left renal staghorn calculi with a single session of mini-endoscopic combined intrarenal surgery in the prone split-leg position with pre-ureteral stenting and the directional enhanced flow imaging ultrasound technique. This is the first report of successful pediatric mini-endoscopic combined intrarenal surgery withou…

Thirty patients with partial of total staghorn calculi or pyelic calculi greater than 30 mm were treated by extracorporal piezo-electric lithotripsy (PEL) exclusively with an EDAP LT 01 lithotripter equipped with an ultrasound localisation system. Nineteen patients had a pyelic calculus and the others a partial (n = 9) or total (n = 2) staghorn calculus. All patients first underwent extracorporal lithotripsy (ECL). None of the patients received IV sedation or anesthesia. When fragmentation of the calculus was observed after the first session, a double J stent was inserted before the second ELC…

OBJECTIVE: To examine the effectiveness and complication rate of extracorporeal piezoelectric shock wave lithotripsy (EPL) as monotherapy for the treatment of large renal calculi.
PATIENTS AND METHODS: Treatment consisted of semiambulant EPL as monotherapy combined with protective ureteric stenting in 84 patients. Three groups were established according to stone volume (borderline-stone, partial and complete staghorn). An average of 18,230 shock waves was applied in 3.76 sessions to each patient.
RESULTS: No major complications were observed but there were minor complications in 33% of patie

The clinical and laboratory findings in 78 patients with various forms of urolithiasis depending on the presence of primary hyperparathyroidism (PHPT) were analyzed. PHPT was diagnosed in 17 patients. Group "without PHPT" and group "with PHPT" differed significantly in terms of parathyroid hormone (PTH) level, serum calcium, phosphorus, chloride, alkaline phosphatase, calciuria and kaliuria. In patients with staghorn calculi, PHPT was diagnosed in 12.5%, and staghorn calculi in the presence of PHPT were identified in 17.7% of cases. Hypercalciuria in the group "with PHPT" was detected in 82.4%…

A model for immediate production of staghorn calculi in dogs is described. A paste made of gray investment is injected into the renal pelvis and allowed to solidify for 30 min. Calculi thus formed have a striking similarity to naturally occurring staghorn calculi and can be utilized by urology teaching programs to initiate residents in the technique of anatrophic nephrolithotomy.

CASE PRESENTATION: A 25-year-old woman presented to the emergency department with two weeks of crampy right-flank pain, and urinary urgency and frequency. She was found to have a staghorn calculus filling her entire right renal pelvis on computed tomography imaging.
DISCUSSION: In contrast to ureteral calculi, staghorn calculi are more commonly observed in female patients and typically present with an indolent clinical course. A low threshold for imaging should be maintained, as prompt referral to urology for stone removal or treatment is necessary. Staghorn calculi have a high likelihood of …

Background: Staghorn calculi are large renal stones which occupy most of the renal pelvis and are commonly associated with chronic or recurrent upper urinary tract infections (rUTIs). They often require more aggressive management, such as percutaneous nephrolithotomy (PCNL) or nephrectomy, although conservative management may be a safer option for select patients, particularly those with significant comorbidities. The presence of chronic stones or recurrent infections in the kidney increases the risk of complications, including fistula and abscess formation. Case Presentation: A 68-year-old fe…

Complete staghorn calculi present one of the most challenging conditions in managing upper urinary tract stones. The complexity of treatment increases significantly when combined with severe scoliosis, particularly in patients with a Cobb angle exceeding 90°. This case report describes the treatment of a patient with severe scoliosis, characterized by a Cobb angle greater than 90°, who also presented with complete staghorn calculi. The patient successfully underwent single-stage, totally ultrasound-guided, tubeless mini-percutaneous nephrolithotomy, achieving excellent stone clearance without …

Percutaneous nephroureterolithotripsy was performed a total of 309 times in 225 renal units of 209 patients with upper urinary tract calculi, a mean of 1.37 times per kidney. Residual calculi, 5 mm or greater in diameter, were observed in 3 of 100 kidneys (3%) with single calculi, 12 of 102 kidneys (11.8%) with multiple calculi, and 8 of 23 kidneys (34.8%) with staghorn calculi. The number of sessions, duration of nephrostomy, and the frequency of residual calculi were significantly higher in problem calculi, such as multiple, large and staghorn calculi, than in other calculus types. A combina…

In Europe up to nine per cent of people suffer from renal calculi during their lifetime. Staghorn calculi are common and account for ∼11% of cases. Classic presentations include persistent loin pain, recurrent pyelonephritis or cystitis-like symptoms, renal colic or occasional haematuria. We present what we believe to be the first documented case of large bowel obstruction caused by a benign colonic stricture formed secondary to extravasation of a staghorn calculus.

Background and objective Staghorn calculi are big, intricate kidney stones that reside in the calyceal and renal pelvis. They present serious surgical difficulties and a high risk of complications. A less invasive option to standard percutaneous nephrolithotomy (PCNL), mini-percutaneous nephrolithotomy (mini-PCNL) uses smaller tract diameters to minimize bleeding, postoperative pain, and hospitalization while preserving similar stone-free rates (SFR). However, there is still little proof of its effectiveness and safety for staghorn stones, especially in local contexts. This study aimed to asse…

Staghorn renal calculi are large, branched stones in the kidney that partially or completely fill the renal pelvis and renal calyces. Percutaneous nephrolithotomy (PCNL) is the gold standard treatment for staghorn calculi. However, we report a retrograde intrarenal surgery (RIRS) performed to treat a staghorn calculus in a patient with a solitary kidney and a deformed urinary tract. The 37-year-old male patient presented with right-sided lumbar pain. The computed tomography (CT) scan found a solitary kidney on the right side with an opaque 4.5 cm × 2.4 cm renal stone and grade I hydronephrosis…
