CASE REPORT: A 42-year-old female presented with long-standing symptoms suggestive of gastroesophageal reflux disease improved after proton pump inhibitor treatment. An upper endoscopy revealed an intrathoracic position of the stomach (type 4 hiatal hernia) with no mucosal abnormality. Barium swallow demonstrated gastric herniation with gastric volvulus without stenosis. Diaphragmatic disruption is an uncommon consequence of blunt trauma and is often overlooked unless there is a high index of clinical suspicion. A case of eventration of left dome of diaphragm following trauma with mesenteroaxial volvulus of the stomach is presented. We report a rare case of Bochdalekhs hernia, congenital posterolateral diaphragmatic hernia with mesenterioaxia volvulus of the stomach, in an adult. A 23-year-old man suddenly felt severe pain in his left chest after drinking beer. Gastric volvulus is a rare disease. We recently encountered a gastric volvulus associated with Bochdalek hernia and severe hypopotassemia. A 32-year-old woman experienced epigastric pain and recurrent vomiting. The changes of the electrocardiogram in this patient (K1.8mEq/l) were inverted T wave and ST depression. Two cases of impending gastric rupture, associated with malrotation and midgut volvulus, are reported. The diverticulous mucosal protrusions were characteristically revealed as a box-shaped gastric shadow in the plain X-ray and upper GI series. Two cases were operatively treated with only the seromuscular suture of the defect without excision of the mucosal protrusion. Midgut volvulus and malrotation were reduced. Post-operative course was uneventful.

Cases of acute or chronic abdominal complaints in adults can be accounted for as conditions resulting from volvulus of the stomach with oesophagial hernia. In a total of 73 cases of hiatus hernias the author has observed 6 patients with a simultaneous volvulus of the stomach (8,1%) from 1967 to 1976. Pathogenesis, symptoms, surgical indication and the operative technique are discussed. By means of gastropexy, fundiplication and narrowing of the oesphageal hiatus the patients could be freed of complaints and recurrences.

AIM: To evaluate the outcomes of patients who underwent laparoscopic repair of intra-thoracic gastric volvulus (IGV) and to assess the preoperative work-up.

A rare association of pneumoperitoneum induced by pneumatosis cystoides intestinalis with chronic mesenteroaxial volvulus of an irreducible stomach in a paraesophageal hiatus hernia is described. The importance of accurate radiologic work-up is emphasized. The possible significance of this association to explain the pathogenesis of pneumatosis cystoides intestinalis is discussed.
