During October, 1987, 593 sera were collected from risk groups in Sudan's only major deepwater port, Port Sudan. The risk groups included prostitutes, lorry drivers and prisoners. A large proportion of the study participants practised high risk behaviour which included sexual promiscuity, medical treatment by injection, scarification and tattooing. Hepatitis virus infections are the most common cause of liver disease worldwide. Sudan is classified among the countries with high hepatitis B virus seroprevalence. Exposure to the virus varied from 47%-78%, with a hepatitis B surface antigen prevalence ranging from 6.8% in central Sudan to 26% in southern Sudan. Onchocerciasis has been reported in Sudan since 1908 and now prevails in three endemic regions known as the southern, northern and eastern foci. The southern focus is the largest, with nodule and blindness rates exceeding 80% and 12%, respectively, in certain villages. Sheathed microfilariae (mean length 278 +/- 10 microns SEM; mean width 7.2 +/- 0.8 microns) were detected in the blood of 7/14 housed camels (Camelus dromedarius). Microfilaraemic camels of either sex were inappetent, lethargic, reluctant to move and exhibited weakness in the hind limbs; some remained in sternal recumbency. This paper describes an outbreak of dracunculiasis in Mazmum, a town in central Sudan. The study included collection of clinical and epidemiological data from 319 patients treated in hospital, a review of the hospital records, a house survey covering a sample of 757 subjects, a school survey covering 1390 schoolchildren, and examination of water sources. Using individual birth history data from the Sudan Fertility Survey, 1979, parity-related differences in fertility are demonstrated, as well as differences between socioeconomic groups. Rural women, women with no education and those married to uneducated husbands show rapid parity progression and its cumulative effects on fertility which are consistent over all birth intervals. Fifteen strains of the agent of bovine farcy were isolated from lymph nodes of affected cattle. Quantitative analyses of mycolic acids revealed values that allowed the assignment of these strains to the genus Mycobacterium. The organisms bore a greater resemblance to Mycobacterium farcinogenes than to Mycobacterium senegalense.

Seventy-eight urinary stones from the Sudan were analysed quantitatively; 75.6% of upper urinary tract (UUT) stones and 37.8% of lower tract (LUT) stones were composed of pure calcium oxalate monohydrate. Twenty-six cases of acute volvulus of the sigmoid in Khartoum Hospital are analysed. The condition accounts for 17.8 percent of all cases of acute intestinal obstruction. The majority of patients were from the Southern tribes whose diet contains a large amount of fibre. Of the different methods of surgical management simple operative detorsion and deflation gave the best results. The overall mortality rate was 23.1 percent.

A new focus of onchocerciasis is reported from Upper Atbara River area in the Sudan. This river is a known seasonal breeding place of Simulium damnosum. The area was sparsely populated in the past but now, following agricultural development, is inhibited mainly by immigrants from Northern Nigeria and Chad. Out of 46 patients in the Sudan who had been diagnosed clinically as having trachoma 25 strains of the trachoma agent were isolated. Bacterial contamination was easily controlled by streptomycin and vancomycin. On repeated passaging, chick embryos lost their susceptibility to the trachoma agent during the summer.

The prevalence and distribution of trachoma in the Sudan has been studied. The morbidity rate of 83.2 per 1000 in the Northern Province decreases southwards until it reaches 0.94 in the extreme south (Equatoria Province). 2.
