[Sudden death syndrome in infants].

The literature data on the so-called sudden death syndrome (SDS) in children are presented. Theories interpreting this condition from epidemiological, morphological, microbiological, immunological, physiological and biochemical positions are discussed. SDS is thought to be an abstract notion including various definite pathological conditions which are thus far difficult to interpret from nosological positions. A possible role of unfavourable constitutional factors in the SDS genesis, particularly under conditions of virus infection conducive to their manifestation is suggested.

Peptides, Breathing, and Sudden Infant Death Syndrome.

Shi et al. recently identified a brainstem peptidergic circuit in mice that is activated at birth and stabilizes breathing during the transition from the intra-utero environment to air breathing. This network remains important for maintaining stable breathing and respiratory homeostasis throughout development, and deficiencies in this network may be linked to sudden infant death syndrome (SIDS).

[Congenital hyperinsulinism revealed by sudden infant death].

We report the case of a girl of 5 and a half months admitted for discomfort and consciousness loss at home and supported on sudden infant death protocol. Workup was negative. Autopsy showed only signs of asphyxia. Microscopic examination of the pancreas showed hypertrophic beta cells of Langerhans islets, explaining death linked to severe hypoglycemia by inappropriate insulin hypersecretion. This observation highlights the importance of the management of sudden infant unexpected death according to the protocol of the National Health Authority, which includes an autopsy with complete sampling,

Assigning cause for sudden unexpected infant death.

We have reached a conundrum in assigning cause of death for sudden unexpected infant deaths. We summarize the discordant perspectives and approaches and how they have occurred, and recommend a pathway toward improved consistency. This lack of consistency affects pediatricians and other health care professionals, scientific investigators, medical examiners and coroners, law enforcement agencies, families, and support or advocacy groups. We recommend that an interdisciplinary international committee be organized to review current approaches for assigning cause of death, and to identify a consens

Escherichia coli and Sudden Infant Death Syndrome.

This review examines the association of strains of Escherichia coli with sudden infant death syndrome (SIDS) and the possible role these bacteria play in this enigmatic condition. The review addresses evidence for E. coli in SIDS infants, potential sources of E. coli in the environment, colonization by commensal and pathogenic strains, the variety of currently accepted pathotypes, and how these pathotypes could compromise intestinal integrity and induce inflammation. Both intestinal and extraintestinal pathotypes are compared in relation to the apparent liability in which virulence traits can

Sudden infant death syndrome and cardiac arrhythmias.

There is a considerable body of evidence that common bacterial toxins, absorbed from the mucosal surface or delivered as part of a transient bacteremia, have a pathogenic role in sudden infant death syndrome (SIDS). The candidate organisms are Staphylococcus aureus and Escherichia coli. Death in SIDS is rapid, with infants progressing from well, or only mildly unwell, to death in less than 20 min. This mode of death is not typical of infection but it is consistent with toxin action on cardiovascular or respiratory control. Both S. aureus and E. coli secrete toxins (cytolysins and colicins) tha

Sudden Infant Death Syndrome and parental smoking.

Prenatal exposure to tobacco smoke is a major risk factor associated with Sudden Infant Death Syndrome (SIDS) and the risk has increased despite continued advice against this practice. Evidence from the UK suggests the prevalence of maternal smoking during pregnancy has risen amongst SIDS mothers (from 50% to 80%) when the rate amongst expectant mothers in the general population has fallen (from 30% to 20%) confirming pooled estimates from recent studies of a four-fold risk. An additional risk from postnatal exposure has also been identified; increasing with the number of smokers in the househ

Sudden infant death triggered by dive reflex.

The dive reflex is the reflex mechanism most frequently considered in the aetiopathogenesis of sudden infant death syndrome (SIDS). This seems to persist in human beings as an inheritance from diver birds and amphibians. It has been reported that washing the face with cold water or plunging into cold water can provoke cardiac deceleration through the intervention of the ambiguus and the vagal dorsal nuclei. This report describes a case of SIDS that offers a unique insight into the role of the dive reflex in determining a lethal outcome. Examination of the brainstem on serial sections revealed

Sudden infant death syndrome: the colon connection.

The etiology of sudden infant death syndrome (SIDS) is not known. Various maternal and infant risk factors have been identified. Adoption of the non-prone position has reduced the incidence of SIDS but has not eliminated the problem. Some sulfate reducing bacteria in the colon produce hydrogen sulfide (H2S) which is as toxic as hydrogen cyanide. Normally, the colonic mechanism for metabolizing and detoxifying H2S is very effective and no H2S appears in the exhaled breath although small amounts are present in the flatus. We are putting forth the hypothesis that in some cases of SIDS colonocytic

The metabolic investigation of sudden infant death.

Inherited metabolic disorders account for a small but significant number of sudden unexplained deaths in neonates, infants and occasionally older children. In particular, inherited disorders of fatty acid oxidation may closely mimic sudden infant death syndrome. Post-mortem investigations offer the final opportunity to establish a diagnosis. Such diagnoses are of great importance to the families concerned and provide the opportunity for genetic counselling and antenatal diagnosis. Current advances in technology, particularly in the case of electrospray ionization tandem mass spectrometry, have

Air pollution and sudden infant death syndrome.

BACKGROUND: Sudden infant death syndrome (SIDS) affects approximately 1 in 1000 live births and is the most common cause of infant death after the perinatal period. OBJECTIVE: To determine the influence of air pollution on the incidence of SIDS. METHODS: Time-series analyses were performed to compare the daily mortality rates for SIDS and the daily air pollution concentrations in each of 12 Canadian cities during the period of 1984-1999. Serial autocorrelation was controlled for by city, and then the city-specific estimates were pooled. Increased daily rates of SIDS were associated with incr

Febrile convulsions and sudden infant death syndrome.

It has been suggested that sudden infant death syndrome (SIDS) and febrile convulsions are related aetiologically. We compared the risk of SIDS in 9877 siblings of children who had had febrile convulsions with that of 20 177 siblings of children who had never had febrile convulsions. We found no support for the shared susceptibility hypothesis.

Pulmonary eosinophilia in sudden infant death syndrome.

A recent immunohistochemical study found increased numbers of eosinophils, but no mast cells, in the pulmonary parenchyma of infants who died of sudden infant death syndrome (SIDS). The present study tested the hypothesis that this pulmonary eosinophilia could be IgE-mediated. Histomorphometry was used to compare the numbers of eosinophils, mast cells, and IgG-, IgA-, IgM- and IgE-expressing lymphoid cells in the lungs of two groups of infants. Twenty-eight subjects aged less than 1 year were selected from post-mortem records of infant deaths between 1989 and 1992. Fourteen were cases of SIDS

[Position and sudden death of the infant].

In western countries, the sudden infant death syndrome (SIDS) is the leading cause of mortality in infants under one year of age. In an official statement on prevention from the French Ministry of Health, sleeping in the supine position or on the side is recommended for all infants unaffected by a particular medical condition. This clear restrictive recommendation is based on valid epidemiological data but raises questions in the minds of paediatricians and general practitioners since the most recent recommendations for treatment of gastro-oesophageal reflux indicate that the prone position at

Staphylococcal toxins and sudden infant death syndrome.

AIMS: To investigate the hypothesis that commonly occurring bacterial toxins cause sudden infant death syndrome (SIDS) by (1), determining in which tissues bacterial toxins are concentrated after intravenous injection in rats; and (2), seeing if the same tissues contain detectable toxins in cases of SIDS. METHODS: The tissue distribution of intravenously injected staphylococcal enterotoxin A (SEA), enterotoxin B (SEB), enterotoxin C (SEC), enterotoxin D (SED), toxic shock syndrome toxin (TSST-1), and alpha-haemolysin was studied in rats using immunohistology and polyacrylamide gel electrophor

Suffocation, recurrent apnea, and sudden infant death.

We reviewed the cases of 27 young children from 27 different families who were suffocated by their mothers. The certainty, or near certainty, of suffocation was based on reliable observation or recording of the suffocation, maternal confession, or successful prosecution in a criminal court. Eighteen of the children are alive, although one has severe brain damage; nine are dead. Twenty-four were reported to have had previous episodes of apnea, cyanosis, or seizure, and 11 had had 10 or more such episodes that were either invented or caused by the mother. Repetitive suffocation usually began bet

[Sudden infant death--an emotional trigger mechanism].

Southall and coworkers have demonstrated in a recent study that attacks of lifelessness with sudden and severe hypoxemia and cyanosis are caused by a combination of respiratory arrest in expiration, and a right-to-left shunting of the blood through the lungs due to increased pulmonary vascular resistance. This mechanism is thought to be the cause of death. They have also defined the precipitating factors which are sudden, naturally occurring stimuli causing fear, anger or pain. They underline the importance of elements of surprise and unexpectedness. These observations lend strong support to t

Recurrence incidence of sudden infant death syndrome.

The incidence of sudden infant death syndrome (SIDS) between birth and 2 years of age in South Australia was found to be 2.1/1000 live births. The incidence in previous siblings was 10 times that expected, in second degree relatives five times, and in third degree relatives four times that expected for the community. The minimum incidence in the next subsequent sibling was five times that expected. Except for one family in which both twins died during the same night, the surviving twin has never died (23 infants). The incidence of SIDS in adopted infants was similar to the incidence in natural
