Malignant transformation of intracranial meningeal melanocytoma. Case report and review of the literature.

Meningeal melanocytoma is an uncommon pigmented neoplasm that affects the CNS and develops in the cranial and spinal leptomeninges. Here we report on a case of malignant transformation of intracranial supratentorial meningeal melanocytoma which recurred after 3 years as malignant melanoma. This case demonstrates that the biological behavior of melanocytoma is uncertain and that these lesions may recur as malignant melanoma.

Recurrent parietal lobe supratentorial ependymoma, ZFTA fusion-positive, CNS WHO grade 3, with new dural and calvarial reactive changes in a child: illustrative case.

BACKGROUND: Supratentorial ependymoma, ZFTA fusion-positive, CNS WHO grade 3, is a rare pediatric brain tumor characterized by brisk mitotic activity and sometimes microvascular proliferation and/or necrosis. Typical treatment includes a combination of resection, chemotherapy, and radiation therapy. These tumors often present at age 3 or 4 years and have a poor prognosis. OBSERVATIONS: The authors present the case of a 4-year-old girl who presented asymptomatically with recurrence of a supratentorial ependymoma, ZFTA fusion-positive, CNS WHO grade 3, of the right parietal lobe with a homozygo

CT and MR imaging in atypical teratoid/rhabdoid tumors of the central nervous system.

INTRODUCTION: Atypical teratoid/rhabdoid tumors (ATRT) are rare aggressive neoplasms of the CNS affecting predominantly very young children. METHODS: We retrospectively reviewed the imaging findings of 9 CT and 32 MR examinations of the brain and spine of 33 children. RESULTS: Of the 33 tumors, 11 were located in the infratentorial compartment, 16 in the supratentorial compartment, 5 in both cranial compartments, and 1 in the lower thoracic spinal cord. The mean age of the children with infratentorial or infra- and supratentorial tumors was significantly lower than the mean age of the childr

Clinical characteristics, surgical management, and prognostic factors for supratentorial hemangioblastoma: A retrospective study.

BACKGROUND: Supratentorial hemangioblastoma is an extremely rare neoplasm. The aim of this study is to delineate the clinical features among cystic and solid supratentorial hemangioblastoma patients and evaluate the risk factors for progression-free survival (PFS). METHODS: We conducted a literature search in PubMed for histopathologically identified supratentorial hemangioblastoma between 1947 and 2021 and extracted and collected the clinical features of patients treated at our own institute. The rate of PFS was determined using Kaplan-Meier analysis. Differences in categorical factors, such

Closed needle biopsy in the diagnosis of intracranial mass lesions.

A retrospective study was performed on 54 patients with intracranial mass lesions who underwent closed needle biopsy using freehand and stereotactic biopsy techniques. A diagnosis of brain neoplasm was established in 44 of 54 patients, and nonneoplastic abnormalities were discovered in an additional six cases. Permanent neurologic worsening was noted in two instances following biopsy, and a single patient died as a result of the procedure. Closed needle biopsy, particulary when done stereotactically, appears to be a resonably safe and efficacious means of establishing a tissue diagnosis in car

Successful radiation therapy for supratentorial primitive neuroectodermal tumor and epidermolysis bullosa simplex.

Epidermolysis bullosa simplex (EBS) is a heritable skin disorder characterized by skin fragility and blistering. While its most severe variant, dystrophic epidermolysis bullosa (DEB) is associated with squamous cell carcinoma (SCC), the development of extracutaneous neoplasms in EBS is extremely rare. We report a novel case of supratentorial primitive neuroectodermal tumor (sPNET) in a 7-year male with EBS. Experience of radiation therapy and its challenges in children with EBS has rarely been reported.

World Health Organization Grade III Supratentorial Extraventricular Ependymomas in Adults: Case Series and Review of Treatment Modalities.

CONTEXT: Supratentorial ependymomas and their anaplastic variants are relatively uncommon central nervous system neoplasms that afflict both adults and children. AIMS: Discuss the clinical and pathological features of patients with anaplastic ependymomas involving an extraventricular supratentorial location and review modalities and options of treatment for those rare tumors. SETTINGS AND DESIGN: Whereas the treatment algorithm in the pediatric population is well established, however, treatment in the adult population is less defined. Treatment options are exposed through the author's cases

Surgical management of supratentorial ependymomas.

INTRODUCTION: Supratentorial ependymomas are rare neoplasms accounting for just ten to 15 new cases in the UK per year. This article discusses the surgical management of these tumours. MATERIALS AND METHODS: We present our experience over the past 12 years looking, in particular, at the location, histological grading, postoperative complications, survival and progression-free survival. A literature review of publications discussing the surgical management of ependymoma over the past 10 years is then presented. RESULTS: The data shows that complete surgical resection confers a significant sur

A report of a desmoplastic non-infantile ganglioglioma in a 6-year-old boy with review of the literature.

Desmoplastic infantile gangliogliomas (DIG) are uncommon supratentorial brain tumors with a usually good prognosis despite an aggressive radiological appearance that typically occurs in infants below the age of 24 months. DIGs are exclusively supratentorial, generally have a voluminous size, and are partially cystic. Total surgical removal is sufficient for the treatment of these tumors, and no chemotherapy or radiotherapy is indicated if complete resection is achieved. Except for age difference, DIG and desmoplastic non-infantile gangliogliomas are radiologically and histologically similar. N

Cerebral neoplasms in adults.

MRI plays a critical role in the diagnosis, management, and follow-up of adult supratentorial neoplasms. However, there is considerable overlap in the imaging findings of these lesions. New imaging methods, such as functional MRI, diffusion imaging, and spectroscopy may further improve diagnostic specificity and surgical management. Knowledge of the pathogenesis of these tumors, imaging characteristics, and available novel imaging tools will aid the radiologist in making meaningful contributions in the evaluation and treatment of these lesions.

Aurora kinase B is a potential therapeutic target in pediatric diffuse intrinsic pontine glioma.

Pediatric high-grade astrocytomas (HGAs) account for 15-20% of all pediatric central nervous system tumors. These neoplasms predominantly involve the supratentorial hemispheres or the pons--diffuse intrinsic pontine gliomas (DIPG). Assumptions that pediatric HGAs are biologically similar to adult HGAs have recently been challenged, and the development of effective therapeutic modalities for DIPG and supratentorial HGA hinges on a better understanding of their biological properties. Here, 20 pediatric HGAs (9 DIPGs and 11 supratentorial HGAs) were subject to gene expression profiling following

Low-grade gliomas.

Supratentorial low-grade gliomas comprise a group of primary central nervous system neoplasms with characteristics of neuroglial cells (i.e., astrocytes and oligodendrocytes) and relatively low anaplastic potential, although through time they tend to behave more aggressively. Immediate surgical intervention and subsequent radiation therapy are advocated by some workers; at present, however, it remains unclear whether early intervention prolongs survival or changes the natural history of the disease, especially in those patients who present with a seizure and a normal interictal examination.

Atypical Teratoid Rhabdoid Tumor With TTF-1 Expression: A Case Report and Possible Insight Into Its Embryologic Origin.

Atypical teratoid rhabdoid tumor (AT/RT) of the brain is a highly malignant (CNS WHO grade 4) embryonal neoplasm. Majority of AT/RTs occur in the posterior fossa although supratentorial examples may also be observed. Pathologically, the tumor typically consists of cells with rhabdoid morphology that are immunoreactive for synaptophysin, EMA, and variably for GFAP. Diagnosis is typically confirmed by observing loss of INI-1 or BRG-1 expression from the nucleus of the tumor cells. Little is known regarding the embryogenesis of AT/RT. We therefore present here a case of supratentorial AT/RT in a

C11orf95-RELA fusion present in a primary supratentorial ependymoma and recurrent sarcoma.

Ependymomas are rare glial tumors of the central nervous system that arise from the cells lining the ventricles and central canal within the spinal cord. The distribution of these tumors along the neuroaxis varies by age, most commonly involving the spinal cord in adults and the posterior fossa in children. It is becoming evident that ependymomas of infratentorial, supratentorial, and spinal cord location are genetically distinct which may explain the differences in clinical outcomes. A novel oncogenic fusion involving the C11orf95 and RELA genes was recently described in supratentorial ependy

[Alteration of the early R1-component of the blink reflex in coma: topographical and prognostic value (author's transl)].

Alteration of the early component of the blink reflex (delayed latency or abolished reflex) is considered to be relatively specific for pontine lesions (multiple sclerosis, vacular lesions of the brain-stem, neoplasms, syringes); provided, the trigeminal and facial nerve are unaffected. In comatose patients the electrically, by stimulation of the supraorbital nerve elicited blink reflex was super-imposed (50 or 100 responses) with the LAB 8/e-computer (off-line). It was found that the R1 component was abolished even in cases with supratentorial lesions and secondary brain-stem alterations, whi

Intracranial Mesenchymal Tumor with FET-CREB Gene Fusion: A Case Report of Late Recurrence.

Intracranial mesenchymal tumors with FET‑CREB fusion represent a rare tumor entity newly defined in the 5th edition of the WHO classification of central nervous system tumors. Due to overlapping morphological features, these neoplasms were previously diagnosed under different names, including intracranial angiomatoid fibrous histiocytoma and intracranial myxoid mesenchymal tumor. Clinically, they need to be differentiated from meningioma, high‑grade glioma, solitary fibrous tumor, and other intracranial neoplasms. Most supratentorial lesions achieve good outcomes after gross total resection, a

Multicentric gliomas: two case reports and a review of the literature.

Multicentric gliomas are among the rarest of multiple intracranial neoplasms. Two biopsy proven cases are described with a review of the literature. Both cases had supratentorial and posterior fossa lesions. Computed tomography was performed in both cases, open and stereotaxic biopsy was done in one case and open biopsy of both lesions in the other case. The major problem in multicentric gliomas is the management. The two cases were managed differently based on the clinical condition and the location of the lesions.

Neuropathology of pediatric brain tumors.

Pediatric central nervous system neoplasms include a spectrum of both glial and nonglial tumors that differ significantly in location and biological behavior from those of adults. Brain tumors in infants and children most often arise from central neuroepithelial tissue, whereas a significant number of adult tumors arise from central nervous system coverings (e.g., meningioma), adjacent tissue (e.g., pituitary adenoma), or metastases. Most adult brain tumors are supratentorial malignant gliomas, whereas the most common malignant pediatric brain tumor is the cerebellar primitive neuroectodermal

Intra-fourth-ventricular choroid plexus papilloma miming ependymoma.

Choroid plexus papilloma (CPP), a low incidence central nervous system (CNS) tumor, typically develops as an intraventricular neoplasm arising from the epithelium of the choroid plexus. Infratentorial CPP is predominantly clustered in adults with roughly 70% in the fourth ventricle, while supratentorial CPP commonly found in the lateral ventricles, is the most frequent location in children. The clinical and imaging features of CPP are not typical and may induce the misdiagnosis as other types of primary brain tumors. In this paper, we described a fourth-ventricular CPP that was misdiagnosed as
