Gastro-oesophageal reflux disease--experience in daily practice: symptoms and symptom assessment.

Gastro-oesophageal reflux disease is one of the most common medical problems in daily practice, with many guidelines on diagnosis and treatment available. The prevalence and incidence of reflux disease are rising. In a period of 10 years, the incidence of reflux oesophagitis has almost doubled, as has the number of pills and tablets of acid-suppressive therapy sold. The decreased number of patients with severe reflux oesophagitis is indicative of increased public awareness. Heartburn and regurgitation are the hallmarks of reflux disease. The symptom score in patients with the mild reflux oesop

A Cross-sectional Exploratory Study on Symptom Correlations in Multiple Myeloma Patients During Initial Treatment Using the Edmonton Symptom Assessment System.

The Edmonton Symptom Assessment System-Revised (ESAS-r) is a simple, reliable, patient-reported outcome measure designed to assess nine symptoms experienced by cancer patients, including multiple myeloma (MM). While each symptom can be evaluated individually, limitations in time or resources may make it difficult to assess all items in routine practice. In such cases, asking about "well-being," the simplest and most general item in the ESAS-r, may still provide clinically meaningful insight into a patient's overall condition. Although "well-being" is an essential indicator of a patient's overa…

Myeloproliferative Neoplasm Symptom Assessment Total Symptom Score (MPN-SAF TSS) in Chronic Myeloproliferative Neoplasms with Relation to Genetic Burden and Thrombosis.

The Myeloproliferative Neoplasm Symptom Assessment Total Symptom Score (MPN-SAF TSS) is a surrogate marker for symptom evaluation in chronic myeloproliferative neoplasms (MPNs). However, insufficient data are available regarding the relationship among the MPN-SAF TSS, JAK2 mutation allele burden, and thrombosis. In this retrospective analysis, we aimed to determine the genetic burdens, clinical features, and relationships with MPN-SAF TSS in MPN patients. One hundred thirty JAK2V617F-positive patients with MPNs were included in our study. We calculated the MPN-SAF TSS for all patients and comp…

Assessment of pharmacotherapy for negative symptoms of schizophrenia.

Negative symptoms in schizophrenia include diminished ability to communicate, motivate, and socialize as potentially debilitating aspects of the illness that are associated with long-term impairment. Despite such burden, the domain has been underrepresented in drug development and treatment research. In this article, we review research regarding pharmacotherapy for negative symptoms, with a focus on studies published during the past 2 years. Clearly positive studies were limited to N-methyl-D-aspartate agonists, while antipsychotics and antidepressants did not show substantial benefit, and cog

Psychotic symptoms in the elderly--assessment and management.

BACKGROUND: Psychotic symptoms in the elderly arise in mood disorders, schizophrenia, dementia, and delirium. OBJECTIVE: This article provides a brief overview of the presentation and management of each condition. DISCUSSION: Management follows logically from a thorough mental state examination, medical work up and accurate diagnosis. Depression and mania with psychotic symptoms respond best to an antidepressant and mood stabiliser respectively, usually in combination with antipsychotic medication. Severe cases require electroconvulsive therapy. Atypical antipsychotics are now the treatment

Bladder-outlet obstruction--assessment of symptoms.

Assessment of symptoms plays a central role in the evaluation of men with suspected bladder-outflow obstruction (BOO) secondary to benign prostatic hyperplasia (BPH). It is on the basis of symptoms that treatment designed to relieve outflow obstruction is recommended. However, a variety of studies have suggested that a considerable proportion of men presenting with suspected outflow obstruction are not obstructed as determined by pressure-flow criteria. The relationship between many so-called "obstructive" symptoms and BOO has not yet been defined. Furthermore, all previous studies have define

Symptom rating scale for assessing hyperthyroidism.

A hyperthyroid symptom scale (HSS) was designed and administered to ten subjects with untreated Graves' disease. All subjects had clinical and chemical evidence of hyperthyroidism and reproducible HSS scores of 20 or more points. During sequential treatments with propranolol hydrochloride (phase 2) followed by propylthiouracil (phase 3) there was a significant decline in the HSS scores at each phase. Accompanying the decrease in HSS scores was a decrease in heart rate, but there was no change in thyroid function test results at phase 2 and a decrease in heart rate, thyroid function test result

Methods for assessing positive and negative symptoms.

Once adequate reliability has been achieved, however, it is important to document internal consistency and external validity. This chapter has summarized some of our work with the former topic. It suggests that the SANS has high internal consistency, while the SAPS is somewhat less internally consistent. What are the clinical and research implications of these results concerning internal consistency? On a superficial and statistical level, these results might be considered to suggest that the SAPS is less valid than the SANS, and perhaps that therefore it is not useful. On the other hand, it m

Validation of the gastrointestinal symptom score for the assessment of symptoms in patients with functional dyspepsia.

AIM: To validate the gastrointestinal symptom score as an outcome measure for functional dyspepsia. METHODS: In focus groups, 10 dyspepsia-specific items including nausea, sickness, vomiting, bloating, abdominal cramps, early satiety, acidic eructation/heartburn, loss of appetite, retrosternal discomfort, epigastric pain/upper abdominal pain were identified. Ninety-five patients with functional dyspepsia and 56 healthy controls were recruited and responsiveness evaluated by analysing gastrointestinal symptom score data from 357 patients from previous placebo-controlled trials. Validation of the Head and Neck Patient Symptom Checklist as a nutrition impact symptom assessment tool for head and neck cancer patients.

PURPOSE: The purpose of this study was to test the validity of the Head and Neck Patient Symptom Checklist (HNSC). METHODS: Three hundred and sixty-eight treatment-naive individuals with head and neck cancer prospectively completed the HNSC and the Patient-Generated Symptom Global Assessment (PG-SGA). The predictive validity was determined by comparing the HNSC symptoms interference scores to the PG-SGA scores. Multivariate logistic regression was used to determine the HNSC symptoms scores associated with reduced dietary intake, ≥ 5 % weight loss over 6 months, and reduced functional performa

Assessing symptom distress in cancer patients: the M.D. Anderson Symptom Inventory.

BACKGROUND: The purpose of this project was to develop the M. D. Anderson Symptom Inventory (MDASI), a brief measure of the severity and impact of cancer-related symptoms. METHODS: A list of symptoms was generated from symptom inventories and by panels of clinicians. Twenty-six symptoms and 6 interference items were rated by a validation sample of 527 outpatients, a sample of 30 inpatients from the blood and bone marrow transplantation service, and a cross-validation sample of 113 outpatients. Clinical judgment and statistical techniques were used to reduce the number of symptoms. Reliability

A New Questionnaire to Assess Respiratory Symptoms (The Respiratory Symptom Experience Scale): Quantitative Psychometric Assessment and Validation Study.

BACKGROUND: Smokers often experience respiratory symptoms (eg, morning cough), and those who stop smoking, including those who do so by switching completely to electronic nicotine delivery systems (ENDS), may experience reductions in symptoms. Existing respiratory symptom questionnaires may not be suitable for studying these changes, as they are intended for patient populations, such as those with chronic obstructive pulmonary disease (COPD). OBJECTIVE: This study aimed to develop a respiratory symptom questionnaire appropriate for current smokers and for assessing changes when smokers stop s

Assessment of lipedema awareness and symptom levels in women: A cross-sectional study.

This cross-sectional and descriptive study was conducted with 395 women to assess lipedema awareness, lipedema symptom levels, and the screening probability of lipedema. Data were collected using an Introduction Form, and the Questionnaire and Screening Model for Lipedema. The mean score on the lipedema screening questionnaire was 6.05 ± 4.58. It was determined that 15.4% of women had a high probability of developing lipedema. We found that participants demonstrated awareness regarding lipedema and that sociodemographic characteristics were associated with lipedema risk.

Real-world performance evaluation of a telemonitoring system for Parkinson's disease symptom assessment.

BACKGROUND: Parkinson's disease (PD) nurses constitute a reliable modality for appraising the motor status of patients, second only to expert neurologists. Consequently, employing PD nurses for the assessment of medical device performance is appreciated to be a fundamental benchmark in the field. OBJECTIVE: To evaluate the precision of a telemonitoring device in measuring motor symptoms when utilized at patient's home during everyday life conditions. METHODS: To ascertain the extent of correlation between device-reported outcomes and PD nurse assessments and the precision with which the devi

Diagnosis of Sports-Related Concussion Using Symptom Report or Standardized Assessment of Concussion.

IMPORTANCE: The Sports Concussion Assessment Tool-5 (SCAT5) has been recommended for concussion evaluation and utilizes both a subjective reported symptom grading scale and objective measures of concussion including a cognitive evaluation: the Standardized Assessment of Concussion (SAC). The SAC includes testing for orientation, immediate memory, concentration, and delayed recall; a 10-word list is used to assess immediate memory and delayed recall. OBJECTIVE: To determine the diagnostic accuracy of components of the SCAT5 and to provide a framework for clinical interpretation. DESIGN, SETTI

Time Trends in Concussion Symptom Presentation and Assessment Methods in High School Athletes.

BACKGROUND: Concussion rates have increased significantly over the past decade. This may reflect an increase in the knowledge and diagnosis of the symptoms of a concussion rather than a true increase in the incidence. Assessing trends in the way that concussions are presenting to and being identified by clinicians over the same period may provide additional insight into the apparent rise in concussions. PURPOSE: To evaluate patterns of change in concussion symptom presentation, diagnostic/evaluation methods, and symptom resolution time reported for United States high school athletes from the

Clinical Insight Into Latent Variables of Psychiatric Questionnaires for Mood Symptom Self-Assessment.

BACKGROUND: We recently described a new questionnaire to monitor mood called mood zoom (MZ). MZ comprises 6 items assessing mood symptoms on a 7-point Likert scale; we had previously used standard principal component analysis (PCA) to tentatively understand its properties, but the presence of multiple nonzero loadings obstructed the interpretation of its latent variables. OBJECTIVE: The aim of this study was to rigorously investigate the internal properties and latent variables of MZ using an algorithmic approach which may lead to more interpretable results than PCA. Additionally, we explored

Psychometric Validation of the Chinese Version of Gambling Symptom Assessment Scale (C-GSAS).

Reliable tools for assessing gambling symptom severity in gambling disorder (GD) are critical for both research and clinical intervention. The Gambling Symptom Assessment Scale (GSAS) is widely used but lacks validation in Chinese-speaking populations. Our study aimed to evaluate the psychometric properties of the Chinese version of GSAS (C-GSAS) among 186 treatment-seeking GD patients. Internal consistency was excellent (Cronbach's α = 0.93), and exploratory factor analysis revealed a two-factor model: Cognitive-Preparatory and Emotional-Consequential Factors. Confirmatory factor analysis sup…

Psychometric properties of a daily obsessive-compulsive symptom scale for ecological momentary assessment.

Despite growing interest in ecological momentary assessment (EMA) in psychopathology and clinical observation of day-to-day fluctuations in obsessive-compulsive disorder (OCD) symptoms, there is not a standardized EMA measure of such symptoms that can guide systematic research. In the absence of such a measure, prior EMA research in OCD has utilized heterogeneous approaches to sampling momentary and daily OCD symptoms, which limits the ability to compare results between studies. The present study sought to examine the psychometric properties of a daily OCD symptom (d-OCS) measure that assesses
