New placement of recording electrodes on the thyroid cartilage in intra-operative neuromonitoring during thyroid surgery.

OBJECTIVE: During thyroid surgery, extreme caution is needed not to harm the recurrent laryngeal nerve and to avoid vocal cord palsy. Intra-operative neuromonitoring became increasingly popular as an adjunct to the gold standard of visual identification of the recurrent laryngeal nerve (RLN). Electromyographic (EMG) responses are normally recorded by electrodes attached to the endotracheal tube. Alteration in position can lead to false loss of signal. We developed thyroid cartilage electrodes that can be fixed directly onto the thyroid cartilage. STUDY DESIGN: Prospective clinical cohort METH…

Experimental study of needle recording electrodes placed on the thyroid cartilage for neuromonitoring during thyroid surgery.

BACKGROUND: Needle electrodes placed on the thyroid cartilage (TC) are an alternative to endotracheal tube (ET) electrodes for assessing recurrent laryngeal nerve (RLN) function during thyroid surgery. Needle electrodes placed on the TC were evaluated in an experimental porcine model. METHODS: Continuous intraoperative neuromonitoring was used to record the electromyogram. Each TC side was delineated into nine areas to determine the optimal placement of the electrode, and needle electrode area, depth and orientation for optimal electromyographic (EMG) amplitudes were evaluated. RLN root locat…

Biomechanical study of the thyroid cartilage: A model of bi-digital strangulation.

The presence of fracture on neck elements is an indication of violence. Both the hyoid bone and the larynx can be damaged by a strangulation mechanism. Thyroid cartilage, more specifically, may present lesions in response to this mechanical stress. These lesions result in fractures at the bases of the horns of the thyroid cartilage. This study focuses on the thyroid cartilage behavior in cases of bi-digital strangulation, using an anthropometric and biomechanical approach. To develop a biomechanical model, we performed an anthropometric study taking into account 14 distances measurements as we…

How Age and Frequency Impact the Thyroid Cartilages of Professional Singers.

OBJECTIVES/HYPOTHESIS: Young professional singers can easily reach very high pitches. In contrast, older singers often complain that they ha … tly depends on pitch and significantly correlates with age (r2 = 0.7, P < 0.001). CONCLUSIONS: The thyroid cartilage is flexible and its formability is especially important during singing. At higher pitches, the cartilage was more deformed. The larynx in older singers showed less thyroid cartilage deformation.

The investigations of mechanical properties of ossificating thyroid cartilages of the human larynx.

40 thyroid cartilages were used to investigations, they were taken from bodies of people, died between the ages of 19-99, by the use of the endurance machine with controlled speed of the increase of deformation. It was observed that the durability for flexion and the inflexibility of the investigated cartilages depends on the level of the process of their ossification, that is estimated by the help of the radiological index of the ossification (RIO). On the other hand, the searches of durability forcing of the regions of cartilages that presented different levels of the ossification process, p…

Biomechanics of a previously unknown thyroid cartilage fracture in fatal strangulation.

After the death of a 63-year-old woman following pressure against the neck, the injury was assessed forensically and a radiological-preparatory examination of the osseous structure of the larynx was conducted. We used fine preparation and, for further characterization of the fracture, radiological imaging in spiral CT, 3D reconstruction and fine-focus technique (mammography). While a skeletal injury with the basal fractures of the upper horns of the thyroid cartilage was clearly visible in the CT and 3D reconstruction, the radiological visualization of a 3-4mm wide wall-penetrating dehiscence …

Laryngeal muscle responses to mechanical displacement of the thyroid cartilage in humans.

Speakers may use laryngeal sensory feedback to adjust vocal fold tension and length before initiating voice. The mechanism for accurately initiating voice at an intended pitch is unknown, given the absence of laryngeal muscle spindles in animals and conflicting findings regarding their existence in humans. Previous reports of rapid changes in voice fundamental frequency following thyroid cartilage displacement suggest that changes in vocal fold length modulate laryngeal muscle contraction in humans. We tested the hypothesis that voice changes resulting from mechanical perturbation are due to r…

Non-Traumatic Thyroid Cartilage Fracture from Sudden Neck Motion: Clinical Insights.

BACKGROUND The thyroid cartilage is the most prominent laryngeal cartilage located beneath the hyoid bone. Non-traumatic fractures of the larynx, including the thyroid cartilage, are rare and can occur when the glottis is closed and intrathoracic pressure suddenly increases. This report describes the case of a 42-year-old man presenting with hoarseness and pain when swallowing and a diagnosis of non-traumatic thyroid cartilage fracture following sudden neck movement, highlighting the importance of recognizing atypical mechanisms of laryngeal injury. CASE REPORT A 42-year-old healthy man experi…

Thyroid cartilage needle vs. endotracheal tube electrodes in EBSLN monitoring during thyroidectomy.

During thyroidectomy, both needle electrodes (NE) and endotracheal tube electrodes (ETE) can be used. Incomplete contact of the endotracheal tube electrode with the vocal cords, endotracheal tube electrode may hinder an optimal outcome and even result in an inability to obtain an electromyography wave while neuromonitoring the external branch of the superior laryngeal nerve (EBSLN). There is no study that compares NE and ETE for EBSLN monitoring. Therefore, this study compares NE and ETE recordings during EBSLN monitoring. Twenty-six consecutive patients undergoing total thyroidectomy were inc

Giant cell tumor of thyroid cartilage: A diagnostic dilemma on aspiration cytology.

Giant cell tumor of the larynx is an uncommon entity with only 44 cases reported in the literature. These tumors occur most commonly in the epiphysis of the long bones of female patients in third decade. Here in we report a case of 23 years old male patient who presented with an anterior neck swelling since past 4 months. Ultrasound and computed tomography of neck revealed a heterogenously enhancing lesion involving posteroinferior half of right thyroid cartilage with extension into the extra laryngeal strap muscle and intralaryngeal right true vocal cord and false vocal cord. The findings wer…
