[Open diaphyseal tibial fractures in children].

In a collective study of 22 surgical clinics 138 open diaphyseal tibial fractures in children from 0 to 14 years were examined. The incidence of open fractures, in relation to all tibial shaft fractures comes up to 6.9%, one third (2.3%) are second degree and third degree open fractures. 80% of these fractures are caused by traffic accidents. In 33% there exist multiple fragments. In first degree open fractures conservative treatment generally is practiced, while in second and third degree open fractures osteosynthesis often is performed.

Tibial tuberosity fracture as a complication of tibial tuberosity advancement.

OBJECTIVES: To retrospectively compare two clinical subsets of dogs suffering tibial tuberosity (TT) fracture (incidental finding or sudden onset severe lameness) as a complication of tibial tuberosity advancement (TTA) and to report the surgical management and outcome of TT fracture as a complication of TTA. MATERIAL AND METHODS: The medical records of 10 dogs with eleven TT fractures or crest fractures after TTA were reviewed. The outcome and complications were determined from clinical and radiographic follow-up examinations. Limb function was evaluated between six and 12 weeks postoperativ

The Tibial Plateau Map: Fracture Line Morphology of Intra-Articular Proximal Tibial Fractures.

The purpose of this study was to characterize the patterns of a large series of tibial plateau fractures with the use of fracture mapping, with regard to different fracture types using the OTA/AO and Schatzker classification. Patients with intra-articular fractures of the tibial plateau were evaluated, using the OTA/AO and Schatzker classification on CT scans. For fracture mapping, the axial slice that completely displayed the tibial joint plane was first identified, then matched to a template congruently, and the fracture lines were identified and reproduced. In addition to epidemiological da

Longitudinal tibial stress fracture.

Fatigue-type stress fractures occur following repetitive loading of normal bone. These occur frequently in the tibia, although vertical orientation to the fracture is much less common than transverse orientation. Without a convincing history of new or accelerated muscular activity, imaging can be difficult to interpret and evaluation may require more than one imaging modality to exclude other diagnostic considerations, including neoplasm and osteomyelitis.

The camel jockey's tibial fracture.

An isolated fracture of the middle and lower third of the tibia occurred with marked regularity in a large number of camel-racing jockeys. The mechanism of this fracture was analysed. It was concluded that the compression-rotation of the leg below the knee between the trunks of adjacent racing camels produced this injury, but the sparing of the fibula could not be explained. Simple measures are described such that the incidence of these and other injuries sustained by the jockeys can be markedly reduced.

Bilateral fracture of the tibial spine.

Fracture of the intercondylar eminence of the tibia in childhood is a relatively common injury, but to our knowledge a bilateral case has never been reported. A patient with bilateral fractures is presented and the literature is reviewed. An 8-year-old girl fell from a tree and sustained bilateral fractures of the tibial spine. According to Meyers' and McKeever's classification normal x rays revealed a type III fracture in the left knee and a type II fracture in the right knee. The left knee was treated by open reduction and internal fixation, and the right knee was treated conservatively. At

Fracture of the tibial tuberosity in adolescents.

Fracture of the tibial tuberosity is an uncommon injury and rarely occurs after closure of the proximal tibial epiphysis. Three cases of tibial tuberosity fracture over a period of 11 years were treated by open reduction, internal fixation, and immobilization for 6 weeks in plaster of Paris (1970-1980). The postoperative follow-up times were 5, 6, and 7 years. No subsequent deformities were detected. Two patients had secondary posttraumatic patella infera without any symptoms. All patients returned to normal activity. The results demonstrated that this injury in the adolescent knee heals witho

Functional impact of tibial malrotation following intramedullary nailing of tibial shaft fractures.

BACKGROUND: Tibial malrotation is a complication that is seen in approximately 30% of patients following locked intramedullary nailing. In this cohort study, we evaluated the hypothesis that tibial malrotation would lead to impaired functional outcomes. METHODS: Patients with a unilateral tibial shaft fracture who were managed with intramedullary nailing between 2003 and 2007 were identified with use of ICD-10 (International Classification of Diseases, 10th Revision) codes. After institutional review board approval and written informed consent had been obtained, specific assessment of eligibl

[The unreamed tibial intramedullary nail in treatment of tibial fractures--initial experiences].

The unreamed tibial nail (UTN) combines the advantages of the external fixator (preservation of the cortical blood supply) with that of conventional intramedullary nailing (closed system, high patient comfort, no pin problems) in the treatment of the lower leg fracture [5]. Within 1 year (III/92 to III/93) 31 closed and 2 open fractures were stabilised with UTN. In 67% (22 persons) we found fractures of the A1, A2 and A3 types according to the AO classification. In 6 patients there was a combined lesion with involvement of the ankle joint. A follow-up of 27 patients was done, extending to an a

Classification of fractures of the tibial condyles.

We analysed 131 fractures of the tibial condyles in 130 patients, using a modification of the classification of Schatzker, McBroom and Bruce (1979). The patients were reviewed at an average of 7.6 years after the injury. Fifty-five (42%) fractures had been treated conservatively and 76 (58%) operatively. Medial unicondylar and medially tilted bicondylar fractures tended to redisplace into varus position and lateral unicondylar and laterally tilted bicondylar fractures into valgus. There were significant differences when the results were evaluated according to the methods of Hohl and Luck (1956

[Multiple recurrence of tibial fracture].

After a historical review of the term refracture and the related nomenclature, three cases of multiple refracture of the tibia are presented. Radiological findings confirm that the major etiology in refracture is bone necrosis as a result of vascular damage caused by the initial trauma and/or the surgical maneuvers performed to treat it. Normal loads then lead to microfractures of the avascular bone, which is unable to provide adequate substitution or remodelling. In our three cases not only was the periosteal blood supply diminished, but also the medullary canal was narrowed or closed at the

Fractures of the tibial tuberosity in adolescents.

The cases of fourteen adolescents with fifteen physeal fractures of the tibial tuberosity were reviewed to more accurately define specific fracture patterns, to establish treatment for the different types, and to determine the incidence of complications. A modified classification scheme with a greater emphasis on intra-articular extension of the fracture and communution of the tuberosity was devised. Closed or open reduction, as necessary, gave satisfactory results. The primary indications for surgery were: (1) displacement of one or more fragments of the tuberosity anterosuperiorly, and (2) e

Morphology of occult posterior malleolus fractures associated with tibial shaft fractures.

AIMS: Occult posterior malleolar fractures (PMFs) associated with tibial shaft fractures are thought to occur most commonly in spiral fracture types of the tibia. We hypothesize that tibial diaphyseal fracture patterns would be associated with certain PMFs, highlighting the pathomechanics of the injury. METHODS: A retrospective review was performed on data collected between 1 January 2013 and 9 November 2020. The inclusion criteria were patients aged over 16 years with a diaphyseal tibial fracture who had undergone a CT of the affected lower limb. The Mason and Molloy posterior malleolar frac

Delayed union of fibular fractures accompanying fractures of the tibial shaft.

Among 440 adult patients with tibial shaft fracture and accompanying fibular fracture there were eight cases with radiographically ununited fibulae 4 months after the injury, each with uneventful tibial union. Fractures with severe soft-tissue injuries were excluded from this study. In 293 patients the treatment method of the tibial fracture was conservative, comprising closed reduction and immobilization by long plaster cast. In 147 patients it was intramedullary Küntscher nailing, and all the eight cases with delayed fibular union occurred among these, the frequency being 5.4%. The typical a

Tibial shaft fractures treated with functional braces. Experience with 780 fractures.

We have reviewed our recent results with functional bracing of tibial shaft fractures in adults in order to define its role in management. We also analysed several parameters of these fractures to discover those which influence healing. A total of 780 tibial fractures treated in prefabricated functional braces were followed to union; shortening of less than 10 mm and angulation of less than 5 degrees in any plane were our parameters for successful treatment. The average time before applying a brace was 3.8 weeks for closed fractures and 5.2 weeks for open ones. Closed fractures healed in an av

Distal metaphyseal horizontal tibial fractures in children.

BACKGROUND: Pediatric horizontal distal metaphyseal tibial fractures are relatively uncommon injuries. While nonoperative treatment remains the standard of care for most cases, certain fracture patterns may carry a higher risk of treatment failure, and optimal treatment strategies are still unclear. The aim of this study was to investigate the frequency, anatomical location of these fractures within the metaphysis, and risk factors associated with treatment failure in nonoperative management. METHODS: A retrospective, registry-based cohort study was conducted to evaluate treatment success in

Arthroscopic suture fixation of tibial spine fractures.

A tibial spine fracture refers to an intraarticular fracture of the osseous insertion of the anterior cruciate ligament at the proximal tibia, commonly seen in pediatric and adolescent patients. This fracture is classified based on the degree of displacement and the presence or absence of an intact posterior hinge point. For significantly displaced fractures, surgical reduction and fixation are often recommended. Both open and arthroscopic approaches have been described. This technical note describes our technique for arthroscopic-assisted reduction and fixation of tibial spine fractures using
