Horizontal root fractures in posterior teeth without dental trauma: tooth/root distribution and clinical characteristics.

AIM: To describe the clinical characteristics and radiographic findings of horizontal root fractures (HRF) in posterior teeth without a history of dental trauma. METHODOLOGY: A total 24 patients and 31 HRF cases in 28 posterior teeth were collected from 2006 to 2015. Clinical examinations and radiographic imaging were evaluated. Value of confidence intervals of the proportions was calculated for data presentation. RESULTS: The number of males (54%) was similar to females (46%). The patients were predominantly between 50 and 70 years of age (75%). Most HRF cases were found in nonendodonticall

Fate of developing tooth buds located in relation to mandibular fractures in three infancy cases.

The fate of developing tooth buds located in relation to mandibular fractures was investigated in three infancy cases. Three infants, 2 girls and a boy, aged from 1 year and 5-months old to 2 years and 6-months old, were treated for dislocated mandibular fracture in the symphyseal region by manual reduction and fixation with a thermoforming splint and circumferential wiring under general anesthesia. Fracture healing was uneventful in all cases. A few years later, no obvious deformity of the jaw or malocclusion was observed; however, malformation of the crown was found in one of the permanent t

Fractures of the mandible involving the impacted third molar tooth: an analysis of 47 cases.

An attempt is made to correlate mesio-angular, horizontal, disto-angular and vertical tooth impaction with the incidence, displacement and management of fractures in the region of the mandibular third molar. Evidence was taken principally from a radiographic study of 47 cases of fracture of the mandible at this site. No particular single mandibular third molar angulation showed itself more commonly in this series of fractures. However, fractures involving unerupted mesio-angular third molard tended to remain undisplaced and could, therefore, be treated by simpler measures.

A Case of Anterior Single Tooth Implant with Fractured Zirconia Abutment due to Trauma.

In recent years, a wide variety of materials have been used in dental implant treatment. In selecting the superstructures and abutments to be used it is important to consider their potential effect on the stability and durability of the planned implant. Excessive force applied to an implant during maintenance commonly results in complications, such as fracture of the superstructure or abutment, and loosening or fracture of the screws. This report describes a case of implant treatment for a 23-year-old man with esthetic disturbance due to trauma to the maxillary anterior teeth. The left maxilla

Orbital Apex Syndrome Causing Blindness Secondary to a Vertical Root Fracture of a Tooth.

The authors report a case of a 35-year-old man who presented with left maxillary toothache associated with left sided facial pain and a rapidly progressive loss of vision in the left eye. Clinical and radiological assessments revealed it to be an odontogenic orbital apex syndrome secondary to a vertical root fracture of a tooth. The infection was treated and controlled, postoperative follow-up showed no recurrent inflammation and the patient recovered well. However, the vision was lost permanently. Odontogenic orbital apex syndrome is a rare, aggressive disease. Once the infection spreads, it

Autogenous transplantation of mandibular third molar to replace tooth with vertical root fracture.

Autogenous tooth transplantation (ATT) can be considered when there is a hopeless molar tooth and suitable donor present. This report presents an unconventional case of successful ATT of a third molar replacing the adjacent fractured second molar in a 33 year old woman. This wisdom tooth had completely developed roots. Root-end filling with Calcium Enriched Mixture (CEM) cement was performed in the third molar. The second molar was extracted non-traumatically without any bone removal; the wisdom tooth was immediately transplanted into the recipient socket. No endodontic treatment was carried o

[Root fracture and tooth luxation. Endo-orthodontic technic after 15 years].

The authors described a personal treatment method for the traumatic root fractures in the permanent incisors, and the review of their results after 15 years. The method consists in the good immobilisation and fixation of the injured teeth, with orthodontic bands and brackets and arch wire. They put this orthodontic contention for one-two months, and after x-ray control and pulp vitality checks, removed it. They presented also the use of the same technique in the luxations of permanent incisors, and compared the results. In the root fractures the authors obtain good long distance results, such

Load capacity and fracture modes of instrumented tooth roots under axial compression.

OBJECTIVE: To investigate the influences of root canal instrumentation on the load capacity and fracture modes of tooth roots under axial compression by performing mechanical tests and finite element analysis (FEA). METHODS: Thirty bovine incisor roots were trimmed into cylinders of 5.0 mm diameter. They were randomly divided into two groups, one with root canals instrumented to ∼2.0 mm in diameter, and one without instrumentation. The specimens were fractured under uniaxial compression at a crosshead speed of 0.2 mm/min, and then micro-CT was used to reveal the fracture patterns in three dim

Management of an Unsuccessful Regenerative Endodontic Procedure after Tooth Fracture: A Case Report.

Dental trauma is a very frequent occurrence in children and adolescents, which creates a great impact on the esthetics, functions, and phonetics. Traumatic dental injuries can range from simple enamel fractures to permanent tooth loss. This case report presents an eight-year-old patient with an uncomplicated crown fracture of tooth 21, and 30 days after trauma, it was diagnosed as necrotic pulp. The first treatment choice was a regenerative endodontic procedure (REP), however, the failure led to apexification with Mineral Trioxide Aggregate (MTA). The chosen rehabilitation was a composite vene

A short period of maxillomandibular fixation for treatment of fractures of the mandibular tooth-bearing area.

PURPOSE: This study was aimed to determine whether a short period of maxillomandibular fixation (MMF) followed by an arch bar splint wired to the lower jaw is a suitable alternative to conventional MMF for treatment of fractures of the mandibular tooth-bearing area. PATIENTS AND METHODS: Thirty patients with mandibular fractures associated with no other facial fractures were selected. They were randomly assigned into 2 groups for treatment with conventional MMF (group A) and MMF for a short period of 2 weeks followed by an arch bar splint wired to the lower jaw (group B). Complications were r

Partial pulpotomy and tooth reconstruction of a crown-fractured permanent incisor: a case report.

Pulp exposure due to traumatic injuries in the anterior permanent teeth of adolescents is a common occurrence. A vital permanent maxillary incisor with complex crown fracture and pulp exposure was treated by partial pulpotomy and assessed clinically through pulpal sensitivity tests and radiographically for periapical healing. Partial pulpotomy consisted of pulp tissue removal to a depth of only 1 to 2 mm, then capping the pulpal wound with calcium hydroxide, lining with resin-modified glass-ionomer cement, and restoration with resin composite. At each recall (7, 15, 21, 50, 90, 150, and 250 da

[Resin short post retention for reattachment of a fractured crown in a young permanent tooth with complicated crown-root fracture and root fracture: a case report].

This report presents a case of a 9-year-old child with a complicated crown-root fracture of the maxillary central incisor, accompanied with a root fracture. The treatment strategy was minimally invasive, focusing on vital pulp preservation, root fracture recovery, and crown restoration. The fractured crown was reattached using resin short posts to enhance retention, resulting in aesthetic and functional restoration of the anterior teeth. A 2-year follow-up revealed favorable clinical and radiographic outcomes.

Dental implants significantly increase adjacent tooth loss risk due to root fracture.

DESIGN: This retrospective cohort study aimed to investigate the risk and variables of tooth loss for teeth adjacent to dental implants compared to teeth nonadjacent to implants. The study followed the STROBE guidelines and was approved by the Institutional Review Board. COHORT SELECTION: The study included patients treated with dental implants at UCSF School of Dentistry between 2000 and 2020. The inclusion criteria for teeth adjacent to implants required the implant to support a fixed prosthesis and a follow-up period of at least 12 months. Nonadjacent teeth also required a follow-up period

Co-Influence of Restoration Bonding and Inlay Cavity Design on Fracture Load of Restored Tooth.

INTRODUCTION: The aim of this study was to investigate the co-influence of indirect mesio- occlusal-distal (MOD) cavity geometry and inlay restoration bonding on quasi-static fracture load of the restored tooth. METHODS: Forty-eight intact human molar teeth were selected and prepared for standardized edge-shaped or round-shaped MOD cavities. The resin composite (Cerasmart, GC) inlays were bonded with the state-of-the-art inlay bonding protocol or with intentionally deteriorated bonding using n-hexane-wax solution for preconditioning. Restored teeth were loaded along the long axis of the tooth

Fracture load and microcrack comparison of crowns manufactured from tooth-shaped and traditional blocks.

This study intended to analyze microcracks and fractographic markings on the surface of all ceramic crowns after milling and compare the fracture loads. 90 crowns were manufactured from two feldspathic (Priticrown-Pr and Vita Mark II-Vi) and a lithium disilicate (EmaxCAD-Em) blocks (n = 30). Two groups (n = 15) were prepared for each ceramic. In the first group, crowns were analyzed twice via the fluorescent penetrant method for microcrack detection, after the manufacturing process and thermal cycles. The load to fracture test was applied at a crosshead speed of 0.5 mm/min until catastrophic f

Atypical radiographic presentation of a horizontal mid-root fracture in a maxillary central incisor tooth.

Prognosis of a horizontal mid-root fracture is favorable, primarily because the dental pulp tends to maintain its vitality and the fracture segments are completely intraalveolar. Healing usually occurs with deposition of calcified tissue. However, if the segment coronal to the fracture becomes nonvital and infected, healing occurs by interposition of granulation tissue. This report describes a case of a horizontal mid-root fracture in a right maxillary central incisor tooth, where the apical fractured segment was significantly displaced in a linear direction. This was attributed to the pressur

Influence of Tooth Orientation on the Detection of Vertical Root Fracture in Cone-beam Computed Tomography.

INTRODUCTION: The purpose of this study was to assess the influence of tooth orientation in relation to the projection plane of the x-rays on the detection of vertical root fracture (VRF) with different filling materials using cone-beam computed tomographic (CBCT) imaging. METHODS: Thirty single-rooted human teeth were endodontically instrumented, and VRF was induced in half of the sample. The roots were individually placed in the dental socket of a phantom head composed of a dry human skull and mandible, and CBCT images were obtained of each root with the longitudinal axis in 2 orientations:

Osteomyelitis of the jaw (with pathological fracture) following extraction of an impacted wisdom tooth. A case report.

Osteomyelitis is an infection and inflammation associated with the bone structures: bone marrow, cortical bone, periosteum, blood vessels and nerves. Although it does not have a frequent presentation, it can sometimes lead to complications such as pathological fractures or even septicemia. We present a clinical case study, to assess the relationship between osteomyelitis of the jaw and pathological fractures, after the extraction of an impacted wisdom tooth. This case highlights a rare complication following the surgical removal of mandibular third molar; a slow evolution of the pathology foll

Influence of a parafunctional oral habit on root fracture development after trauma to an immature tooth.

Root fractures in immature teeth are rare because the resilience of the alveolar bone is more favorable to the occurrence of luxation. This article reports a case of traumatic injury in an immature permanent tooth that progressed to root fracture, having a parafunctional oral habit as the possible modifying factor of case evolution. A 12-year-old boy presented for treatment complaining of a defective restoration and mild pain on the maxillary right central incisor. The patient had a history of crown fracture in this tooth due to trauma 2 years before. The clinical examination showed healthy gi
