Advances in vaccine development for human trichuriasis.

Trichuriasis known as whipworm infection caused by Trichuris trichiura, is a highly prevalent soil-transmitted helminthiasis in low- and middle-income countries located in tropical and subtropical areas and affecting approximately 360 million people. Children typically harbour the largest burden of T. trichiura and they are usually co-infected with other soil-transmitted helminth (STH), including Ascaris lumbricoides and hookworm. The consequences of trichuriasis, such as malnutrition and physical and cognitive growth restriction, lead to a massive health burden in endemic regions. Despite the…

A case of gastroenteritis associated with gastric trichuriasis.

A rare human case of gastroenteritis and eosinophilic ascites associated with gastric trichuriasis is described. The patient was a 32-yr-old woman who was working in a farm near Pohang, Korea. She complained of abdominal pain, diarrhea, and vomiting. Endoscopic examination found focal linear hyperemia on the mucosa of the stomach antrum, and endoscopic biopsy confirmed eosinophilic inflammation of the mucosa and submucosa of the stomach, terminal ileum, and cecum. The biopsy specimen of the stomach included a female Trichuris trichiura which was covered by many inflammatory cells on its surfac…

Trichuriasis: localized inflammatory responses in the colon.

Most patients with trichuriasis have light worm burdens. Data regarding the inflammatory response to Trichuris worms in the colon of lightly infected persons are scant. Nine patients whose Trichuris infection was found by colonoscopy had biopsies taken from a site adjacent to visible worms and from a second site some 20 cm distally. The biopsies were studied by routine and immunohistochemical methods. None of the biopsies showed mucosal ulceration, significant congestion, fibrosis, gland distortion or goblet cell mucin depletion. There was no difference between worm and worm-free sites in term…

Is trichuriasis associated with iron deficiency anemia?

We performed quantitative fecal examinations, hemograms, and serum iron determinations on 103 first-grade children from Vieques Island, Puerto Rico, to determine whether trichuriasis was associated with iron deficiency and anemia. Although hemoglobin values tended to be slightly lower in Trichuris-infected children, there was no association between trichuriasis and serum iron or transferrin saturation values. These data demonstrate that in lightly infected children such as the population studied trichuriasis is not associated with iron deficiency anemia.

Treatment of trichuriasis with a new drug, mebendazole.

Effectiveness of mebendazole, a new anthelminthic drug recommended for treatment of Trichuris trichiura infection, was evaluated in 50 children attending a pediatric outpatient clinic in New York City. Mebendazole, 100 mg. administered orally twice daily for three days resulted in an apparent cure of 48 children. Posttreatment stools of the remaining two children had a substantial reduction in the number of eggs. A second course of treatment, identical to the first, led to the elimination of eggs in the stool specimens of these two children. Mebendazole appears to be an effective drug against

Helper T-cell subsets in mouse trichuriasis.

The fate of a primary infection with the nematode parasite Trichuris muris varies between different inbred strains of mouse. Most strains are resistant to infection, expelling the parasite before the adult worms develop. However, a few strains are unable to mount a protective immune response and will harbour chronic infections. Mouse trichuriasis thus provides on excellent helminth model in which to dissect the mechanisms of immunity operating during acute versus chronic infection. Here, Kathryn Else and Richard Grencis describe their recent work on the polarization of the helper T-cell respon…

A clinical evaluation of the efficacy of mebendazole in the treatment of trichuriasis.

In a single-blind clinical study of mebendazole, a new, broadspectrum anthelmintic, 70 patients with light to severe infections of Trichuris trichiura were treated. Ten subjects received placebos, and 60 received 100 mg mebendazole twice a day for three and four days. The drug was well tolerated by the patients, and no toxic side effects were observed. Egg count comparisons using the McMaster method on feces collected before and one month after treatment indicated a cure rate for trichuriasis of 65.6% and 89.3% and a total egg reduction of 69.5% and 95.9% for three- and four-day regimens, resp…
