Trimethoprim-Sulfamethoxazole for Pediatric Osteoarticular Infections.

BACKGROUND: Trimethoprim-sulfamethoxazole (TMP-SMX) is active against most Staphylococcus aureus isolates but is not widely used for the treatment of pediatric osteoarticular infections. METHODS: This was a comparative effectiveness study of hospitalized patients ≤18 years treated with TMP-SMX vs. other antibiotic regimens for acute osteoarticular infections between 2016 and 2021 at 3 hospitals using inverse probability of treatment weighted propensity score analysis. The primary outcome was treatment failure, a composite of unanticipated emergency department (ED) or outpatient visits, hospit…

Fatal adverse drug reaction to trimethoprim.

BACKGROUND: Trimethoprim is the antibiotic of choice for treating uncomplicated community acquired urinary tract infections. However, before prescribing any drug it is vital to obtain a detailed drug history to exclude possible drug allergy. OBJECTIVE: We present the case of a fatal adverse drug reaction due to trimethoprim. We discuss some of the problems that led to the patient's death, and consider how to prevent similar events. DISCUSSION: Re-exposing a patient to a drug suspected of causing an adverse reaction is associated with considerable risk of morbidity and mortality. A detailed h…

Trimethoprim-sulfamethoxazole therapy for ocular toxoplasmosis.

BACKGROUND: Toxoplasmosis is a leading cause of retinochoroiditis. Conventional multidrug therapy using sulfadiazine, pyrimethamine, and folinic acid is increasingly difficult to procure and administer safely. METHODS: To evaluate the efficacy of trimethoprim-sulfamethoxazole, a fixed-combination antibiotic, patients with active toxoplasmosis were treated with trimethoprim-sulfamethoxazole (Bactrim DS) with or without adjunctive clindamycin and prednisone for 4 to 6 weeks. RESULTS: All patients in this study (n = 16) had resolution of active retinochoroiditis and had improved vision, with an…

Acute pancreatitis due to sulfamethoxazole-trimethoprim.

We have reported a case of sulfonamide-induced acute pancreatitis. A review of the literature showed only eight previously reported cases. Adverse reactions to sulfonamides are well known, but acute pancreatitis is a very serious complication. If evidence of pancreatic involvement occurs during sulfonamide therapy, the medication should be discontinued immediately and, if necessary, be replaced by an alternative. Other causes of pancreatitis must be excluded.

Sulfamethoxazole-trimethoprim therapy for Wegener's granulomatosis.

Cyclophosphamide has proved to be the most effective therapy for Wegener's granulomatosis, but mortality remains high at many medical centers, and the necessity for giving this toxic agent for many years to prevent relapses remains a major problem. Successful treatment of this disease with sulfamethoxazole-trimethoprim has been reported by DeRemee et al, and experience in a series of ten patients at Thomas Jefferson University Hospital, Philadelphia, confirms its effectiveness. Nine patients are in remission, and the condition of one patient improved. Relapses occurred in four patients after i…

Bacterial prostatitis: treatment with trimethoprim-sulfamethoxazole.

Fifteen patients with chronic bacterial prostatitis were treated with trimethoprim-sulfamethoxazole for three months. In 60 per cent of the patients a recurrence of chronic bacterial prostatitis developed during a follow-up period of from one to two and one-half years. It appears that factors other than a diffusion barrier to effective antimicrobial agents into the prostatic gland are responsible for the persistence of chronic bacterial infections of the prostate.

Trimethoprim-sulfamethoxazole therapy of pulmonary melioidosis.

A 21-year-old-airman stationed in the Philippines presented with cavitary pulmonary melioidosis. When treated with trimethoprim-sulfamethoxazole, there was prompt resolution of his symptoms and the left upper lobe abscess cavity. It is suggested that this antimicrobial combination is an effective alternative regimen for pulmonary melioidosis.

Sulphadiazine/trimethoprim once daily in maxillary sinusitis: a randomized double-blind comparison with sulphamethoxazole/trimethoprim B.I.D.

In a randomized double-blind study fifty-four patients suffering from acute maxillary sinusitis were treated for 10 days with daily doses of sulphadiazine/trimethoprim (1 g) and sulphamethoxazole/trimethoprim (1.92 g), respectively. The efficacy was evaluated clinically at two follow-up visits. X-ray investigations were performed at admission and after the therapy. Of thirty-nine patients finally evaluated, thirty-seven showed a favourable result. After 6-8 days of therapy there was significant difference in cure rates in favour of sulphadiazine/trimethoprim (p less than 0.05) while the outcom…

Recurrent pancreatitis after trimethoprim-sulfamethoxazole rechallenge.

We report a female patient who repeatedly developed pancreatitis after trimethoprim-sulfamethoxazole (TMP/SMX) use. During childhood she had undergone an ureterosigmoidostomy after which she had been on TMP/SMX 480 mg daily as prophylaxis for pyelonephritis for many years. The patient presented with abdominal pain caused by acute pancreatitis. No other cause, except for TMP/SMX use, could be identified. A causal relationship was confirmed by relapse of the pancreatitis after rechallenge. Our case is unique in demonstrating that acute pancreatitis related to the use of TMP/SMX may occur even af…

Possible trimethoprim/sulfamethoxazole-induced aseptic meningitis.

OBJECTIVE: To report a case of trimethoprim/sulfamethoxazole (TMP/SMX)-induced aseptic meningitis. CASE SUMMARY: An 18-year-old woman diagnosed with acute myeloid leukemia was admitted for a bone marrow transplant. She had already attained remission with daunorubicin, thioguanine, and high-dose cytarabine. A routine lumbar puncture performed on admission revealed an abnormally elevated leukocyte count, and meningitis was suspected. The patient had been taking TMP/SMX (trimethoprim 120 mg) twice daily on Monday, Tuesday, and Wednesday for the past 3 months; no other medication was being used. …

Trimethoprim-sulphamethoxazole therapy in Nocardia keratitis.

PURPOSE: To describe the clinical features, microbiological features and treatment outcome of nine patients with Nocardia keratitis treated with topical trimethoprim- sulphamethoxazole drops. METHODS: Retrospective review of nine patients with culture-proven Nocardia keratitis. RESULTS: Nine patients with Nocardia keratitis were treated with topical trimethoprim-sulphamethoxazole drops. The average duration of treatment was 25 +/- 9 days. Five of the nine patients presented with superficial ulcers with margins studded with yellowish white discrete pinhead sized infiltration; the other four p…
