Conflict and human African trypanosomiasis.

Human African Trypanosomiasis (HAT) has reemerged in sub-Saharan Africa as a disease of major public health importance. The success of HAT elimination in sub-Saharan Africa is subject to the feasibility of controlling, eliminating, or mitigating the determinants of incidence in affected countries. Conflict has been widely recognized and cited as a contributing factor to the resurgence of HAT in many countries, as well as to continuing HAT incidence in politically unstable and resource-poor regions. Despite extensive anecdotal and qualitative recognition of the role of conflict, there has been

Diamidines for human African trypanosomiasis.

Aromatic diamidines are potent trypanocides. Pentamidine, a diamidine, has been used for more than 60 years to treat human African trypanosomiasis (HAT); however, the drug must be administered parenterally and is active against first-stage HAT only, prior to the parasites causing neurological deterioration through invasion of the CNS. A major research effort to design novel diamidines has led to the development of orally active prodrugs and, remarkably, a new generation of compounds that can penetrate the CNS. In this review, progress in the development of diamidines for the treatment of HAT i

Chemotherapy of human african trypanosomiasis.

Human Africa trypanosomiasis is a centuries-old disease which has disrupted sub-Saharan Africa in both physical suffering and economic loss. This article presents an update of classic chemotherapeutic agents, in use for >50 years and the recent development of promising non-toxic combination chemotherapy suitable for use in rural clinics.

Hypocretin and human African trypanosomiasis.

OBJECTIVES: To detail clinical and polysomnographic characteristics in patients affected with Trypanosoma brucei gambiense (Tb.g.) human African trypanosomiasis (HAT) at different stages of evolution and to measure and compare cerebrospinal fluid (CSF) levels of hypocretin-1 with narcoleptic patients and neurologic controls. METHODS: Twenty-five untreated patients affected with T.b.g. HAT were included. The patients were evaluated using a standardized clinical evaluation and a specific interview on sleep complaints. Diagnosis of stages I and II and intermediate stage was performed by CSF cell

[African human trypanosomiasis. Therapeutic strategies].

Although therapeutic strategies for trypanosomiasis appear to be straightforward, their application in the field raises a number of questions which are successively examined below. After a quick reminder of the available drugs, we discuss: Criteria for eligibility: problems of beginning treatment for immunological suspects and determination of the development phase. Present-day problems: availability of drugs; side effects; therapeutic failures. While efforts should be continued to make existing drugs available, it is important that fundamental research be carried out to enhance their utilisat

Natural resistance to African trypanosomiasis.

Glossina infected with African trypanosomes infest 10(7) Km2 of intertropical Africa. Ten thousand cases of human sleeping sickness are officially recorded each year and 35 X 10(6) human beings are at risk. Animal trypanosomiasis impedes the use of 7 X 10(6) Km2 of land adequate for cattle raising and constitutes a major constraint to increasing protein production in Africa. Several approaches are used to combat trypanosomiasis (a) vector eradication has been successful in certain defined situations but cannot be realistically extended to the whole area at risk, (b) prophylactic drugs, which a

Genetic resistance to African Trypanosomiasis.

Genetic resistance to African trypanosomiasis occurs in certain breeds of livestock, many species of wildlife, some strains of mice, and possibly humans. The term trypanotolerance is used to describe this trait, which in domestic livestock is best exhibited by the indigenous West African taurine breeds of cattle, N'Dama and West African Shorthorn, that have been in Africa for 5,000 to 7,000 years. Confirmation not only that these breeds are genetically resistant to trypanosomiasis but also that they are at least as productive as other indigenous breeds is currently leading to their increased u

[Treatment of human African trypanosomiasis].

The melarsoprol is still the only active drug during the nervous stage of African human trypanosomiasis, its prescription still relies on empiric rules and because of its toxicity, it appears necessary to study in gaseous phase chromatography and mass spectrometry, its pharmacokinetic in the serum and the cerebrospinal fluid (CSF) of the patients at the evolutive stage of the illness. Because of the early penetration of the Trypanosoma into the central nervous system, new therapeutic investigations should only search molecules capable to pass through the blood brain barrier: this in unfortunat
