Complex Disseminated Tuberculosis with Oral and Gastrointestinal Involvement: Histopathologic and Clinical Insights.

Background and Clinical Significance: Extrapulmonary tuberculosis (TB) remains a diagnostic challenge, particularly when affecting rare sites such as the oral cavity and digestive tract. We report the case of a 55-year-old woman with disseminated (miliary) tuberculosis presenting with atypical oral lesions initially suspected to represent a malignant tumor. Case Presentation: The patient had a history of recurrent depressive disorder, cognitive impairment, sleep disturbances, and nicotine/alcohol dependence. She presented with painful ulcerations of the oral cavity, dysphagia, odynophagia, and

Oral delivery of bovine tuberculosis vaccine to free-ranging white-tailed deer.

INTRODUCTION: Free-ranging white-tailed deer (Odocoileus virginianus) are a self-sustaining reservoir for bovine tuberculosis (bTB) in northeastern lower Michigan, (United States) continually putting the area's cattle industry at risk. Liberal recreational deer harvest, baiting bans, and mitigation measures on farms have reduced but not eliminated bTB in deer nor have they eliminated transmission to cattle. With apparent prevalence in deer being low (1-2%) but constant, vaccination could be an additional tool to aid in addressing the problem and merits investigation. Mycobacterium tuberculosis

Wild-type distributions of seven oral second-line drugs against Mycobacterium tuberculosis.

OBJECTIVES: To determine wild-type minimum inhibitory concentration (MIC) distributions for Mycobacterium tuberculosis, as the background data for defining susceptibility breakpoints are limited. METHODS: We determined wild-type MIC distributions of M. tuberculosis using a 96-stick replicator in Middlebrook 7H10 (7H10) medium for ethionamide (ETH), prothionamide, thiacetazone, cycloserine, rifabutin (RFB), clofazimine and linezolid in consecutive susceptible clinical isolates (n = 78). RESULTS: Tentative epidemiological wild-type cut-offs (ECOFF) were determined for all investigated drugs wh

Oral tuberculosis presenting as a destructive lower-lip ulcer: A case report.

Oral and maxillofacial tuberculosis is uncommon, and labial involvement is unusual. This report describes lower-lip tuberculosis presenting as a destructive ulcer that clinically resembled malignancy and highlights the value of combined histopathological and microbiological assessment. A 24-year-old man presented with a three-month history of a progressively enlarging lower-lip ulcer causing functional interference and intermittent bleeding. Examination showed a solitary 18 × 11 mm ulcer with an irregular destructive surface, elevated margins, yellow-brown crusting and slough, haemorrhagic are

Oral rifampicin interferes with urine dipstick tests for patients with pulmonary tuberculosis.

BACKGROUND: Rifampicin (RMP) and its major metabolite, desacetyl rifampicin (dRMP), can interfere with urine dipstick tests (UDTs) in patients with tuberculosis (TB) who receive oral RMP. This study sought to examine the effects of RMP and dRMP on UDTs using 2 different urine dipsticks (i.e., Arkray's Aution Sticks 10EA and GIMA's Combi-Screen 11SYS Plus sticks). METHODS: Urine colorimetry was applied to measure RMP concentrations and determine the range of total RMP concentration in the urine within 2-6 and 12-24 h of oral administration of RMP. In vitro interference assays and confirmatory

Discovery of natural-product-derived sequanamycins as potent oral anti-tuberculosis agents.

The emergence of drug-resistant tuberculosis has created an urgent need for new anti-tubercular agents. Here, we report the discovery of a series of macrolides called sequanamycins with outstanding in vitro and in vivo activity against Mycobacterium tuberculosis (Mtb). Sequanamycins are bacterial ribosome inhibitors that interact with the ribosome in a similar manner to classic macrolides like erythromycin and clarithromycin, but with binding characteristics that allow them to overcome the inherent macrolide resistance of Mtb. Structures of the ribosome with bound inhibitors were used to optim

Oral Amyloidosis Secondary to Tuberculosis in an Adult Patient: A Report of a Rare Case.

Oral amyloidosis involving the lateral border of the tongue secondary to tuberculosis is a rare clinical finding. We present the case of a 35-year-old woman with presumed amyloidosis secondary to tuberculosis after increased tongue volume and irregularly shaped lateral edges were noticed. The present study was motivated by the correlation between tuberculosis and its involvement as a trigger in the appearance of amyloidosis, which, in this case, presents with oral manifestations. This retrospective case report describes a patient whose incisional biopsy confirmed oral amyloidosis secondary to

Tuberculosis of oral cavity: a series of one primary and three secondary cases.

INTRODUCTION: Although lateral locking plate has shown promising results in distal femur fracture, there are high rates of varus collapse and implant failure in comminuted metaphyseal and articular fractures. This systematic review evaluates the functional outcomes and complications of dual plating in the distal femur fracture. MATERIALS AND METHODS: Manual and electronic search of databases (PubMed, Medline Embase and Cochrane Central Register of Controlled Trials) was performed to retrieve studies on dual plate fixation in the distal femur fracture. Of the retrieved 925 articles, 12 were in

[Reactivation of tuberculosis with Mycobacterium bovis infection of the oral mucosa during immunosuppression].

HISTORY: A 70-year-old woman who had for five years been treated with tumour necrosis factor (TNF)-a-inhibitors for rheumatoid arthritis was admitted because of treatment-refractory oral ulcerations and persisting considerable soft-tissue swelling over the left maxilla. INVESTIGATIONS AND DIAGNOSIS: Multiple mucosal biopsies from the left maxillary sinus revealed necrotizing granulomatous inflammation suspicious of mycobacterial infection. This was subsequently confirmed in concurrent microbiological cultures and ultimately identified as Mycobacterium bovis. This species of the mycobacterium

Formulation and Scale-up of Delamanid Nanoparticles via Emulsification for Oral Tuberculosis Treatment.

Delamanid (DLM) is a hydrophobic small molecule therapeutic used to treat drug-resistant tuberculosis (DR-TB). Due to its hydrophobicity and resulting poor aqueous solubility, formulation strategies such as amorphous solid dispersions (ASDs) have been investigated to enhance its aqueous dissolution kinetics and thereby improve oral bioavailability. However, ASD formulations are susceptible to temperature- and humidity-induced phase separation and recrystallization under harsh storage conditions typically encountered in areas with high tuberculosis incidence. Nanoencapsulation represents an alt

Microbiological diagnosis of pulmonary tuberculosis in children by oral swab polymerase chain reaction.

Microbiological diagnosis of pediatric pulmonary tuberculosis (TB) is challenging due to the difficulty of collecting and testing sputum from children. We investigated whether easily-obtained oral swab samples are useful alternatives or supplements to sputum. Oral swabs and induced sputum (IS) were collected from 201 South African children with suspected pulmonary TB. IS samples were tested by mycobacterial culture and Xpert MTB/RIF. Oral swabs were tested by PCR targeting IS6110. Children were categorized as Confirmed TB (microbiologic confirmation on IS), Unconfirmed TB (clinical diagnosis o

HIV/TB co-infection: literature review and report of multiple tuberculosis oral ulcers.

Human immunodeficiency virus/tuberculosis (HIV/TB) co-infected subjects demonstrate enhanced HIV replication and plasma viremia; CD4+ T-cell depletion; morbidity and mortality; and susceptibility to secondary bacterial and fungal infections compared to subjects solely infected with HIV. As the incidence of HIV/TB infection has been increasing, one would have expected to encounter oral lesions of tuberculosis more frequently. However, such oral lesions are uncommon. The lesions usually occur as ulcerations of the tongue. We report an additional case in an HIV/TB co-infected 39 year-old black ma

Oral milk exosome-PLGA nanoparticles enhance anti-tuberculosis efficacy of PBTZ169 and bedaquiline.

The oral delivery of next-generation anti-tuberculosis drugs, such as PBTZ169 and bedaquiline (BDQ), is hindered by their poor solubility and low bioavailability. We, here, developed a bioinspired delivery platform that combines milk exosomes with poly(lactic-co-glycolic acid) (PLGA) nanoparticles to deliver the two drugs separately. This nanodelivery system leverages the gastrointestinal stability and mucosal penetration of exosomes, along with the high encapsulation efficiency of PLGA, significantly enhancing drug hydrophilicity and stability. In murine models, the exosome-coated nanoparticl
