[Purulent pleurisy and empyema. With the exception of pleural tuberculosis].

CLINICAL SETTING: Purulent collections in the pleural cavity usually occur as complications of pneumonia in immunodeficient or socially underprivileged patients. PUNCTURE ASPIRATION: The key to diagnosis, pleural aspiration is indicated in case of sufficiently abundant collections, especially in patients with fever. Exploratory puncture is a therapeutic emergency, allowing optimal antibiotic therapy when a causal germ is isolated and drainage of the purulent collection. If there is the slightest doubt, imaging techniques should be used to guide the puncture. DRAINAGE: Drainage is essential a

Pleural Tuberculosis following Infliximab Therapy for Ankylosing Spondylitis.

We present a case of pleural tuberculosis (TB) in a patient on infliximab for ankylosing spondylitis. A 36-year-old male presented to our hospital with low back ache of inflammatory type along with multiple symmetric inflammatory type of joint pain. Further clinical examination, laboratory and radiological investigations were suggestive of ankylosing spondylitis. He was initially treated with nonsteroidal anti-inflammatory drugs but citing poor response it was decided to initiate biologic therapy using infliximab (antitumor necrosis factor-alpha). Mantoux test and chest radiograph were done be

Thoracoscopic pleural biopsy improves yield of Xpert MTB/RIF for diagnosis of pleural tuberculosis.

BACKGROUND AND OBJECTIVE: Extrapulmonary tuberculosis (EPTB) accounts for ~15% of all TB patients, and TB pleural effusion is the second most common site of EPTB. The diagnosis of pleural TB is challenging due to the pauci-bacillary nature of the disease. Histopathology of thoracoscopically obtained pleural biopsy provides the highest diagnostic yield. The Xpert MTB/RIF assay (Xpert) is a PCR test that can identify both Mycobacterium tuberculosis (MTB) and rifampicin resistance. Currently, there is a lack of clarity regarding the value of Xpert on pleural tissue. We report our experience of us

Interferon-gamma release assay performance of pleural fluid and peripheral blood in pleural tuberculosis.

BACKGROUND: The diagnosis of pleural tuberculosis (TB) remains to be difficult. Interferon-gamma release assay (IGRA) is a promising method for diagnosing TB in low TB burden countries. The release of interferon-gamma (IFN-γ) by T lymphocytes increases at a localized site of infection with Mycobacterium tuberculosis antigen. This study aimed to examine the clinical accuracy of T-SPOT.TB on pleural fluid and peripheral blood for the diagnosis of pleural TB in high TB burden country. METHODS: 168 subjects with pleural effusion were enrolled prospectively and examined with T-SPOT.TB on pleural f

Evaluation of Mycobacterium tuberculosis derived cell-free DNA using pleural fluid and paired plasma samples for the diagnosis of pleural tuberculosis.

Pleural tuberculosis (pTB) is a grave clinical challenge. A novel cell-free M. tuberculosis DNA (cfM.tb-DNA) probe-based-qPCR assay was developed for the diagnosis of pTB. Total cell-free DNA was extracted from pleural fluid (PF) and paired plasma samples and cfM.tb-DNA was quantified by probe-based qPCR targeting devR (109-bp) gene of M. tuberculosis in patients with pleural effusion. Patient categorization was done using 'Composite-Reference-Standard' formulated for the study. Assay cut-offs were determined from samples in the 'Development set' (n = 17; 'Definite & Probable' pTB; n = 9 and '

Pleural parasite infection presenting with an isolated pleural effusion misdiagnosed as tuberculosis: a case report.

Pleural parasitic infection is an extremely rare disease of the pleura caused by a variety of parasites, with paragonimiasis infection being the most common. The lack of specific clinical symptoms for paragonimiasis makes it easy to misdiagnose as tuberculosis, causing unnecessary drug-related adverse effects and financial burdens from incorrect treatment. We report a case of a pediatric patient presenting with an isolated pleural effusion that was misdiagnosed as tuberculosis; the patient was eventually diagnosed with pleuropulmonary paragonimiasis infection after immunologic and serologic te

Pleural fluid lysozyme as a diagnostic biomarker of pleural tuberculosis: A systematic review and meta-analysis.

OBJECTIVE: Pleural fluid lysozyme (LP) and its ratio to serum lysozyme (LP/LS) have been proposed as potential biomarkers for diagnosing tuberculous pleural effusion (TPE). We assessed the diagnostic accuracy of LP and LP/LS for TPE and evaluated their ability to differentiate TPE from other effusions. METHODS: We queried the PubMed and Embase databases for studies indexed until October 2021. We included studies that (a) provided information regarding the sensitivity and specificity of LP or LP/LS for the diagnosis of TPE, or (b) compared LP or LP/LS between TPE and malignant or parapneumonic

Factors associated with negative pleural adenosine deaminase results in the diagnosis of childhood pleural tuberculosis.

BACKGROUND: Until now, the influential factors associated with pleural adenosine deaminase (ADA) activity among children remain unclear. This retrospective study was therefore conducted aiming to investigate the factors associated with negative pleural ADA results in the diagnosis of childhood pleural tuberculosis (TB). METHODS: Between January 2006 and December 2019, children patients with definite or possible pleural TB were recruited for potential analysis. Then, patients were stratified into two categories: negative pleural ADA results group (experimental group, ≤40 U/L) and positive pleu

Pleural and Pulmonary Tuberculosis in a Patient Presenting with a Massive Exudative Pleural Effusion: A Diagnostic Challenge.

Worldwide, Mycobacterium tuberculosis (MTB) is the leading cause of death due to a single infectious agent. According to the CDC, there were 9,025 reported new cases of active TB in the U.S. in 2018. There are various diagnostic modalities available to diagnose infection with MTB, whether it is latent or active. We present a diagnostically challenging case of pulmonary and pleural TB in a patient with known risk factors. It serves as a reminder of the high false negative rate of many of the tests for TB.

[The clinical picture of pleural tuberculosis (based on the results of thoracoscopy in 500 patients with a pleural exudate)].

The author presents results of 500 thoracoscopies performed in 500 patients with pleural effusion. Tuberculosis and malignant tumors prevailed (51.8 and 20.8% respectively). Endoscopic examination of pleural cavity amplified by pleural biopsy and cytohistological study of obtained material was shown to be highly effective. In the author's opinion, wet pleurisy is sure to be a surgical problem. To solve it, he considers necessary to allocate 8-10 hospital beds for every 1.5 million of population (in thoracal department of surgical and pulmonological centres, TB dispensaries) right up to organiz

Sensitivity of GeneXpert in pleural biopsy specimens in diagnosing tuberculosis in undiagnosed exudative pleural effusion.

Pleural effusion is among the most common forms of paucibacillary extrapulmonary tuberculosis. Diagnosis is often clinical or based on elevated pleural fluid adenosine deaminase (ADA) levels. However, diagnostic uncertainty arises when clinical suspicion remains high despite low ADA levels, especially in patients already on empirical anti-tubercular therapy, which reduces bacillary load. The aim of this study is to evaluate the diagnostic utility of the Cartridge-Based Nucleic Acid Amplification Test (CBNAAT) in pleural biopsy samples with histopathologically confirmed tuberculosis. In this pr

GC-MS metabolomics identifies novel biomarkers to distinguish tuberculosis pleural effusion from malignant pleural effusion.

BACKGROUND: Tuberculous pleural effusions (TBPEs) and malignant pleural effusions (MPEs) are two of the most common and severe forms of exudative effusions. Clinical differentiation is challenging; however, metabolomics is a collection of powerful tools currently being used to screen for disease-specific biomarkers. METHODS: 17 TBPE and 17 MPE patients were enrolled according to the inclusion criteria. The normalization gas chromatography-mass spectrometry (GC-MS) data were imported into the SIMCA-P + 14.1 software for multivariate analysis. The principal component analysis (PCA) and orthogon

Pleural IFN-γ release assay combined with biomarkers distinguished effectively tuberculosis from malignant pleural effusion.

BACKGROUND: Tuberculosis (TB) remains a major public health concern on a global scale, especially in developing nations. So far, no formal guidelines are available for the diagnosis and treatment of tuberculosis pleurisy. The diagnosis of TB is worsened by the immense difficulty in differential determination of tuberculosis pleural effusion (TPE) and malignant pleural effusion (MPE). The purpose of this investigation is to assess the differential diagnostic efficiencies of the pleural IFN-γ release assay (IGRA) and widely-used biochemical parameters in the distinction analysis of TPE and MPE.
