Conjoint twins (1).

Two cases of Conjoined twins are reported. The etiology, incidence, diagnosis and management are discussed. It is suggested that if conjoined twins are diagnosed before labour, of if there is dystocia, Caesarean section is the treatment of choice.

Conjoined thoracopagus twins.

A case of vaginally delivered stillborn female thoracopagus conjoined twins is presented. Each twin had a cleft lip and cleft palate. They shared a common pericardial sac and a common abdominal cavity. The liver was conjoined, and the small bowel was common to both twins. One twin was asplenic. Although the twins were monozygotic, their viscera were neither identical nor mirror-image to each other, and the visceral anomalies were unique to each twin. The hearts were fused, and communications existed between the circulations at the atrial and ventricular levels. One twin had single atrium, soli…

Chorangiopagus parasiticus twins.

Chorangiopagus parasiticus twins (CAPP) occur due to asymmetric abnormality of monozygous, twinning. It is the commonest form of asymmetric twinning, occurring in 1% of monozygous twins. The asymmetric twin survives by parasitizing the more normally developed co-twin by connecting with the chorionic circulation, hence, chorangiopagus parasiticus. CAPP twin is a markedly edematous foetus with relatively well-developed legs, incomplete pelvis and lower spine. The body cavity contains some incomplete abdominal viscera but usually no thoracic organs. The upper portion of the twin consists of edema…

Delivery of twins.

The delivery of twins presents considerable challenges to the obstetric team, particularly in terms of decision-making, technical skills required and the need to respond quickly to changing circumstances. There is a serious lack of sound evidence upon which to base decisions concerning the method of delivery of twins. The trend towards the routine use of caesarean section is not supported by evidence of improved outcome for the infants, while maternal outcome is compromised. Specific circumstances that may have a bearing on the need for caesarean section include gestational age, presentation o…

Snoring in twins.

Snoring is a prerequisite for obstructive sleep apnoea (OSA) and is known to run in families. Recent studies have provided sufficient evidence for a familial predisposition to OSA. In our study, 492 monozygotic and 284 dizygotic twins were contacted by telephone and asked to attend an interview which included questions of life habits, medical history, sleep habits and disorders, with particular emphasis on snoring. Our study showed that the probandwise concordance rate for habitual snoring was higher in monozygotic twins than in dizygotic ones, but the difference was not significant. The compa…

Syndromes in twins.

There is a higher incidence of perinatal mortality and morbidity in twins than in singleton pregnancies. The main reason for this increase is the higher incidence of preterm delivery in twins. There are special conditions unique to twins that also contribute to this increased perinatal morbidity and mortality. These conditions include monoamniotic twins, congenital anomalies unique to twins, ie, conjoined twins and acardia, intrauterine fetal demise, and twin-to-twin transfusion syndrome. These conditions are the subject of this review. The prenatal determination of chorionicity is discussed f…
