A rare case of male urethral diverticulum presents in this article. A 34-year-old man was referred to our clinic because of post-void dribbling. The retrograde cystogram and cystoscopy confirmed a 3 cm urethral diverticulum in the penile urethra without having urethra stricture. Open surgical repair was scheduled to excise urethral diverticulum and restore urethral continuity. Corpus spongiosum and dartus fascia was used to reinforce the repair.

A 27-year-old man who has sex with men presented with a painful lesion at the urethral meatus. A diagnosis of genital herpes was suspected, and initial tests for sexually transmitted infections (STIs), including syphilis serology, were negative. Urethral stricture is a narrowing of the urethra due to scar tissue. It causes obstructive voiding dysfunction and can lead to long-term damage of the entire urinary tract. The probability of therapeutic success is dampened by a high rate of recurrence. Therefore, a careful taking of the patient's history and further diagnostics are crucial for finding the appropriate form of therapy. This review highlights the clinical presentation and diagnostic workup of urethral strictures.

Urethral strictures are very commonly encountered in urologists' day-to-day work. Many specific therapeutic and reconstructive procedures are available for treatment of the different types of urethral stricture, all of which can be recommended when the indications for each are carefully observed. This paper highlights operative concepts that allow a surgical therapy that is tailored to the localisation and length of the stricture and the severity of spongiofibrosis.

According to the author, impotence associated with traumatic urethral strictures arises rather due to operative intervention than primary trauma. Inaccurate manipulations of the surgeon may cause destruction of the bulbocavernous muscles and blood vessels reaching the penis at bulbar level. Another cause of impotence is chronic prostatitis which may follow long-term bougienage. Adequate operation allowing for anatomic and functional features of the perineum does not entail impotence.

Urethral stricture is a challenging surgical problem since the ancient time. Its outcome is very poor because of the high recurrence rate 40.0-50.0% whatever procedure is conducted to treat the condition. A total of 5 patients with recurrent posterior urethral strictures underwent endoscopic placement of an expandable endourethral stent made of titanium. Patient age ranged from 17 to 66 years (mean age 42.6 years). Followup ranged from 13 to 20 months (mean 14.1 months). Of the patients 4 presently have unobstructed voiding with no incontinence. To date no side effects have been directly related to the stents and no incrustations or calculi have formed. Our preliminary study supports the use of titanium urethral stents as an alternative form of treatment for selected urethral strictures.

Urethral reconstruction in women encompasses a variety of different entities including female urethral stricture, female urethral diverticulum, and vesicovaginal fistula. Although very different in their causes, they all have in common a vague and nonspecific onset of symptoms and, usually, a delayed diagnosis. Once identified and evaluated, urologists must review the management options with the patient, which range from minimally invasive procedures to complex reconstructive surgeries. In complicated cases, we recommend referral to an experienced specialized center for definitive management.
