Female urinary incontinence rehabilitation.

Pelvic floor rehabilitation (PFR) is an important and recommended strategy for the treatment of many urogynecological disorders including urinary incontinence (UI). The recognised pioneer of PFR is the American gynecologist Arnold Kegel who, over 50 years ago, proposed pelvic floor muscle exercises (PFME) to prevent and/or treat female UI. Kegel's techniques were successfully used by others too, but as the years passed these techniques sank into unjustified oblivion. In the 1980s in Europe the medical world's interest in PFME techniques gained ground, contemporaneously with functional electric…

Colpocleisis and urinary incontinence.

OBJECTIVE: The study was undertaken to review results of colpocleisis performed for advanced pelvic organ prolapse (POP) in elderly women, w … t suburethral sling and 12 (19%) underwent suburethral plication procedures for treatment of stress urinary incontinence (SUI). Three patients with dementia were unable to offer subjective symptoms. One patient died in hospital from multisystem organ failure unrelated to colpocleisis. When last seen, 2 (3%) of the remaining 60 patients had some persistence of symptoms …

Pharmacotherapy of urinary incontinence.

Aging is associated with many changes that may predispose an individual to urinary incontinence. An appropriate pharmacologic treatment plan depends on identification of the type of incontinence and minimization of aggravating factors. Stress incontinence is caused by incompetence of the internal urethral sphincter and is most common in postmenopausal women. This type of incontinence may respond to estrogen therapy and/or alpha-adrenergic agonists. Urge incontinence may occur in both men and women as a result of inappropriate detrusor muscle contraction. This condition may be treated with estr…

Ultrasonography in urinary incontinence.

Linear array ultrasound techniques were utilized in place of conventional radiologic procedures to study the dynamics of the urethrovesical junction and proximal urethra in patients with urinary incontinence. This ultrasound procedure provided an objective demonstration of the mobility of the urethrovesical junction and documented the presence of an anatomic defect. It aided in the selection of patients suitable for surgical correction of stress incontinence and their postoperative follow-up. Ultrasound was also employed to demonstrate uninhibited detrusor contractions in patients with vesical…

Embryology of urinary incontinence.

Congenital incontinence may be divided into primary and secondary types. Primary incontinence refers to the development of a conduit which bypasses the normal sphincter mechanism, while secondary incontinence develops after a congenital outlet obstruction leads to bladder decompensation and overflow. The embryogenesis of the various forms of primary incontinence is described in detail and management is discussed.

Urinary incontinence in the elderly.

The incontinent elderly face many difficulties from a problem that can have a multiplicity of causes. Etiologic factors of incontinence may be medical, psychologic or social. The cortical inhibitory stimuli that prevent involuntary voiding tend to decline with age. When carefully measured, some degree of uninhibited neurogenic bladder is often found in the elderly. Nonetheless, incontinence does not necessarily result. Precipitating factors, often emotional, determine the outcome.

[Urinary incontinence in children].

Urinary incontinence in children and adolescents is most often of functional origin. Questioning and clinical examination with a bladder diary should look for underlying urological or neurological causes. The type of incontinence, daytime or nocturnal, must be specified to adapt treatments. Renal and bladder ultrasound is recommended, and urodynamic studies are not routinely used. Life style advices and bowel management are prerequisites for treatment. Depending on the type of symptom, drug treatment, pelvic floor treatment, behavioral measures or stimulation of the posterior tibial nerve can …

Male Stress Urinary Incontinence.

Male stress urinary incontinence most commonly presents secondary to prostate treatment of prostate cancer or benign prostatic hyperplasia, although it can be seen following trauma or secondary to neurologic dysfunction. Patient selection is supremely important when choosing the appropriate nonoperative or surgical treatment. Although most patients prefer the passive mechanism of adjustable balloon devices or slings, the artificial urinary sphincter provides the most efficacious and durable results with regard to patient satisfaction and continence. Changes in prostate treatment modalities and…

Urinary incontinence in women.

Urinary incontinence affects women of all ages. History, physical examination, and certain tests can guide specialists in diagnosing stress urinary incontinence, urgency urinary incontinence, and mixed urinary incontinence. First line management includes lifestyle and behavior modification, as well as pelvic floor strength and bladder training. Drug therapy is helpful in the treatment of urgency incontinence that does not respond to conservative measures. In addition, sacral neuromodulation, intravesical onabotulinumtoxinA injections, and posterior tibial nerve stimulation can be used in selec…

Treatment of urinary incontinence.

The treatment of urinary incontinence is a dynamic and evolving field. New therapies, techniques, and procedures, as well as some subtle refinements in treatments currently used, offer hope for the millions of people suffering from this condition. Recent attention has been directed toward reevaluating the efficacy and durability of some standard treatments for stress urinary incontinence, including pelvic floor exercise, bladder neck suspensions, and pubovaginal slings. Occlusive or supportive devices have evolved into a suitable alternative in some patients. Extracorporeal magnetic therapy an…
