Highly selective vagotomy and truncal vagotomy and pyloroplasty for duodenal ulcer: a clinical review.

A review was conducted of the operations of highly selective vagotomy (HSV) and truncal vagotomy and pyloroplasty (TVP) performed for peptic ulcer disease. Gastric acid output was reduced equally after both procedures: basal acid output was reduced by 80% and maximal acid output by 50% to 60%. Not surprisingly, therefore, the recurrence rates were similar (6.6% after HSV compared to 5.6% after TVP). However, the mortality following HSV was 0% compared with 0.7% after TVP. The incidence of all side effects was lower following HSV, so that the results in 86% of patients who underwent this operat…

Gastric motility and emptying following parietal cell vagotomy and selective gastric vagotomy. Experimental investigation.

After parietal cell vagotomy in dogs antral motility and gastric emptying time is unaffected. Addition of a drainage procedure changed the motility considerably by decreasing the force of contractions and diminishing the number of peridtaltic waves. After cutting the antral nerves complete atony occurred, but emptying rate was unaltered as the contrast meal passed passively through the anastomosis. Selective gastric vagotomy without drainage caused atony and significantly prolonged emptying time.

Comparison of highly selective vagotomy with truncal vagotomy and pyloroplasty: results at 8-15 years.

In 1983 we reported the early results (mean 5 years) of a prospective randomized comparison of highly selective vagotomy (HSV) with truncal vagotomy and pyloroplasty (TVP) where all 137 operations were performed by the same surgeon. HSV was significantly better than TVP in terms of Visick grading and side-effects. The same patients were assessed at a mean of 12 years (range 8-15 years) after operation. There was no difference on assessment using Visick grading between TVP (59 patients) and HSV (57 patients) (grades I and II, 75 per cent in each case). However, 20 per cent of TVP patients (but …

[A gastro-electromyographical study on selective vagotomy and proximal selective vagotomy in the dog (author's transl)].

The gastro-electromyographical changes after vagotomy were studied by means of chronic in-situ experiments on eighteen dogs. Basic electric rhythms (slow spikes) were classified into normo-peristalsis and anti-peristalsis. The periodical rhythm of normo-peristalsis was partly disturbed after selective vagotomy but was more regular after feeding. Normally, rate changes of normo-peristalsis show typical patterns depending on the types of meals. These distinctive features were observed more clearly in S.V. and S.P.V. dogs. The incidence of normo-peristalsis and anti-peristalsis during fasting was…

Persistent dysphagia after laparoscopic vagotomy.

BACKGROUND: Laparoscopic vagotomy represents a new and less invasive treatment for peptic ulcer disease, but the problem of postvagotomy dysphagia has not been solved. The aim of this study was to determine the etiologic factors related to long-term laparoscopic postvagotomy dysphagia. METHODS: Two female and 11 male patients with a mean age of 48.5 years who underwent laparoscopic vagotomy were investigated retrospectively. Preoperative diagnosis included duodenal ulcer resistant to medical treatment, gastric hypersecretion, gastric outlet obstruction, cholelithiasis, and gastroesophageal re…

[Vagotomy for bleeding pyloroduodenal ulcer].

In the choice of surgical tactics and operative method in bleeding pyloroduodenal ulcer the author suggests forming several uniform groups with concrete recommendations for the terms and types of organ-preserving operations with vagotomy. Tactical principles of treatment, elaborated from clinical material number of urgent and emergency operations to be reduced to 15% (74). Among 74 patients who underwent surgery for urgent and emergency indications the mortality rate was 8.1%. The late-term results were excellent and good in 88% and poor in 12% of patients.

[Recurrent peptic ulcer after vagotomy].

The examination of 38 patients with recurrent and peptic ulcers after vagotomy (reconstructive operations were fulfilled in 25 out of them) has established that the main cause of recurrent and peptic ulcers developing after vagotomy is the preserved elevated secretory activity after operation and high content of gastrin in the blood. Surgery is thought to be the main method of treatment of such patients.

Truncal vagotomy in morbid obesity.

Vagotomy has been shown to reduce body weight in several species of experimental animals. Due to the relative safety and simplicity of the procedure and the long-clinical evaluation of vagotomy in ulcer disease, truncal vagotomy without drainage has been performed in a series of 21 morbidly obese patients. The mean maximum body weight was 12.8 +/- 3 kg (s.e.). In the 14 patients observed for 12-40 months, the mean weight decrease is 20 +/- 4 kg (range: 0-51). Apart from lesion of the oesophagus in one patient, there have been no operative complications. In one 45-year-old patient sudden death …

Proximal gastric vagotomy. Initial experience.

Proximal gastric vagotomy was performed in 223 patients with chronic duodenal ulceration between 1973 and 1977. The follow-up ranged from 6 to 78 months with a mean of 39 months. There was no operative mortality. Adverse postoperative sequelae, including delayed gastric emptying, dumping, diarrhea, and reflux gastritis, were seen in less than 3% of patients. Ulcers recurred in 11 patients (4.9%). We conclude that proximal gastric vagotomy is an effective, safe, and satisfactory operation for chronic duodenal ulcer over the short term.

Dysphagia after proximal gastric vagotomy.

Ten patients were examined before and after proximal gastric vagotomy with videotape recordings of barium passage in esophagus, esophageal pressure recordings, and the insulin test. Four of the patients complained of dysphagia after the operation. The dysphagia subsided spontaneously after 1--2 months. Patients with dysphagia showed dilatation of the body of the esophagus and a tapered narrowing in the distal segment. The pressure recordings showed increased frequency and strength of rhythmic pressure waves compared with the preoperative findings. Our results support the theory of a neuromuscu…

A porspective study of parietal cell vagotomy and selective vagotomy-antrectomy for treatment of duodenal ulcer.

A prospective, randomized, study involving 92 patients who required elective operation for treatment of duodenal ulcer was performed to compare the results of Parietal Cell Vagotomy (PCV) and selective vagotomy-antrectomy Billroth I (SV-A-BI). The protocol was broken twice. One patient was unable to undergo PCV because of pyloric stenosis and one patients underwent Billroth II anastomosis instead of Billroth I because of post-bulbar stenosis. Performance of PCV was never aborted because a patient was obese. There were no deaths. Diarrhea, dumping and other gastric complaints were less frequent…

A followup report of a prospective evaluation of vagotomy-pyloroplasty and vagotomy-antrectomy for treatment of duodenal ulcer.

A prospective randomized study of 200 consecutive patients who required elective operation for treatment of duodenal ulcer was conducted. Truncal vagotomy and drainage (V-D) was done in 108 patients with a 2% mortality and truncal vagotomy and antrectomy (V-R) were performed in 92 patients with no mortality. Ninety-four per cent of these patients were followed 5-8 years after operation or until their death if that preceded the termination of the study. The immediate postoperative morbidity including stomal dysfunction and reoperation was greater after V-D than after V-R. In the opinion of the …

[Method of selective proximal vagotomy].

A method of selective proximal vagotomy has been developed experimentally. It provides additional parasympathetic denervation of an acid-producing zone of the stomach by means of circular transsection of gastric and esophageal muscular membranes above and below cardial sphincter with forming of are flux esophageal-gastric valve. Clinical use of this method in 75 patients confirmed experimental data about a good function of the created valve and adequate decrease of acid production. This improved short and long-term results and reduced the time of temporary disability.
