Knowledge of urticarial vasculitis is very important, because its therapy and prognosis are different to other clinically similar diseases, eg idiopathic urticaria. We present a 72-year-old female patient suffering from recurrent urticaria associated with slight fever, arthralgia, occasional abdominal pain, as well as highly elevated erythrocyte sedimentation rate. Respiratory vasculitis is characterized by granulomatous changes pathologically. Both upper and lower respiratory tracts may be involved. It is convenient to separate three entities: (1) Wegener's granulomatosis, (2) allergic granulomatosis and angiitis, or Churg-Strauss syndrome, and (3) polymorphic reticulosis/lymphomatoid granulomatosis complex. Churg-Strauss syndrome responds well to glucocorticoids. Cytotoxic drugs appear necessary for ultimate control of Wegener's granulomatosis. Necrotizing vasculitis is a term used to describe vessel wall necrosis due to neutrophil infiltration. Current evidence strongly suggests that these cells are responding to elaboration of chemotactic factors of the complement cascade released at the site of deposition of immune complexes in the vessel wall. Cutaneous allergic vasculitis alone or combined with a systemic condition is of interest to the phlebologist when it occurs in the leg. All the clinical and paraclinical examinations must be oriented towards looking for infection, toxaemia, or even a so-called auto-immune disease. Treatment, although rarely very successful as regards the etiology, includes a considerable range of therapeutic possibilities, e.g. vasculotropic and anti-inflammatory drugs and immunosuppressive chemotherapy, to induce rapid sedation and often definitive cure.

Of the primary vasculitis pediatricians are familiar with, Kawasaki disease and IgA vasculitis are the most common. The other large, medium and small vessel vasculitis are seldom seen in practice. Though rare, early diagnosis and appropriate management is critical for the best outcome. Primary vasculitis in the pediatric age group have several differential diagnoses which range from infections to monogenic causes such as deficiency of Adenosine Deaminase -2. Each child, therefore, needs a careful systematic approach.
