[Leiomyosarcoma of the inferior vena cava].

HISTORY AND CLINICAL FINDINGS: In a 34-year-old woman who underwent ultrasound investigation because of recurrent pyelonephritis a space-occupying lesion in the liver was an incidental finding. Computed tomography (CT) and magnetic resonance imaging (MRI) confirmed a tumour in segment I of the liver. The patient had been on contraceptives for 12 years. She had no symptoms. INVESTIGATIONS: Laboratory tests, including tumour markers, were unremarkable. Diagnostic tests to exclude malignant tumour with metastases were negative (thyroid scintigraphy, mammography, coloscopy and gastroscopy). Furth

Vena cava filter devices.

Vena cava filters are effective in preventing pulmonary embolism (PE) in patients with deep vein thrombosis or PE who either have contraindications to anticoagulation or have sustained a PE despite adequate anticoagulation. Although vena cava filters are not without complications, clinically significant morbidity and mortality are low. The use of vena cava filters as primary prophylaxis or therapy for deep vein thrombosis and PE should await the results of controlled trials.

Injuries of the inferior vena cava.

Beginning in 1946, 577 patients with inferior vena cava injuries were managed at a single institution. After decreasing from 37 to 30 percent, the mortality rate showed a distinct increase in the last 7 years studied. This increase was related to an increasing percentage of patients who arrived in the emergency center in severe shock and required resuscitative thoracotomy. In-hospital care advances have not kept pace with improvements in prehospital care. Although venous complications have not been infrequent, morbidity has not been a significant long-term problem. Fatal pulmonary embolism occ

Leiomyosarcoma of the inferior vena cava.

The thirty-second case of leiomyosarcoma of the inferior vena cava and the twelfth resectable patient in the series is presented. A review of the literature demonstrates a marked female preponderance (5 to 1). The most common presenting symptom is right abdominal pain and a palpable mass. The different surgical problems generated by the involved segment of the cava are discussed. Noteworthy is the high incidence of Budd-Chiari syndrome owing to hepatic vein obstruction with involvement of the upper third of the cava in the postmortem cases. An argument is developed for debulking the tumor for

Inferior Vena Cava Syndrome.

Inferior vena cava syndrome (IVCS) is an infrequent but potentially life-threatening condition resulting from obstruction of the inferior vena cava (IVC), leading to impaired venous return. Hepatic vena cava syndrome is a rare cause of IVCS. We report the case of a 58-year old man with a complex medical and surgical history who presented with abdominal pain and distension. He had cirrhosis of the liver with moderate ascites, and diagnostic paracentesis confirmed spontaneous bacterial peritonitis. The patient developed refractory hypotension requiring vasopressors, encephalopathy, and hypoxic r

Inferior vena cava-syndrome.

Inferior vena cava syndrome (IVCS) is caused by agenesis, compression, invasion, or thrombosis of the IVC, or may be associated with Budd-Chiari syndrome. Its incidence and prevalence are unknown. Benign IVCS is separated from malignant IVCS. Both cover a wide clinical spectrum reaching from asymptomatic to highly symptomatic cases correlated to the underlying cause, the acuity, the extent of the venous obstruction, and the recruitment and development of venous collateral circuits. Imaging is necessary to determine the underlying cause of IVCS and to guide clinical decisions. Interventional th

Inferior vena cava filters.

Use of inferior vena cava (IVC) filters has increased dramatically in recent decades, despite a lack of evidence that their use has impacted venous thromboembolism (VTE)-related mortality. This increased use appears to be primarily driven by the insertion of retrievable filters for prophylactic indications. A growing body of evidence, however, suggests that IVC filters are frequently associated with clinically important adverse events, prompting a closer look at their role. We sought to narratively review the current evidence on the efficacy and safety of IVC filter placements. Inferior vena c
