Periodic leg movements during sleep and wakefulness in narcolepsy.

The objectives of the study were to measure the prevalence of periodic leg movements during NREM and REM sleep (PLMS) and while awake (PLMW) and to assess the impact of PLMS on nocturnal sleep and daytime functioning in patients with narcolepsy. One hundred and sixty-nine patients with narcolepsy and 116 normal controls matched for age and gender were included. Narcoleptics with high and low PLMS indices were compared to assess the impact of PLMS on sleep and Multiple Sleep Latency Test (MSLT) variables. More narcoleptics than controls had a PLMS index greater than 5 per hour of sleep (67% ver

Multiple sleep latency test and maintenance of wakefulness test.

In addition to overnight polysomnography, there are two special tests with which every sleep technologist should be familiar: the multiple sleep latency test and the maintenance of wakefulness test. These two tests classify excessive daytime sleepiness using objective data. The role of the sleep technologist is to understand and perform an accurate test so that sleep clinicians can use that data in diagnosing and treating their patients. This article provides step-by-step directions for performing these tests.

Bicoherence of intracranial EEG in sleep, wakefulness and seizures.

The hypothesis that the intracranial EEG has local structure and short-term non-stationarity is tested with a little-studied measure of non-linear phase coupling, the bicoherence in human subdural and deep temporal lobe probe data from 11 subjects during sleeping, waking and seizure states. This measure of cooperativity estimates the proportion of energy in every possible pair of frequency components, F1, F2 (from 1 to 50 Hz in this study), that satisfies the definition of quadratic phase coupling (phase of component at F3, which is F1 + F2, equals phase of F1 + phase of F2). Derived from the

Patterns of sleep and wakefulness in treated narcoleptic subjects.

In most reported studies of treatment for narcolepsy, the efficacy of treatment has been assessed in the sleep laboratory, but not in the natural work and home settings. We therefore assessed the sleep/wake patterns of 25 subjects with narcolepsy, whose symptoms were considered under satisfactory control by the subjects and their physicians, using polygraphic recorders, and compared these with the patterns of 25 control subjects matched for age and gender. Data from one matched pair of subjects were excluded for technical reasons. There was no difference between the two groups of subjects in t

Dopaminergic transmission and the sleep-wakefulness continuum in man.

Modulation of dopaminergic transmission on daytime alertness and performance and on nocturnal sleep were studied in man using 30, 60 and 90 mg pemoline, a dopamimetic drug, and 2, 4 and 6 mg pimozide, a dopamine receptor antagonist. Pemoline lengthened daytime sleep latencies and improved attention, and increased wakefulness during nocturnal sleep. Rapid eye movement (REM) sleep was reduced with 90 mg pemoline, but this was due entirely to increased wakefulness. Pimozide had little effect on overnight sleep, but increased the tendency to fall asleep and impaired performance during the day. The…

Differential activation of respiratory muscles during wakefulness and sleep.

Evidence exists that strongly suggests that an imbalance in both the magnitude and timing of electrical activity between the upper airway and chest wall inspiratory muscles can exist during sleep. Too little or delayed activity of upper airway inspiratory muscles is associated with upper airway narrowing or collapse in patients with obstructive apnea. Preferential activation of upper airway muscles is associated with a decrease in upper airway resistance or in reopening of the airway following an apnea. These interrelationships between the upper and lower respiratory muscles may be exacerbated

Narcolepsy: regional cerebral blood flow during sleep and wakefulness.

Serial measurements of regional cerebral blood flow were made by the 135Xe inhalation method during the early stages of sleep and wakefulness in eight normal volunteers and 12 patients with narcolepsy. Electroencephalogram, electro-oculogram, and submental electromyogram were recorded simultaneously. In normals, mean hemispheric gray matter blood flow (Fg) during stages I and II sleep was significantly less (-9.2 percent) than waking values (84.3 +/- 13 ml per 100 gm brain per minute). Maximum regional blood flow decreases during sleep occurred in the brainstem-cerebellar (-25.1 percent), righ…

Effects of Blindness on Sleep/Wakefulness States in Mice.

To examine the effects of blindness on sleep/wakefulness states, we compared locomotor activity and delayed recovery from isoflurane anesthesia induced by hypnotics during light and dark periods in sighted CBA/N and blind CBA/J mice. Locomotor activity around the switch from the dark to light period significantly differed in both mice. Delayed recovery induced by brotizolam was attenuated in both periods in CBA/J mice. In addition, the period specificity of delayed recovery caused by suvorexant or diphenhydramine in CBA/N mice was abolished in CBA/J mice. These results suggest that blindness i…

Regulation of wakefulness by astrocytes in the lateral hypothalamus.

The lateral hypothalamus (LH) is an important brain region mediating sleep-wake behavior. Recent evidence has shown that astrocytes in centr … d during Wake-to-NREM sleep transitions. Chemogenetic activation of LH astrocytes potently promotes wakefulness and maintains long-term arousal, while chemogenetic inhibition of LH astrocytes decreases the total amount of wakefulness in mice. Moreover, by combining chemogenetics with fiber photometry, we show that activation of LH astrocytes significantly inc …

Neural-immune interactions in disorders of sleep-wakefulness organization.

Novel findings on the effects of inflammatory molecules on neuronal circuits, and on molecular interactions between immunity and sleep, in health and disease, shed light on the pathogenesis of disorders of past (encephalitis lethargica) and present concern (human African trypanosomiasis and narcolepsy), which share alterations in sleep-wakefulness transitions. Although these three disorders differ in etiology, synaptic interactions with immune-response-derived molecules could play a pathogenetic role. Knowledge obtained on neural-immune interplay during senescence also has implications for age…

Management of a wake-up stroke.

Current national guidelines advocate intravenous thrombolysis to treat patients with acute ischaemic stroke presenting within 4.5 hours from symptom onset, and thrombectomy for patients with anterior circulation ischaemic stroke from large vessel occlusion presenting within 6 hours from onset. However, a substantial group of patients presents with acute ischaemic stroke beyond these time windows or has an unknown time of onset. Recent studies are set to revolutionise treatment for these patients. Using MRI diffusion/FLAIR (fluid-attenuated inversion recovery) mismatch, it is possible to identi

Hypocretins: the timing of sleep and waking.

The appropriate time and place for sleep and waking are important factors for survival. Sleep and waking, rest and activity, flight and fight, feeding, and reproduction are all organized in relation to the day and night. A biological clock, the suprachiasmatic nucleus (SCN), synchronized by photic influences and other environmental cues, provides an endogenous timing signal that entrains circadian body rhythms and is complemented by a homeostatic sleep pressure factor. Cholinergic, catecholaminergic, serotonergic, and histaminergic nuclei control wakefulness and mutually interact with the SCN …

Waking up to sleeping sickness.

Devastating epidemics of human African trypanosomiasis are currently re-emerging in many sub-Saharan countries. In the past three decades, clinical research into this important disease has been neglected, as have urgently needed initiatives on drug development, disease surveillance and vector control. Recent impetus has aimed to provide a free supply of antitrypanosomal drugs, to develop a new orally active trypanocidal agent and to attack the tsetse vector with modern technology. In addition, pan-African initiatives to co-ordinate control efforts have begun. These all provide some hope for th
