This article summarizes six years of studies with an experimental model, which uses a static positioning frame, pressure-sensitive film, and a microcomputer-based videodigitizing system, to characterize the biomechanics of the human cadaver wrist by measuring the contact areas and pressures in a variety of normal, simulated traumatic, and surgically treated conditions. The results of these studies have provided clinically relevant information about the normal anatomy and functional mechanics of the wrist, as well as guidelines for the treatment for a number of different fractures and ligament …

The ligaments of the wrist were studied by dissecting ten fixed and seven fresh frozen wrists. In three other specimens multiple cross-sections were prepared. These studies show that the wrist ligaments can be classified into two groups: extrinsic and intrinsic. In the extrinsic group, the deep volar radiocarpal ligaments are three strong and very important structures connecting the radius to the capitate, the radius to the lunate, and, in a deeper layer, the radius to both the scaphoid and the lunate. A ligamentous deficiency was noted frequently between the capitate and the lunate. There als…

The term osteoarthritis (OA) of the wrist can be used as an umbrella term for various, often independent areas of OA, as the wrist is made up of several joints. Radiocarpal OA often occurs after untreated ligament injuries, incorrectly healed bone fractures in the carpus or after radius fractures involving the joint. A typical sequence of propagation is known for radiocarpal OA following scapholunate (SL) insufficiency or scaphoid pseudarthrosis. Other causes include inflammation, crystal deposits or bone necrosis. Ulnocarpal arthrosis occurs posttraumatically or primarily when there are diffe…

LEARNING OBJECTIVES: After reading this article and reviewing the supplemental videos, the participant should be able to: 1. Identify common wrist conditions that may be encountered on evaluation. 2. Describe provocative maneuvers used to confirm a diagnosis of wrist disorder. 3. Develop a systematic approach to examination of the wrist. 4. Determine appropriate diagnostic maneuvers for radial, central, and ulnar wrist pain.
SUMMARY: The wrist is a complex structure, and providers caring for hand and upper extremity conditions need to have an understanding of the wrist examination and provoca…

In the past, the diagnostic imaging algorithm for evaluating the painful wrist included initial plain radiographic examination followed by arthrography, tomography, bone scintigraphy, or computed tomography. In recent years, magnetic resonance imaging (MRI) has been proven efficacious for diagnosing a number of maladies of the bones, ligaments, and soft tissues. MRI can be of aid in evaluation of carpal instability, disorders of the triangular fibrocartilage, ulnar impaction syndrome, distal radioulnar joint (DRUJ) instability, fracture, avascular necrosis (AVN), tendinopathy, nerve entrapment…

We present a case of synovitis of the wrist due to histoplasmosis, diagnosed only after extensive surgery and culturing. Treatment with amphotericin B in combination with radical surgery was effective in curing the disease. This manifestation was probably an exacerbation of a latent chronic infection with Histoplasma capsulatum, although it was unclear why the exacerbation occurred. Synovitis resistant to treatment should be assessed with great care, especially in view of the growing number of immunocompromised patients. Close collaboration between surgeon, rheumatologist, pathologist and micr…

Some basic principles of orthopedic management of common conditions of the hand and wrist have been presented. These basic principles and an awareness of the potential complications should aid radiologists in evaluation of postoperative films. The interested reader is referred to Green's Operative Hand Surgery and Chapman's Operative Orthopaedics for more detailed information.

Annales de chirurgie de la main et du membre superieur : organe officiel des societes de chirurgie de la main = Annals of hand and upper limb surgery1992fre
Fifty patients with wrist arthritis mainly secondary to a scaphoid pseudarthrosis (44%) or a Kienböck disease (22%) were operated on by one surgeon by "total" denervation. With a post-operative follow up of 5 years, pain, strength and range of movement were reviewed. Four complications are by means of stressed: one painful neuroma and three radial nerve paresthesiae. 72% of the patients had pain improvement with an average of 72 points on a scale of 100 points. Strength and range of movement were not significantly improved.

Carpal instability is the result of ligamentous injury to the wrist. Although any ligament may be injured, certain recurring patterns are frequently seen. Common to the treatment of all instability patterns is the need to transfer load from the capitate to the radius: reliably, without stressing normal joints, and while preserving functional motion. Limited wrist arthrodesis fulfills these needs. The radius and carpal bones are shaped so that in their "normal" position, they are preloaded. Were it not for complex ligamentous systems, the bones would immediately assume unloaded "neutral anatomi…

The wrist joint is involved in a great majority of cases of rheumatoid arthritis. Tenosynovitis and synovitis can lead to severe destruction of the joint and to spontaneous ruptures of the tendons. Therefore, early tenosynovectomy and wrist-joint synovectomy are required. At the advanced stage of the disease, arthroplasty is necessary. Arthrodesis is rarely indicated in the treatment of severe wrist destruction in rheumatoid arthritis.

Wrist instabilities are frequently underdiagnosed and undertreated because they are complex and difficult to understand. A study of the anatomy of the wrist and the pathomechanics of these injuries leads to a better understanding of these problems and to a more specific diagnosis. Treatment involves stabilization of the unstable segment by ligament repair in the acute injury and by limited intercarpal arthrodesis in the chronic case.
