Prevalence, risk factors, and adverse maternal and fetal outcomes of anemia during pregnancy: a retrospective study in China, 2012-2020.

BACKGROUND: Anemia is a public world health problem among pregnant females especially in developing countries. China is a representatively developing country with vast territory and significant economic and cultural differences in different regions. Currently most studies on anemia in pregnancy ignored economically backward and resource-poor cities or districts and anemia during pregnancy in China was not accurately estimated. The goal of this study was to assess the prevalence, risk factors, and adverse outcomes of anemia among pregnant females across different regions of China using a large,

Treating without guidelines: management and outcomes of lung cancer diagnosed during pregnancy - a systematic review.

BACKGROUND: Lung cancer during pregnancy is exceedingly rare and poses unique diagnostic and therapeutic challenges. Clinical decisions must balance urgent, potentially lifesaving maternal treatment with fetal safety, often in the absence of evidence-based guidelines. This systematic review aimed to characterize clinical features, treatment patterns, and maternal-fetal outcomes among reported cases of primary lung cancer diagnosed during pregnancy, focusing on case reports and case series. METHODS: We systematically searched PubMed, Embase, Scopus, and Google Scholar from 1948 through March 2

Modified letrozole vs GnRH antagonist protocols in ovarian aging women for IVF: an open-label, multicenter, randomized controlled trial.

For women with diminished ovarian reserve or of advanced age, controlled ovarian stimulation presents a significant challenge during in vitro fertilization cycles. This multi-center, open-label, randomized controlled trial enrolled 318 women with diminished ovarian reserve (defined as an antral follicle count < 5 or anti-Müllerian hormone level of 0.1-1.1 ng/mL) or advanced age (40-45 years) between 2020 and 2023. Participants were assigned to either a modified letrozole protocol (mLP, n = 159) or a gonadotropin-releasing hormone antagonist protocol (n = 159). Primary outcomes, cumulative clin

Influence of pre-eclampsia on fetal growth.

OBJECTIVE: We examined the effect of pre-eclampsia on fetal growth in a cohort of pregnant women delivering singleton infants. METHODS: Ana … 6 times more likely to deliver an SGA newborn as compared with normotensive women (95% CI 2.3-5.7). Advanced maternal age, African-American race, parity or smoking status did not modify the associations of pre-eclampsia with LBW and SGA. CONCLUSIONS: Our results are consistent with other reports that have documented a strong relationship between pre-eclampsia and rest …

The Clinical Significance of Embryonic Chromosomal Errors in Recurrent Pregnancy Loss: an Analysis of 1107 Miscarriages.

The objective of this study was to characterize the relationship between embryonic chromosomal errors in the products of conception (POC) an … spective of findings on routine evaluation. While embryonic chromosomal errors were associated with advanced maternal age, early GA of loss, and a negative routine RPL evaluation, the prevalence of chromosomal errors remained high in all subpopulations. These findings suggest that primary cytogenetic testing on POCs should be offered at the time of second and subsequen …

Inequalities in Birth Weight in Relation to Maternal Factors: A Population-Based Study of 3,813,757 Live Births.

BACKGROUND: Despite numerous studies of women having children later in life, evidence of the relationship between maternal factors and newbo … neonates was overall associated with lower maternal education. CONCLUSIONS: Regardless of parity, advanced maternal age is strongly associated with a decreased neonatal birth weight, implying complications in early pregnancy and the antenatal period as well as obstetric complications. Parental age-related aneuploidy in human germ cells and offspring: a story of past and present.

Parental age is the most important aetiological factor in trisomy formation in humans. Cytogenetic studies on germ cells reviewed here imply … or. Trisomy of individual chromosomes may remain low, linearly rise, or exponentially increase with advanced maternal age. Maternal age related trisomies involve achiasmatic and normochiasmate chromosomes, and chromosomes with disturbed recombination and distally located chiasmata. Severe maternal morbidity trends over 20 years in a tertiary referral stand-alone maternity unit.

OBJECTIVE: Severe maternal morbidity (SMM) is a better indicator of quality of care than maternal mortality, which is a rare event. Risk factors such as advanced maternal age, caesarean section (CS) and obesity are increasing. The aim of this study was to examine the rate and trends in SMM at our hospital over a 20-year period. STUDY DESIGN: Retrospective review was performed of cases of SMM from January 1st 2000 to December 31st 2019. Yearly rates for SMM and Major Obstetric Haemorrhage (MOH) were calculated (per 1000 maternities) and linear regression analysis was used to model the trends o…

Management of diabetes and pregnancy--when to start and what pharmacological agent to choose?

Gestational diabetes mellitus (GDM) complicates 3-15% of pregnancies depending upon the geographic location and ethnic groups, and its incidence is estimated to increase even further due to the increasing rates of obesity in the general population and the trend towards advanced maternal age in pregnancy. GDM is associated with adverse pregnancy outcome such as an increased rate of fetal macrosomia, neonatal metabolic disturbances, and maternal injuries. It has been shown that there is an inverse relation between maternal glycemic control and the risk of complications. When diet and exercise th…

Oxidative Stress and Reproductive Function in the Aging Male.

With the delay of parenthood becoming more common, the age at which men father children is on the rise. While the effects of advanced maternal age have been well documented, only recently have studies started to focus on the impact of advanced paternal age (APA) in the context of male reproduction. As men age, the antioxidant defense system gradually becomes less efficient and elevated levels of reactive oxygen species (ROS) accumulate in spermatozoa; this can impair their functional and structural integrity. In this review, we present an overview of how oxidative stress is implicated in male …

Prenatal genetic counselling.

Genetic concerns and indications for prenatal diagnosis are first recognized by the family physician. Review of personal, pregnancy and family history may indicate concerns beyond that of advanced maternal age. Amniocentesis is still the most frequently used modality for prenatal diagnosis, but detailed ultrasound is valuable for structural abnormalities, and chorionic villus sampling is now being tested as an alternative to amniocentesis.

Dear old dad.

The origin and frequency of spontaneous mutations that occur with age in humans have been a topic of intense discussion. The mechanisms by which spontaneous mutations arise depend on the parental germ line in which a mutation occurs. In general, paternal mutations are more likely than maternal mutations to be base substitutions. This is likely due to the larger number of germ cell divisions in spermatogenesis than in oogenesis. Maternal mutations are more often chromosomal abnormalities. Advanced parental age seems to influence some mutations, although it is not a factor in the creation of oth

Two brothers with 22q13 deletion syndrome and features suggestive of the Clark-Baraitser syndrome.

We report on two brothers with moderate-to-severe mental retardation, severe macrocephaly, obesity, characteristic face, big hands and feet, advanced bone age and brain abnormalities, including frontal cortical atrophy. These two boys resembled the two brothers described by , two maternal cousins subsequently reported by and a Brazilian boy described by . Upon further investigation, we detected a cryptic subtelomeric deletion of chromosome region 22q13, not present in either parent and probably due to a maternal germinal mosaicism. Thus, we describe the first familial case of 22q13 deletion an
