A case of Gilles de la Tourette's syndrome accompanied with alcoholism.

A case of Gilles de la Tourette's syndrome with alcoholism and migraine is described. Multiple motor tics and coprolalia were suppressed by the treatment with haloperidol. The concentration of HVA and 5-HIAA in the cerebrospinal fluid were measured and increment of HVA concentrations was outstanding while his symptoms were aggravated. This might be evidence for hyperactivity of dopaminergic neurons involved in manifest symptomatology of this syndrome.

Hypokalemic vacuolar myopathy of chronic alcoholism. A histological and histochemical study.

Recent reports have emphasized the occurrence of a myopathy in chronic alcoholism associated with hypokalemia. This report of hypokalemic myopathy in a chronic alcoholic, emphasizes the primary myopathic nature of the condition and attributes it to a possible non-specific effect of the hypokalemia on skeletal muscle. It is pointed out, that histological and histochemical changes of muscle in this type of myopathy are indistinguishable from other types of hypokalemic myopathies like periodic paralysis. It is conjectured that in alcoholic myopathy, the underlying disorder might be related to a p…

The role of physical dependence in animal models of human alcoholism.

This paper critically examines the concept that physical dependence is a necessary attribute for animal models of human alcoholism. On the basis of a review of the literature, it is argued that, since the production of physical dependence requires the presence of continuous high blood-alcohol levels, and since the production of preference for alcohol requires intermittent presentation of alcohol, the two cannot, in principle, be established in the same organism at the same time. It is further argued that physical dependence does not play a role in the development of high alcohol intake in anim…

The cost effectiveness of acamprosate in the treatment of alcoholism in Germany. Economic evaluation of the Prevention of Relapse with Acamprosate in the Management of Alcoholism (PRAMA) Study.

Alcoholism places a considerable economic burden on society. The rate of relapse of previously weaned alcoholics has been shown to decrease following treatment with acamprosate. Therefore, this study investigated the cost effectiveness of acamprosate in the treatment of alcoholism in Germany from the perspective of the German healthcare system. In this retrospective analysis of clinical data, the additional direct medical costs per additional abstinent alcoholic incurred by adjuvant acamprosate therapy of previously weaned alcoholics were quantified. In the base-case analysis, average case-rel…

Meningoencephalitis Caused by a Campylobacter fetus in a Patient with Chronic Alcoholism.

We herein report a case of Campylobacter fetus meningoencephalitis in a patient with chronic alcoholism. C. fetus is a rare cause of meningitis. The patient presented with hallucinations and monology, and alcohol withdrawal was initially suspected. After he was unsuccessfully treated for alcohol withdrawal delirium, we diagnosed C. fetus meningoencephalitis. Ampicillin monotherapy gradually improved his clinical status. A previous report stated that C. fetus infection is associated with chronic alcoholism. In patients with chronic alcoholism and disturbed consciousness, an atypical bacterial c…

Supervised Disulfiram's Superior Effectiveness in Alcoholism Treatment: Ethical, Methodological, and Psychological Aspects.

Disulfiram (DSF) causes the ALDH-mediated deterrence of alcohol consumption. We review recent meta-analyses showing the superior effectiveness of supervised disulfiram (SD) in alcoholism treatment compared with oral naltrexone or acamprosate (ACP). The success of SD is also consistent with the almost complete absence of alcoholism in Japanese homozygotes for 'inefficient' ALDH. However, SD is an underused treatment and some clinicians have ethical objections to DSF. We examine these objections and argue that they are based on a misunderstanding of how DSF works. In particular, we argue that SD…

Antisocial Alcoholism in Parents of Adolescents and Young Adults With Childhood ADHD.

Objective: Test the hypothesis that alcoholism, including antisocial alcoholism, is more prevalent among mothers and fathers of children with versus without ADHD. Method: Mothers (312 ADHD group, 235 non-ADHD group) and fathers (291 ADHD group, 227 non-ADHD group) in the Pittsburgh ADHD Longitudinal Study were interviewed along with their adolescent and young adult offspring. Results: Maternal and paternal alcoholism, with and without comorbid antisociality, was more prevalent in the ADHD group. Paternal alcoholism without antisociality was only marginally higher for probands after controlling…

Hypokalemic Paralysis Complicated by Concurrent Hyperthyroidism and Chronic Alcoholism: A Case Report.

Thyrotoxic periodic paralysis (TPP) is characterized by the presence of muscle paralysis, hypokalemia, and hyperthyroidism. We report the case of a young man with paralysis of the lower extremities, severe hypokalemia, and concurrent hyperthyroidism. TPP was suspected; therefore, treatment consisting of judicious potassium (K+) repletion and β-blocker administration was initiated. However, urinary K+ excretion rate, as well as refractoriness to treatment, was inconsistent with TPP. Chronic alcoholism was considered as an alternative cause of hypokalemia, and serum K+ was restored through vigor…

Component processes of memory in alcoholism: pattern of compromise and neural substrates.

Initially, alcohol-related memory deficits were considered only through the prism of Korsakoff's syndrome (KS). It is now clear, however, that chronic alcohol consumption results in memory disorders in alcoholics without ostensible neurologic complications, such as Wernicke's encephalopathy and KS. Most of the principal memory components are affected, including working memory, episodic memory, semantic memory, perceptual memory, and procedural memory. The extent of those cognitive impairments depends on several factors, such as age, gender, nutritional status, and psychiatric comorbidity. Whil

Neural activation during processing of aversive faces predicts treatment outcome in alcoholism.

Neuropsychological studies reported decoding deficits of emotional facial expressions in alcohol-dependent patients, and imaging studies revealed reduced prefrontal and limbic activation during emotional face processing. However, it remains unclear whether this reduced neural activation is mediated by alcohol-associated volume reductions and whether it interacts with treatment outcome. We combined analyses of neural activation during an aversive face-cue-comparison task and local gray matter volumes (GM) using Biological Parametric Mapping in 33 detoxified alcohol-dependent patients and 33 mat

Alcohol and adult neurogenesis: roles in neurodegeneration and recovery in chronic alcoholism.

The concept of "structural plasticity" has emerged as a potential mechanism in neurodegenerative and psychiatric diseases such as drug abuse, depression, and dementia. Chronic alcoholism is a progressive neurodegenerative disease while the person continues to abuse alcohol, though clinical and imaging studies show that some recovery may occur with abstinence. The neural plasticity observed in chronic alcoholism coupled with conflicting reports on alcohol-induced hippocampal neuropathology make this disease ripe for reconsideration in terms of the phenomenon of adult neurogenesis. This review d…

Cortical NAA deficits in HIV infection without dementia: influence of alcoholism comorbidity.

Alcoholism comorbidity is highly prevalent in individuals infected with human immunodeficiency virus (HIV). Each condition is known to affect brain structure, function, and metabolism, but the combined effects on the brain have only recently been considered. Single-voxel, proton MR spectroscopy (MRS) has yielded sensitive measures of early brain deterioration in the progression of HIV, but has limited coverage of neocortex, whereas MRS imaging (MRSI) can simultaneously interrogate large regions of cortex. Included were 15 men with HIV+alcoholism, nine men with HIV alone, eight men with alcohol…

Stress response dampening indexed by cortisol in subjects at risk for alcoholism.

OBJECTIVE: This study examines the stress-response-dampening (SRD) hypothesis of alcoholism from a novel vantage point. Various investigators have demonstrated that persons considered to be at risk for developing alcohol-related problems exhibit attenuation of stress reactions in psychologically challenging experimental sessions after receiving alcohol. These studies have used autonomic nervous system measures for indexing stress responses. In our report we address the question of whether people with a family history of alcoholism exhibit dampening effects of ethanol in the response of a class…

Balance and gait deficits in schizophrenia compounded by the comorbidity of alcoholism.

OBJECTIVE: Alcoholism carries a liability of balance and gait instability that persists with sobriety. Such deficits are less well documented in schizophrenia and may be compounded by comorbidity with alcoholism, which is prevalent in schizophrenia. METHOD: The authors administered quantitative ataxia tests to 10 patients comorbid for schizophrenia and alcohol dependence/abuse, 10 nonalcoholic patients with schizophrenia, 24 nonschizophrenic patients with alcohol dependence, and 27 age-matched comparison men. RESULTS: All three patient groups were impaired relative to the comparison subjects…

[The effect of medikhronal on the psychophysiological and metabolic processes in alcoholism].

Pathopsychological examination, biochemical analysis of blood and urine as well as electroencephalogram (REG) were made in patients of the second (II) stage of alcoholism. The state of higher nervous activity (HNA) before and after treatment with antialcoholic drug medichronal was also analyzed. It has been shown that the course of treatment with medichronal applied to patients with alcoholism results in the removal of the inclination to alcohol, to improvement of functional state of the central nervous system, as well as ethanol (E), acetaldehyde (AcA) and catecholamines (CA) exchange, the di…

Birth cohort differences in features of antisocial alcoholism among men and women.

BACKGROUND: This study examines the relations between birth cohort, gender, and family history of alcohol problems on alcohol dependence, an … r dependence on another drug. CONCLUSIONS: These results suggest that the prevalence of antisocial alcoholism may be increasing for both men and women. These data exemplify how societal change may affect expression of underlying vulnerability for traits thought to be genetically influenced.

Protein metabolism in alcoholism: effects on specific tissues and the whole body.

Ethanol is one of the few nutrients that is profoundly toxic. Alcohol causes both whole-body and tissue-specific changes in protein metabolism. Chronic ethanol missuse increases nitrogen excretion with concomitant loss of lean tissue mass. Even acute doses of alcohol elicit increased nitrogen excretion. The loss of skeletal muscle protein (i.e., chronic alcoholic myopathy) is one of several adverse reactions to alcohol and occurs in up to two-thirds of all ethanol misusers. There are a variety of other diseases and tissue abnormalities that are entirely due to ethanol-induced changes in the am
