Revisiting Eustachian Tube Anatomy and Physiology.

The Eustachian tube (ET) it is a complex structure connecting the middle ear cavity with the nasopharynx. Its function is to maintain middle ear ventilation to facilitate transmission of sound from the tympanic membrane to the cochlea. It is located medial to the parapharyngeal space. The ET and tympanomastoid cavities form an anatomic and functional unit that cannot easily be divided, and it plays a fundamental role in the etiology of different clinical entities. However, the underlying pathophysiological mechanisms are not yet fully understood, and further research is needed to address the g

Computational anatomy: the cerebellar microzone computation.

The cerebellum is a large brain structure. Most of the mass and volume of the cerebellum is made up by the cerebellar cortex. The outer layer of the cerebellar cortex is divided functionally into long, thin strips called microzones. We argue that the cerebellar microzone computation is the aggregate of simple unit computations and a passive effect of anatomy, unaided and unlearned, which we recreate in silico. This is likely to polarise opinion. In the traditional view, data processing by the cerebellum (stated very briefly) is the effect of learned synaptic changes. However, this has become d…

How anatomy influences measurements of snakes.

Anatomy compromises the precision and accuracy of measurements made of the body length and head size of live snakes. Body measures (snout-vent length, SVL) incorporate many synovial intervertebral joints, each allowing flexion and limited extension and compression. Radiographs of the trunk in 14 phylogenetically diverse species in resting and stretched conditions combined with dissections and histological analysis of intervertebral joints show that the synovial nature of these joints underlies the variance in SVL measures. Similarly, the ubiquity and variety of viscoelastic tissues connecting …

Molecular anatomy of adult mouse leptomeninges.

Leptomeninges, consisting of the pia mater and arachnoid, form a connective tissue investment and barrier enclosure of the brain. The exact nature of leptomeningeal cells has long been debated. In this study, we identify five molecularly distinct fibroblast-like transcriptomes in cerebral leptomeninges; link them to anatomically distinct cell types of the pia, inner arachnoid, outer arachnoid barrier, and dural border layer; and contrast them to a sixth fibroblast-like transcriptome present in the choroid plexus and median eminence. Newly identified transcriptional markers enabled molecular ch

Review of olfactory cleft roof anatomy.

Progress in skull-base endoscopic surgery has improved our knowledge of olfactory cleft (OC) anatomy. This article presents a review of current knowledge concerning the OC roof: descriptive anatomy, radiological exploration, and endoscopic observation. The OC is a narrow area in the most superior part of the nasal cavity. Its roof is the thinnest structure separating the nasal cavities from the brain; it comprises three superimposed tissues: nasal mucosa, ethmoid cribriform plate (ECP), and dura mater. The ECP comprises the anterior ECP containing the ethmoidal slit (ES) medially and the cribr…

Surgical Anatomy for Extended Facelift Techniques.

Preservation of the facial nerve is crucial in any type of facial procedure. This is even more important when performing plastic surgery on the face. An intricate knowledge of the course of the facial nerve is a requisite prior to performing facelifts, regardless of the technique used. The complex relationship of the ligaments and the facial nerve may put the nerve at an increased risk of damage, especially if its anatomy is not fully understood. There are several danger zones during dissection where the nerve is more likely to be injured. These include the areas where the nerve branches becom…

Peritoneal innervation: embryology and functional anatomy.

The parietal peritoneum (PP) is innervated by somatic and visceral afferent nerves. PP receives sensitive branches from the lower intercostal nerves and from the upper lumbar nerves. Microscopically, a dense network of unmyelinated and myelinated nerve fibers can be found all over the PP. The unmyelinated fibers are thin and are ending just underneath the PP. The myelinated fibers can penetrate the PP to reach the peritoneal cavity, where they lose their myelin sheath and are exposed to somatic and nociceptive stimuli. PP is sensitive to pain, pressure, touch, friction, cutting and temperature

Acquired portosystemic collaterals: anatomy and imaging.

Portosystemic shunts are enlarged vessels that form collateral pathological pathways between the splanchnic circulation and the systemic circulation. Although their causes are multifactorial, portosystemic shunts all have one mechanism in common-increased portal venous pressure, which diverts the blood flow from the gastrointestinal tract to the systemic circulation. Congenital and acquired collateral pathways have both been described in the literature. The aim of this pictorial essay was to discuss the distinct anatomic and imaging features of portosystemic shunts, as well as to provide a rob

What is normal bladder neck anatomy?

INTRODUCTION AND HYPOTHESIS: Functional anatomy of the bladder neck and proximal urethra has been studied extensively because of the belief that it is important for urinary continence. The aim of this study was to explore the limits of normality for pelvic floor ultrasound parameters of bladder neck and urethral mobility associated with stress urinary incontinence (SUI) and urodynamic stress incontinence (USI). METHODS: A retrospective study was conducted on 589 women seen for urodynamic testing in a tertiary urogynaecology clinic. All women were assessed following a protocol including interv…

Chapter 1: Sinonasal anatomy and function.

An understanding of paranasal sinus anatomy based on important fixed landmarks rather than variable anatomy is critical to ensure safe and complete surgery. The concept of the paranasal surgical box defines the anatomic limits of dissection. The boundaries of the surgical box include the middle turbinate medially, orbital wall laterally, and skull base superiorly. The "vertical component" of the surgical box defines the boundaries of the frontal recess and includes the middle turbinate and intersinus septum medially, medial orbital wall and orbital roof laterally, nasofrontal beak anteriorly, …

Patellofemoral anatomy and biomechanics: current concepts.

The patellofemoral joint, due to its particular bone anatomy and the numerous capsuloligamentous structures and muscles that act dynamically on the patella, is considered one of the most complex joints in the human body from the biomechanical point of view. The medial patellofemoral ligament (MPFL) has been demonstrated to contribute 60% of the force that opposes lateral displacement of the patella, and MPFL injury results in an approximately 50% reduction in the force needed to dislocate the patella laterally with the knee extended. For this reason, recent years have seen a growing interest i…

Atrio-hisian fibers anatomy and electrophysiology.

INTRODUCTION: In this case, electrophysiology and histology could be studied in the same heart. Clinical investigation, clinical electrophysiology, and postmortem serial histological sections of the septum were analyzed. METHODS: A patient with repeated seizures and a short PR interval with narrow QRS complex underwent electrophysiologic studies. RESULTS: The patient died while experiencing a very rapid supraventricular tachycardia and histologic examination showed a atrio-hisian bypass tract. CONCLUSION: In our study, the lack of lengthening of the PR interval in spite of progressively pre
