Atypical Bourneville sclerosis without epilepsy and mental retardation: case report and literature review.

Twenty-four-year-old woman without familiar detected signs of Bourneville's disease or tuberous sclerosis complex (TSC) was diagnosed with t … g the lateral ventricles and the posterior part of the third ventricle. The patients present facial angiofibromas, but from the classical triad of the disease, the epilepsy and mental retardation were absent, the patient never presented seizures. The total removal of the tumor (peace to peace) was performed surgically, the macroscopic features of resected tumo …

Delayed Diagnosis of Tuberous Sclerosis Complex: A Case Report.

Tuberous sclerosis complex (TSC) is a multisystem genetic disorder with variable clinical expression. We report the case of a 26-year-old woman who presented with characteristic cutaneous findings, including facial angiofibromas, periungual fibromas, and a fibrous cephalic plaque. Further evaluation revealed renal angiomyolipomas and retinal astrocytic hamartomas. Despite a history of childhood seizures, the diagnosis was delayed until adulthood. This case highlights the role of dermatological examination in identifying TSC, particularly in patients with subtle or asymptomatic systemic involve…

Spindle Cell Neoplasms Unique to the Sinonasal Tract.

The spindle cell neoplasms include sinonasal tract angiofibroma (STA), glomangiopericytoma (GPC), and biphenotypic sinonasal sarcoma (BSNS). These entities share some clinical, histomorphologic, immunohistochemical, and even molecular characteristics. Nevertheless, there are features that are unique to each tumor type. STA shows heavily collagenized and vascular stroma, with apparent relationship to underlying hormonal changes. GPC consistently exhibits monomorphic, ovoid cytologic appearance with the constant feature of Beta-catenin nuclear accumulation by IHC. Lastly, BSNS is a deceptively b…

Primary ocular presentation of tuberous sclerosis - A case report.

A 25-year-old man presented with decreased vision in the left eye with hypopigmented elevated subretinal lesion over the optic disk with abnormal vasculature, subretinal and retinal hemorrhages, and fluid in the macula. An area of high spike over the disk with corresponding orbital shadowing was seen on B scan ultrasonography. Fundus fluorescein angiography revealed abnormal vasculature. Systemic examination revealed facial angiofibroma, ashleaf spot, and dental pits with multiple cortical tubers on CT brain. Intravitreal injection of bevacizumab led to visual and tomographic improvement. Abno…

Small-cell neuroendocrine carcinoma originating from the lateral nasopharyngeal wall.

A 40-year-old man presented with right-sided nasal blockage and epistaxis of 18 months' duration. Examination revealed a pinkish mass in the right nasal cavity and nasopharynx. Initial biopsy was suggestive of angiofibroma. Complete excision through a lateral rhinotomy approach was performed. The postoperative histopathologic report was suggestive of sinonasal neuroendocrine carcinoma, which was confirmed by immunohistochemistry. To the best of our knowledge, this is the first reported case of sinonasal neuroendocrine carcinoma arising from the lateral wall of the nasopharynx in the English li…

Radiology of the nasopharynx: pictorial essay.

The nasopharynx, located just below the central skull base, is a difficult area to evaluate clinically. Endoscopic examination provides useful information but cannot delineate submucosal disease. Benign lesions of the nasopharynx are relatively uncommon. They include Thornwaldt cyst, juvenile angiofibroma, haemangioma, haemangiopericytoma, Kimura's disease, branchial cleft cyst, oncocytoma, amyloidoma, and non-ossifying fibromyxoid tumours. The most common malignant lesions are carcinomas and lymphomas. Other malignant neoplasms such as adenocystic carcinomas are rarely seen. The purpose of th…

A Triple Laser Combination Treatment for Facial Angiofibromata Management in Tuberous Sclerosis and Literature Review.

Angiofibromas are a common facial manifestation of tuberous sclerosis (TS). However, current treatments have proven ineffective due to high recurrence rates and noncompliance. To address this issue, we developed a new triple laser therapy protocol for more effective management of angiofibromas. We conducted tests to validate its efficacy. This is a prospective study of 10 patients with TS (4 women and 6 men, mean age 26.3 years [15-37 years]) with angiofibromata who received triple sequential laser therapy at our private dermatological clinic conducted from January 2000 to December 2022. We ev…

Topical Use of Mammalian Target of Rapamycin (mTOR) Inhibitors in Tuberous Sclerosis Complex-A Comprehensive Review of the Literature.

BACKGROUND: Tuberous sclerosis complex is a genetically determined multisystem disorder that may affect almost any human organ. The discover … oved to be effective in the treatment of skin lesions related to tuberous sclerosis complex. Facial angiofibromas were the target lesions in most instances. Few studies reported clinical improvement of hypomelanotic macules. These drugs directly address the molecular defect related to tuberous sclerosis complex manifestations. CONCLUSIONS: Currently available …

Tuberous sclerosis: histological analysis with confocal laser scanning microscope of gingival angiofibromatosis.

INTRODUCTION: Tuberous sclerosis (TS) is an autosomal dominant neuro-cutaneous syndrome characterized by multiple hamartomas in various organs, especially on skin and central nervous system. The most common features of TS include facial angiofibromas, hypomelanotic cutaneous macules, shagreen patches in the lumbar area, cerebral cortical tubers, sub-ependymal nodules, sub-ependymal giant cell astrocytomas, cardiac rhabdomyomas, and renal angiomyolipomas. Frequently oral manifestations such as fibrous hyperplasia, angiofibromas and dental enamel pitting are also observed. The aim of this case r…

Mesenchymal-epithelial interactions involving epiregulin in tuberous sclerosis complex hamartomas.

Patients with tuberous sclerosis complex (TSC) develop hamartomas containing biallelic inactivating mutations in either TSC1 or TSC2, resulting in mammalian target of rapamycin (mTOR) activation. Hamartomas overgrow epithelial and mesenchymal cells in TSC skin. The pathogenetic mechanisms for these changes had not been investigated, and the existence or location of cells with biallelic mutations ("two-hit" cells) was unclear. We compared TSC skin hamartomas (angiofibromas and periungual fibromas) with normal-appearing skin of the same patient, and we observed more proliferation and mTOR activa…

Angiomyofibroblastoma of the cervix uteri: a case report.

BACKGROUNDS: Angiomyofibroblastoma (AMFB) is a rare histopathologic finding of the female lower genital tract. This tumor belongs to the group of mesenchymal tumors. Mesenchymal neoplasms of the modified genital skin and mucosa are uncommon. The majority of these lesions are seen in females and, collectively, they form a family of vulvovaginal soft tissue tumors. This family includes fibroepithelial stromal polyps, angiomyofibroblastoma, cellular angiofibroma, aggressive angiomyxoma, vaginocervical myofibroblastoma, vulvar leiomyomatosis, and other smooth muscle tumors. Angiomyofibroblastoma i…

[Embolization of the branches of the external carotid artery].

Authors present their own experience with the embolization of the external carotid artery branches. The main indications to embolization included well vascularized and hemorrhagic brain tumors in facial part of the skull, mainly meningiomas, juvenile angiofibromas, and angioneuromyomas. Embolization of external carotid artery was performed in 15 patients. Complete impatience of blood vessel supplying the tumor was achieved in 12 cases, but it was incomplete in 3 cases. Single serious complication in the form of hemiplegia was noted. There were also mild complications in 5 patients, which did n…

[Neural endoscopy in hydrocephalus treatment in a case of tuberous sclerosis].

Tuberous sclerosis is a rare, autosomal dominant neurocutaneous disease. The disease can also result from spontaneous mutations. Manifestations of this disease are typically characterized by involvement of the CNS (early childhood seizures, multiple brain tumours), skin (facial angiofibromas, Pringle's birthmarks), kidneys and liver (angiomyolipomas), heart and others. We present a case of 26 years old patient with brain tumour, with clinical and radiological signs of hydrocephalus due to a cystic lesion in the III ventricle. The patient was operated upon using neuroendoscopy, with clinical an…

Rigid nasal endoscopy in the diagnosis and treatment of epistaxis.

BACKGROUND AND OBJECTIVES: Epistaxis is one of the common symptoms encountered in the Otorhinolaryngology department. Many times the cause f … yps, enlarged and congested adenoids, scabs or crusts in the crevices of the lateral nasal wall and angiofibroma. Endoscope also helps in the treatment of epistaxis, which includes endoscopic selective nasal packing using gelfoam, endoscopic cautery or diathermy and endoscopic polypectomy. Other patients with adenoids, scabs and crusts and angiofibroma were ma …

[Pulmonary lymphangioleiomyomatosis (LAM)].

Lymphangioleiomyomatosis (LAM) is a rare lung disease that mostly occurs in female patients. A total of 200-400 people are assumed to be infected in Germany. A sporadic form and a form associated with the tuberous sclerosis complex (TSC) can be separated. Mutations of the TSC‑1 and TSC‑2 genes are relevant. Morphologically, pulmonary multicysts and marginal micronodal proliferations of LAM cells are characteristic. Combinations with renal angiomyolipoma are typical and, in cases with TSC glioma, facial angiofibroma and ungual fibroma are seen. Prognosis is favorable (10-year survival: 80%) and…

[Epistaxis in children].

Most epistaxis in children come from the anterior part of the nasal septum. The bleeding is usually under control by manual compression or vestibular packing with a piece of cotton or calcium alginate. In case of severe or recurrent epistaxis, one must look for a bleeding disorder or a nasal/nasopharyngeal tumour, especially juvenile nasopharyngeal angiofibroma in adolescent males. Chemical or electrical cautery of the septum should be suggested in case of recurrent idiopathic epistaxis. When bleeding continues despite such treatments, anterior nasal packing is recommended; life-threatening ep…
