BACKGROUND: Anterior ankle impingement with and without ankle osteoarthritis (OA) is a common condition. Bony impingement between the distal tibia and talus aggravated by dorsiflexion has been well described. The etiology of these impingement lesions remains controversial. This study describes a cam-type impingement of the ankle, in which the sagittal contour of the talar dome is a non-circular arc, causing pathologic contact with the anterior aspect of the tibial plafond during dorsiflexion, leading to abnormal ankle joint mechanics by limiting dorsiflexion.
METHODS: A group of 269 consecuti…

Injuries to the lateral ankle ligaments are the most common sports injuries. Determination of their severity and exclusion of relevant accompanying injuries requires a subtle clinical and a focussed radiological assessment. Treatment is non-operative and functional in the majority of cases. Consequent application of orthoses limiting supination and proprioceptive training are essential to avoid chronic instability. With recurrent ankle sprains one has to distinguish between functional and mechanical instability. The latter can be treated successfully with anatomic reconstruction and ligamentop…

Ankle impingement and instability are well-recognized complications of ankle sprain. Ankle impingement is an important cause of chronic ankle pain in active populations, particularly in the professional athlete. Depending on anatomical location and the structures involved, impingement syndromes are classified into anterolateral, anterior, posterior, posteromedial, and anteromedial types. Clinically, impingement syndromes are characterized by painful limitation of full-ankle movement. Ankle impingement results from repetitive subclinical trauma due to overuse injuries, which, in the subacute or…

Despite the tendency for injuries of the foot and ankle to be readily localized at physical examination, the examiner is advised to perform an ultrasound examination when indicated with a routine and systematic approach. This allows for comprehensive evaluation of all the relevant structures and affords the necessary experience to recognize abnormal anatomy when it is present. Musculoskeletal ultrasound examination strongly complements other diagnostic tools used in the diagnosis and treatment of foot and ankle pathology.

Malunions are fractured bones that have healed in pathological positions. This leads to nonphysiological load transfer. Clinical symptoms at the ankle may include swelling, pain and impaired function. Lateral, posterolateral or posteromedial subluxation of the talus will be visible on the radiographs. Surgical correction may be indicated if the malunion is symptomatic. Different osteotomies have been described, but the goal of surgery will always be the reduction and retention of the subluxed talus in an effort to recreate stable conditions. Eighty percent of patients show good results with si…

CONTEXT: Medial impingement syndrome of the ankle is common in the athletic population. A marginal osteophyte on the leading edge of the medial talar facet and a corresponding "kissing" osteophyte on the tibia, in front of the medial malleolus, may abut and cause pain and limited dorsiflexion.
BACKGROUND: Palpation of the talar osteophyte and standard imaging-especially, the oblique view of the foot-are useful in making the diagnosis. Surgical removal of the osteophyte may be necessary.
CONCLUSIONS: Ankle impingement is commonly seen in running and jumping sports, especially if the athlete h…

Charcot neuroarthropathy is a common cause of morbidity in persons with diabetes mellitus and sensory neuropathy. Although Charcot neuroarthropathy is rare, it likely will become more prevalent in conjunction with increased incidence of diabetes mellitus. Prevention of disease progression remains the mainstay of treatment, with surgical intervention usually reserved for refractory cases. Late deformities are often complicated by chronic ulceration, infection, and osteomyelitis. The clinical presentation is best summarized with the Eichenholtz classification, and progression often follows a pre

Ankle arthroscopy provides the surgeon with a minimally invasive treatment option for a wide variety of indications such as impingement, osteochondral defects, loose bodies, ossicles, synovitis, adhesions, and instability. Posterior ankle pathology can be treated using endoscopic hindfoot portals. These posteromedial and lateral hindfoot portals provide excellent access to the posterior aspect of the ankle and subtalar joint. Also extra-articular structures in the hindfoot, for instance recurrent peroneal tendon dislocation, can be treated by creating an additional portal. The endoscopic hindf…

Ankle inversion injuries are the most common traumatic injuries in dancers. Ankle stability is integral to normal mobilization and to minimizing the risk for ankle sprain. The ability of the dynamic and static stabilizers of the ankle joint to maintain their structural integrity is a major component of the normal gait cycle. In the world of dance, this quality assumes even greater importance given the range of movement and stresses imposed on the ankle during various dance routines.

Foot and ankle injuries are among the most common of all sports-related injuries. Many injuries are simplistic from a diagnostic standpoint, whereas others require a high level of suspicion and the use of appropriate diagnostic imaging modalities. Whether a professional athlete or a weekend warrior, all athletes want minimal disruption to their sporting activity and a quick return to competition. An accurate diagnosis aids in the development of an appropriate rehabilitation program and in return to activity. This article reviews the more common foot and ankle injuries seen in the podiatrist's …

OBJECTIVE: Joint-preserving osteotomies in patients with symptomatic varus malalignment of the ankle. Osseous procedures together with soft-tissue rebalancing are performed at the level of the distal tibia/fibula or calcaneus. Realignment of the joint line will subsequently reduce intraarticular forces at the ankle and thereby slow down the degenerative process.
INDICATIONS: Varus malalignment of the ankle with remaining cartilage in the tibiotalar joint.
CONTRAINDICATIONS: Severe neurovascular deficiencies. Severe instability of the hindfoot. Severe cartilage lesions of the entire ankle (gr…

A 53 year old male patient suffering from painful and swollen ankles showed a large enhancement on skeletal scintigraphy. The DXA bone density values for the neck of the femur and the lumbar vertebrae indicated severe osteoporosis. The conventional diagnostic x-ray revealed stress fractures of both distal tibiae (Looser's transformation zones). The patient had been treated with glucocorticoids for many years because of a liver transplantation. A further risk factor was impaired renal function with a hypovitaminosis D3 and secondary hyperparathyroidism.

Phaeohyphomycosis is a rare mycotic opportunist disease witch usually affects immunodepressed patients. Infection follows a traumatic inoculation of fungi into the skin with formation of a subcutaneous cyst. We report the case of a 20-year-old woman presenting with a subcutaneous, painless, and recurrent cystic lesion on the right ankle for 9 years. A surgical excision was performed. Histopathologic examination showed a granuloma with central abscess surrounded by a thick fibrous tissue. After staining with PAS, septate hyphae and spores were found in the granuloma, consistent with a phaeomyco…

This article describes the relevant surgical detail and MR imaging appearance of common operations performed in the foot and ankle. To evaluate postsurgical patients critically, it is important to understand the primary clinical diagnosis, surgical treatment undergone, the interval since surgery, and patients' current clinical symptoms. Radiography is the most common imaging modality for evaluation of the postoperative ankle and foot. MR imaging may be useful for evaluating the soft tissues and osseous structures in the postsurgical foot and ankle.
