Bacteriuria in pregnancy.

The urinary tract undergoes profound physiologic and anatomic changes during pregnancy that facilitate the development of symptomatic urinary tract infections in women with bacteriuria. While the adverse effects of asymptomatic bacteriuria on maternal and fetal health continue to be debated, it is clear that asymptomatic bacteriuria is the major risk factor for developing symptomatic urinary tract infection and that symptomatic infections are associated with significant maternal and fetal risks. Because the majority of symptomatic urinary tract infections develop in women with bacteriuria earl

[Primary vesico-ureteral reflux. Current data and perspectives].

Primary vesicoureteral reflux is a frequent condition threatening the renal parenchyma. This severe disease can lead to renal dysplasia and scarring. When revealed by acute urinary tract infection, parenchymal damage has, in most cases, already occurred so medical or surgical care comes too late. Ideally, the population should be screened at birth for vesicoureteral reflux, but unfortunately, no biological test is yet available. At the present time, it would be reasonable to propose cystography to "at risk" populations, first in newborns with pyelocaliceal dilation detected antenatally then in

[Pseudomonas aeruginosa as a cause of urinary tract infections].

The aim of this study was to investigate risk factors for urinary tract infection with Pseudomonas aeruginosa, the course of such infection and the therapeutic approach among doctors. A total av 56 patients were included. Identified risk factors were old age, institutionalization, obstruction or other dysfunction of the urinary tract, use of urinary tract catheters and treatment with broad spectrum antibiotics. Urinary tract infection with P aeruginosa has a tendency to persist or relapse. The therapeutic approach varied widely.

Effect of empiric antibiotics against Pseudomonas aeruginosa on mortality in hospitalized patients: a systematic review and meta-analysis.

BACKGROUND: Empiric antibiotics active against Pseudomonas aeruginosa are recommended by professional societies for certain infections and are commonly prescribed for hospitalized patients. The effect of this practice on mortality is uncertain. METHODS: A systematic literature search was conducted using Embase, Medline, PubMed, Web of Science, Cochrane, Scopus and Google Scholar from earliest entry through 9 October 2023. We included studies of patients hospitalized with P. aeruginosa infections that compared mortality rates depending on whether patients received active empiric antibiotics.

Aerococcus mictus as a cause of feline urinary tract infection: a case report.

BACKGROUND: Bacterial urinary tract infections (UTIs) are a noteworthy cause of lower urinary tract disease both in humans and companion animals, especially in patients with systemic comorbidities or underlying anatomical or functional urinary tract abnormalities. Aerococcus species are increasingly recognised as opportunistic uropathogens in human medicine; however, their role in UTIs in companion animals remains poorly characterised. CASE PRESENTATION: The case of a neutered male European Shorthair cat with a history of antepubic urethrostomy and recurrent lower urinary tract signs, in which

[Infectious diseases in older adults: Key considerations for common infections in outpatients].

The article highlights the increased incidence, severity, and mortality of bacterial infectious diseases in older adults, often presenting atypically and complicating diagnosis. Aging-related immune changes (immunosenescence) and chronic conditions heighten susceptibility and worsen outcomes. Pneumonia and urinary tract infections are most common, with diagnostic challenges due to nonspecific symptoms like confusion or functional decline. Effective prevention, timely diagnosis, and tailored treatment, including rational antibiotic use, are critical to improve prognosis in this vulnerable popul

Minimum inhibitory concentration distribution of Mecillinam in clinical Staphylococcus saprophyticus isolates from Europe.

OBJECTIVES: Staphylococcus saprophyticus (S. saprophyticus) is the second most common bacteria causing uncomplicated urinary tract infections (UTIs). It is considered non-susceptible to mecillinam, with no defined breakpoint and only few available minimal inhibitory concentration (MIC) observations. However, this consideration does not correlate with clinical outcome. With this study, we aimed to provide a comprehensive MIC distribution analysis of mecillinam for S. saprophyticus, which could be useful for determining potential breakpoints. METHODS: We studied 112 isolates of S. saprophyticus

[Shortened antimicrobial therapy duration].

SHORTENED ANTIMICROBIAL THERAPY DURATION. Antibiotic treatment durations represent an important field of current clinical research. Indeed, shortening antibiotic duration during bacterial pathologies has several advantages: reducing the emergence of resistance on an individual and collective scale, reducing costs, adverse effects and the environmental impact. However, a rigorous investigation is necessary to properly assess the absence of impact on the individual prognosis. In recent years, several randomized trials have made it possible to validate short durations of antibiotic therapy for fr

Molecular determination of O25b/ST131 clone type among extended spectrum β-lactamases production Escherichia coli recovering from urinary tract infection isolates.

BACKGROUND: Escherichia coli (E. coli) O25b/ST131 clone causes urinary tract infection (UTI) and is associated with a broad spectrum of other infections, such as intra-abdominal and soft tissue infections, that can be affecting bloodstream infections. Therefore, since O25b/ST131 has been reported in several studies from Iran, in the current study, we have investigated the molecular characteristics, typing, and biofilm formation of O25b/ST131 clone type E. coli collected from UTI specimens. METHODS: A total of 173 E. coli isolates from UTI were collected. The susceptibility to all fourth gener

Association between Escherichia coli with NotI-restriction resistance and urinary tract infections.

BACKGROUND: Escherichia coli is the most common cause of urinary tract infections (UTIs). It is widely accepted that uropathogenic E. coli (UPEC) mainly emerge from the distal gut microbiota. Identification of bacterial characteristics that are able to differentiate UPEC from fecal commensal strains will facilitate the development of novel strategies to detect and monitor the spread of UPEC. METHODS: Fifty fecal commensal, 83 UTI-associated and 40 biliary tract infection (BTI)-associated E. coli isolates were analyzed. The NotI restriction patterns of chromosomal DNA in the isolates were dete

Efficacy and safety of levofloxacin as a treatment for complicated urinary tract infections and pyelonephritis.

INTRODUCTION: Urinary tract infections (UTIs) are among the most common causes of sepsis presenting to hospitals. Treating complicated UTIs is extremely important due to their potential mortality. Levofloxacin is a fluoroquinolone antibacterial that has become one of the cornerstones of antibiotic therapy of complicated UTIs and pyelonephritis since its introduction in the 1990s because of its exceptional pharmacokinetic (PK) and pharmacodynamic (PD) profile. However, the emergence of widespread fluoroquinolone resistance over the past decade has prompted investigators to reexamine its place i

The First Report of Phenotypic and Molecular Characterization of Extended-Spectrum Beta-Lactamase-Producing Uropathogens in Sikkim and Darjeeling Hills of India.

Extended-spectrum β-lactamase (ESBL)-producing bacteria are a global health threat both in hospital and in community settings. The emergence of these organisms poses major difficulty in treating infections. This study was carried out to assess major ESBL-producing uropathogens in female patients of Sikkim and Darjeeling by phenotypic and genotypic methods. Out of 1,516 urine samples, 454 uropathogens were isolated with a prevalence rate of 29.94%. Among them, Escherichia coli (74.3%) was the predominant type followed by Klebsiella pneumoniae (20.1%), Pseudomonas aeruginosa (2.4%), and Proteus

[Lower urinary tract infections in men. Urethritis, cystitis and prostatitis].

Recommendations for the treatment of lower non-catheter-related urinary tract infection (UTI) in men are rarely evidence-based. Their management requires the localization of the site of infection, whether it be the urethra, bladder or prostate, and includes antibiotic therapy and in most cases urological assessment. They are often associated with urinary tract procedures or anatomical or functional abnormalities. Nearly 80 % of male UTIs are caused by Enterobacteriaceae. The prevalence of broad-spectrum beta-lactamase-producing strains (ESBL) and quinolone-resistant strains is increasing. The

Urinary tract infections and bacterial prostatitis in men.

PURPOSE OF REVIEW: The purpose of this review is to highlight advances in research on urinary tract infections (UTIs) and bacterial prostatitis in men in the preceding year. RECENT FINDINGS: The antiseptic properties of the prostate secretions might be an important factor for prevention of recurrency. Risk factors for UTI in men include prostate enlargement and urological interventions, such as transrectal prostate biopsy. Preventive treatment of prostate enlargement has been demonstrated to reduce frequency of UTI. Ensuing infections after prostate biopsy, such as UTI and bacterial prostatit

Duration of antibiotic treatment for acute pyelonephritis and septic urinary tract infection-- 7 days or less versus longer treatment: systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Acute pyelonephritis is a frequent cause of morbidity, with a wide variation in duration of therapy. We performed a systematic review of all randomized controlled trials (RCTs) comparing ≤7 days treatment with a longer course. METHODS: Electronic databases were searched to identify RCTs that assessed adults treated for pyelonephritis, comparing a 7 day or shorter versus longer therapy. Primary outcome was clinical failure at the end of the long treatment arm (EOT). Secondary outcomes included clinical failure at the end of follow-up (EOF), microbiological failure, all-cause mortal
