Antibiotic prophylaxis in non-cardiac thoracic surgery. A double-blind study of penicillin vs. cefuroxime.

Patients referred for elective pulmonary surgery were enrolled in a prospective, randomized, double-blind study comparing the prophylactic efficacy of four-dose regimens of penicillin-G 5 million IU and cefuroxime 1.5 g. the first dose given immediately preoperatively. The treatment groups were comparable preoperatively with regard to all tested demographic factors. No significant intergroup difference was found concerning postoperative empyema, wound infection, septicaemia, pneumonia or fever of unknown origin, or any other complication or parameter tested in connection with pulmonary surgery

[Antibiotic prophylaxis in non-elective cesarean section with single-dose imipenem versus multiple-dose cefotaxime].

Antibiotic prophylaxis reduces the incidence of febrile morbidity in women undergoing non-elective cesarean section. A randomized comparison of prophylactic single dose Imipenem, a carbapenem antibiotic with the broadest antimicrobial spectrum of activity of any antibiotic, with multiple doses Cefotaxime, a third generation cephalosporin, was undertaken in 48 women who underwent non-elective cesarean section (22 receiving Imipenem, 26 Cefotaxime). The incidence of febrile morbidity in the Imipenem group was 4.54% versus 7.69 in patients receiving multiple doses of Cefotaxime. There were non se…

Endometrial and vaginal cuff bacteria recovered at elective hysterectomy during a trial of antibiotic prophylaxis.

The utility of culturing the cut edge of the vaginal cuff and endometrium of the uterine specimens of patients undergoing elective hysterectomy and receiving antibiotic prophylaxis has been critically examined. Subjects for this study represent a randomized subpopulation of patients participating in a larger controlled, prospective, double-blind study of antibiotic prophylaxis which has been conducted at Yale-New Haven Hospital during the past five years. On the basis of the organisms recovered, speculation is raised concerning the existence of an endometrial flora and the mechanisms of postop…

ASSOCIATION BETWEEN ANTIBIOTIC PROPHYLAXIS AND ENDOPHTHALMITIS AFTER INTRAVITREAL INJECTION: A Systematic Review and Meta-Analysis.

PURPOSE: To compare the incidence of endophthalmitis after intravitreal injection with versus without topical antibiotic prophylaxis. METHODS: We systematically searched PubMed, Embase, and the Cochrane Library from each database's inception through August 2024. The primary outcome measure was the endophthalmitis rates after intravitreal injection, whereas the secondary outcomes included the distribution and quantity of microorganisms and best-corrected visual acuity changes. Study quality was assessed using the Newcastle-Ottawa Scale. For the meta-analysis, pooled summary estimates were calc…

Global practices in antibiotic prophylaxis for pancreatoduodenectomy: insights from an international survey among HPB surgeons.

BACKGROUND: Optimizing antibiotic prophylaxis might improve outcomes after pancreatoduodenectomy. Current guidelines lack clear recommendations regarding the type and duration of prophylaxis. This survey evaluated global practices for antibiotic prophylaxis in pancreatoduodenectomy. METHODS: An online survey among members of international surgical associations was distributed from April to September 2024. Sixteen questions addressed intraoperative and prolonged antibiotic prophylaxis practices. Responses were analyzed descriptively and compared across continents and center volumes. RESULTS: …

Prospective observational comparative study on antibiotic prophylaxis in cystoscopy: towards a rational use of antimicrobials.

INTRODUCTION: Antibiotic resistance represents a major public health problem driven by inappropriate antimicrobial use. Cystoscopy is a common urological procedure associated with a low risk of urinary tract infection (UTI), and the role of routine antibiotic prophylaxis remains controversial. OBJECTIVE: To evaluate the effectiveness of antibiotic prophylaxis in preventing post-cystoscopy UTI. MATERIALS AND METHODS: A prospective observational comparative study including 297 patients undergoing outpatient cystoscopy. Patients were assigned to Group A (no prophylaxis, n=161) and Group B (cefu…

Efficacy of Antibiotic Prophylaxis in Preventing Infections Following Frontal Sinus Fracture Management: A Retrospective Analysis.

Frontal sinus fractures (FSF) can lead to detrimental intracranial infections, yet there is variability in the literature regarding the efficacy of antibiotic prophylaxis. This study aims to provide guidance on the role of prophylactic antibiotics in mitigating infection following FSF. A retrospective cohort study was conducted from January 2018 to December 2022 comparing the incidence of infection between patients managed with versus without antibiotics. Patients with FSF diagnosed by CT scan and at least 18 years old were included. Our primary outcome was the incidence of infection following…

Interventions for replacing missing teeth: antibiotic prophylaxis for dental implant placement without bone augmentation procedures.

RATIONALE: Some dental implant failures may result from bacterial contamination at implant insertion, and biomaterial-related infections are difficult to treat. Antibiotic prophylaxis is generally indicated for patients at risk of infective endocarditis or with reduced host response, surgery at infected sites, extensive or prolonged procedures, or implantation of large foreign materials. Various prophylactic regimens, including short-term protocols, have been proposed to reduce post-implant infections. However, concerns about adverse events and antibiotic resistance make prophylactic antibioti…

Self-disinfecting adhesive chitosan nanospray therapeutics for on-demand antibiotic prophylaxis against healthcare-associated infections.

Healthcare-associated infections (HAIs) present a critical clinical challenge that significantly increases patient morbidity and healthcare costs. While pH-responsive antibacterial nanoparticles (NPs) have emerged as promising therapeutic platforms, current strategies are hampered by poor tissue adhesion and premature drug release under physiological conditions. Here, we propose sprayable adhesive antibacterial NPs based on catechol-conjugated chitosan with dynamic Cu2+ coordination for infection-triggered vancomycin release in HAI prevention. This multifunctional therapeutic system uniquely i

Rethinking Antibiotic Prophylaxis for Spontaneous Bacterial Peritonitis in Patients With Cirrhosis: First, Do No Harm.

Antibiotic prophylaxis for spontaneous bacterial peritonitis (SBPPr) in patients with cirrhosis has been considered the standard of care since the 1990s and is currently recommended by several major gastroenterological societies. However, the evidence that supports this practice is weak, and there is no clear mortality benefit. The unintended consequences of this strategy are not insignificant, both at the patient and population levels. Recent evidence suggests that SBPPr may even cause harm. Since the widespread implementation of SBPPr 3 decades ago, our overall approach to antibiotic use has…

Antibiotic Prophylaxis for Dental Procedures Following Total Joint Arthroplasty: A Systematic Review and Meta-Analysis.

BACKGROUND: Periprosthetic joint infection (PJI) is a serious complication following total joint arthroplasty (TJA). Hematogenous spread, including from dental procedures, is a proposed mechanism for PJI. While prophylactic antibiotics (PAs) have traditionally been recommended before dental procedures in patients with joint prostheses, recent guidelines question their effectiveness. This study aimed to evaluate the association between dental antibiotic prophylaxis and PJI risk through a systematic review and meta-analysis. METHODS: A systematic literature search was conducted using PubMed, Em…

Perioperative multidrug-resistant bacteria impair clinical outcome after pancreatic surgery: Missed targets of antibiotic prophylaxis.

BACKGROUND: Multidrug-resistant bacteria represent major concerns in perioperative medicine. The extent to which preoperative colonization and postoperative detection of multidrug-resistant bacteria affect outcome in pancreatic surgery is largely unknown. Microbiome analyses provide insights into appropriate perioperative antibiotic prophylaxis. METHODS: This was a retrospective, single-center cohort study of patients who underwent elective pancreatic surgery between January 2018 and December 2022 with a complete set of preoperative screening swabs. RESULTS: Of 212 patients, n = 168 (79.25%)…

Current Practices in Antibiotic Prophylaxis for Transoral Endoscopic Thyroid and Parathyroid Surgery: A Comparative Study.

Background and Objectives: The transoral endoscopic thyroidectomy-vestibular approach (TOETVA) and parathyroidectomy-vestibular approach (TOEPVA) are scar-free alternatives to conventional surgery but are classified as clean-contaminated due to the oral incision, raising concerns about surgical site infections (SSIs). This study evaluates whether perioperative antibiotic prophylaxis (pABX) alone is sufficient compared to extended antibiotic prophylaxis (eABX) in preventing SSIs in TOET/PVA, particularly considering the surgical learning curve. Materials and Methods: A retrospective study analy…

Perioperative Antibiotic Prophylaxis Duration in Patients Undergoing Cystectomy With Urinary Diversion: A Randomized Clinical Trial.

IMPORTANCE: Perioperative antibiotic prophylaxis (PAP) reduces the incidence of surgical site infections (SSIs). Guidelines recommend 24 hours or less of PAP for cystectomy with urinary diversion; however, evidence specifying optimal PAP duration for this surgery is lacking, and prolonged administration of PAP is common practice. OBJECTIVE: To establish noninferiority of 24-hour PAP vs extended-duration PAP in preventing SSIs within 90 days after cystectomy with urinary diversion. DESIGN, SETTING, AND PARTICIPANTS: This single-center, noninferiority randomized clinical trial was performed fr…

Are self-reported anthropometric data reliable enough to meet antibiotic prophylaxis guidelines in orthopedic surgery?

BACKGROUND: Surgical site infection is a serious complication in orthopedic surgery. The use of antibiotic prophylaxis (AP) combined with other prevention strategies has been shown to reduce this risk to 1% for hip arthroplasty and 2% for knee arthroplasty. The French Society of Anesthesia and Intensive Care Medicine (SFAR) recommends doubling the dose when the patient's weight is greater than or equal to 100 kg, and the body mass index (BMI) is greater than or equal to 35 kg/m2. Similarly, patients with a BMIgreater than40 kg/m2 orlesser than18 kg/m2 are ineligible for surgery in our hospital…

Antibiotic prophylaxis of postoperative endophthalmitis after cataract surgery: results of the 2021 ASCRS member survey.

In an online survey of >1200 global cataract surgeons, 66% were using intracameral (IC) antibiotic prophylaxis. This compared with 50% and 30% in the 2014 and 2007 surveys, respectively. Irrigation bottle infusion and intravitreal injection was each used by only 5% of respondents. For IC antibiotics, vancomycin was used by 6% in the United States (52% in 2014), compared with 83% for moxifloxacin (31% in 2014). Equal numbers used compounded moxifloxacin or the Vigamox bottle as the source. There was a decrease in respondents using preoperative (73% from 85%) and postoperative (86% from 97%) top…
