Despite interest in ultraviolet (UV) damage and UV vision in lower vertebrates, there are few recent publications on human UV sensitivity. We obtained dark-adapted spectra from 4 aphakic and 5 normal eyes at 8.8 degrees off-fovea using the staircase method. Our measurements extended from 314.5 nm, near the limit imposed by corneal UV absorbance, to 650 nm. Phakic and aphakic sensitivities resembled the traditional rod spectrum at long wavelengths with a peak around 500 nm. Rigid gas permeable (RGP) contact lenses were prescribed and worn with little difficulty by 10 consecutive aphakic infants. Fluoroperm 92 lenses were used instead of silicone lenses (Bausch & Lomb) because of their lower cost, ease in handling, flexibility in design, and better neutralization of astigmatism. Lenses were worn on a daily wear basis (with occasional overnight wear) to reduce the risk of keratitis. Keratometry readings were taken at the time of surgery with the patient in a supine position. Forty aphakic eyes, with secondary pupillary membranes, underwent pars plana membranectomy and YAG laser discission randomly. Visual improvement was similar in both the groups. IOP remained low for a week in pars-plana membranectomy while it transiently increased following YAG laser discission Complications like anterior chamber reaction, corneal edema and CME were more after pars plana membranectomy than in YAG laser discission. In membranes thicker than 1.2 mm, only pars plana membranectomy is recommended.

Examinations of VEP were performed in 32 cases of pseudophakia (20 implants to the anterior, 12 to the posterior chamber). The impulses were light flashes and a reversible checkboard pattern. Normal records of VEP were obtained in 93 p.c. of cases with the use of a light flash and in 75 p.c. by applying a checkboard pattern. No correlation was found between the visual acuity and the VEP. The authors observed a shortening of the latency time of the N2 bend after implantation of the lens using the light flashes.

The practicality of extended-wear contact lenses in the refractive correction of pediatric aphakia was assessed with 240 eyes in 184 patients. Patient ages at the time of contact lens fitting ranged from 18 days to 9.8 years. One hundred forty-one eyes were fit from 1 day to 55 months postoperatively and then followed for 6 months to 5.7 years (average, 29 months). Only five patients lost more than five lenses. It is the author's opinion that extended wear lenses should be fit with caution on patients who cannot wear or handle daily wear lenses or adapt to aphakic spectacles. Standard protocol after initial fitting should be to examine the anterior segment with the slit lamp for vascular changes every three months indefinitely without final discharge. A review of 100 aphakic extended-wear soft contact lenses is presented for the period July 1980 to August 1981.
