A case of a 10-year-old boy with a congenital arachnoid cyst in the right middle cranial fossa is reported. The symptoms were precipitated by head injury of moderate intensity, occurred some days before the beginning of the clinical picture. BACKGROUND: Intraoperative traumatic cerebrospinal fluid (CSF) fistulas/dural tears (DT) occur in up to 8.7-9.5% of primary lumbar surgical procedures. Further, they recur secondarily in between 8.1% and 17% of cases. It is critical to diagnose and treat these recurrent lumbar DT early (i.e. within 3-4 weeks of the index surgery) to avoid the evolution of adhesive arachnoiditis (AA), and its' permanent neurological sequelae.

A clinical constellation of aseptic meningitis, arachnoiditis, communicating hydrocephalus, and a Guillain-Barré syndrome occurred following lumbar myelography with metrizamide. To our knowledge, Guillain-Barré syndrome has not been previously described following myelography with any contrast agent. Meningeal reactions and hydrocephalus have been reported with other agents, but this is the first instance with metrizamide.

BACKGROUND: Intraoperative ultrasound (iUS) has emerged as a valuable imaging resource in neurosurgical procedures. To improve familiarity with iUS technology and optimize its usage, additional exposure and technical demonstrations are needed. Spinal arachnoid cysts (SACs) are rare spinal cord lesions most often treated with resection, with procedural challenges stemming from poor preoperative lesion visualization and the infrequency of the procedure. An immunocompromised patient presented with acute-on-chronic back pain following minor trauma. This case report presents a rare association between symptomatic AC and idiopathic intracranial hypertension (IIH).
