Giant cell arteritis following varicella zoster vaccination.

BACKGROUND: Giant cell arteritis (GCA) is a form of large-vessel vasculitis affecting patients older than 50years old. Recent reports have suggested that the condition is caused by reactivation of varicella zoster virus (VZV). If that is indeed the case, a vaccine that prevents VZV reactivation should reduce the incidence of GCA. To further test that hypothesis, we assessed the incidence of GCA among patients older than 50years of age who were vaccinated against VZV and compared it to GCA incidence in the general population. METHODS: Using a centralized electronic medical database of the majo…

Takayasu arteritis associated with severe renovascular hypertension.

Takayasu arteritis (TA) is an inflammatory process frequently associated with stenosis and obliteration of the aorta and its primary branches. We report a 16-year-old girl in whom TA manifested mainly with a sudden decrease in visual acuity and severe arterial hypertension in both legs. Bilateral radial pulses were absent. Ultrasound showed renal size asymmetry and raised the possibility of renal artery stenosis. The diagnosis of TA was confirmed by computed tomography angiography, which showed a thickened abdominal aortic wall and narrowing of its lumen. In addition, occlusions of left renal …

[Endovascular intervention in Takayasu Arteritis. Case report].

A four year old patient with no medical history was admitted to our hospital's Emergency Department, suffering from edema and hypertension. During physical examination a low pulse was detected in the left arm and a lack of pulses in lower limbs, with a blood pressure difference greater than 10 mm Hg between both arms. An echocardiogram demonstrated severe aortic abdominal coarctation and ventricular dysfunction. Takayasu arteritis was diagnosed and cardiac angiography was performed. Disconnection of left subclavian artery and severe tightness at the abdominal aorta were confirmed. The definiti…

Varicella zoster virus and giant cell arteritis.

PURPOSE OF REVIEW: Giant cell arteritis (GCA) is a serious disease and the most common cause of vasculitis in the elderly. Here, studies describing the recent discovery of varicella zoster virus (VZV) in the temporal arteries of patients with GCA are reviewed. RECENT FINDINGS: GCA is characterized by severe headache/head pain and scalp tenderness. Many patients also have a history of vision loss, jaw claudication, polymyalgia rheumatica, fever, night sweats, weight loss, and fatigue. The erythrocyte sedimentation rate and C-reactive protein are usually elevated. Diagnosis is confirmed by temp…

Brucella arteritis: clinical manifestations, treatment, and prognosis.

Brucellosis is the most common bacterial zoonosis, and causes a considerable burden of disease in endemic countries. Cardiovascular involvement is the main cause of mortality due to infection with Brucella spp, and most commonly manifests as endocarditis, peripheral and cerebrovascular aneurysms, or arterial and venous thromboses. We report a case of brucellosis presenting as bacteraemia and aortic endarteritis 18 years after the last known exposure to risk factors for brucella infection. The patient was treated with doxycycline, rifampicin, and gentamicin, and underwent surgical repair of a p…

Takayasu arteritis revisited: current diagnosis and treatment.

Takayasu arteritis (TA) is a rare nonspecific inflammatory disease of unknown cause, predominantly affecting the aorta and its main branches, coronary arteries, and pulmonary arteries of young females. It induces a variety of nonspecific inflammatory symptoms and ischemic symptoms due to stenotic lesions. Further progression of TA causes destruction of the arterial wall media, leading to aortic regurgitation and aneurysms or rupture of the involved arteries. Although serological tests specific for TA are not available, new better biomarkers are emerging such as pentraxin3 and matrix metallopro…

Pulmonary artery replacement for pulmonary Takayasu's arteritis.

Takayasu's arteritis is a chronic inflammatory disease that affects the pulmonary artery, as well as the aorta and its major branches. A 59-year-old man presented with a 2-month history of progressive exertional dyspnea. Further examination revealed marked wall thickening of the pulmonary trunk and bilateral proximal pulmonary artery, resulting in severe stenoses with high pressure gradient of 60 mmHg. The patient underwent graft replacement of the bilateral pulmonary artery and the pulmonary trunk with a 16-mm ring-supported extended polytetrofluoroethylene graft under extracorporeal circulat…

Late onset takayasu arteritis and rheumatoid arthritis.

We encountered the rare case of a 48-year-old Caucasian woman who developed Takayasu arteritis (TA) while suffering from seropositive rheumatoid arthritis (RA). Several studies have reported an association between TA and various autoimmune disorders, however, the concurrent presence of Takayasu arteritis and rheumatoid arthritis is described in only few cases in the literature to date. The exact nature of the relationship between TA and RA remains unknown. Perhaps the development of these two diseases represents non-specific systemic inflammatory changes in the presence of a hereditary backgro…

[Takayasu's arteritis in pregnancy: report seven cases].

Takayasu's arteritis is a chronic and non-specific disease of young women in reproductive age that primarily affects the aorta, its branches and the pulmonary artery. Ramirez Cueto G. and Fernandez Del Castillo C. et al. published a case of pregnancy in Mexico and Takayasu's arteritis in 1968. There are no reports of this disease in pregnancy since. The purpose of this study is to describe the clinical course and perinatal outcome of seven pregnant patients with known diagnosis of Takayasu arteritis. The clinical course, laboratory findings, angiographic findings and perinatal outcomes were as…

Atypical temporal arteritis causing posterior circulation stroke.

We report an 80-year-old woman presenting with a posterior circulation infarct secondary to temporal arteritis. The clinical and laboratory features were atypical. This case highlights the importance of having a low threshold for considering a temporal artery biopsy as accurate diagnosis, and prompt intervention may improve outcomes.

Tortuous carotid artery lumens in Takayasu's arteritis.

We report a 68-year-old woman who was incidentally diagnosed as having an occlusion in the right internal carotid artery on magnetic resonance angiography. On ultrasound, the right common carotid artery showed narrow and tortuous lumens, which suggest the uneven thickening of the arterial wall in the nonactive stage of Takayasu's arteritis. Although macaroni-like diffuse and circumferential thickening of the arterial wall is a known ultrasonographic finding of Takayasu's arteritis, the tortuous sign also appears to be characteristic as an ultrasonographic finding of the healed arteritis in age…

Giant cell arteritis presenting as facial swelling.

Facial swelling is commonly ascribed to angioedema and a host of other causes. Temporal arteritis (TA), a disease most often diagnosed in patients over the age of 50 years, frequently presents with nonspecific and often ignored complaints (headache, symptoms of polymyalgia rheumatica, low-grade fever, fever of unknown origin, loss of appetite, depression, joint pains, weight loss, hair loss, and even respiratory symptoms). The diagnosis of TA is highly likely in the presence of new-onset headaches, polymyalgia rheumatica, and a tender, cord-like, or swollen temporal artery. Facial swelling mus…

Severe supraaortal atherosclerotic disease resembling Takayasu's Arteritis.

We report a case of a 64 year-old man whose clinical presentation and neuroimaging findings strikingly resembled those found in Takayasu's Arteritis which is characterized by the triad of absent radial pulses, ischemic retinopathy, and carotid sinus hyperreflexia causing syncopes. Angiographically, the patient exhibited severe atherosclerotic changes of the supraaortic large vessels. Stent-assisted angioplasty resulted in both clinical improvement and increased cerebral blood flow as measured by angiography and ultrasound.
