INTRODUCTION: Intravesical bacillus Calmette-Guérin (BCG) therapy-associated articular complications are uncommon, occurring in only 0.5 to 1% of the patients.
OBSERVATIONS: We report two patients who were given intravesical BCG therapy for superficial bladder cancer. Both patients developed polyarthritis and fever related to intravesical BCG instillation. The outcome of articular manifestations was favorable after administration of nonsteroidal anti-inflammatory therapy.
CONCLUSION: Intravesical BCG therapy-associated articular complications should not be overlooked, as they may result in h

Inhibitors of tumor necrosis factor-alpha (TNF-alpha) have been approved for treating rheumatoid arthritis. As one of the biological response modifiers, etanercept has also been used in the treatment of psoriatic arthritis and inflammatory bowel disease. While etanercept is effective, certain infectious complications, such as tuberculosis, fungus, and cytomegalovirus, have been reported. We report the first Korean case of adenoviral pneumonia in a 55-year-old female who developed disseminated adenoviral infection following etanercept treatment, which resolved after anti-TNF-alpha discontinuati

Lyme disease and adult onset Still's disease share several clinical characteristics such as fever, lymphosplenomegaly, skin involvement, and arthritis. Borrelia burgdorferi has been recognized as the cause of Lyme disease and an infectious agent has been suspected, at least as triggering factor, in adult onset Still's disease. This work is concerned with a patient affected by atypical Lyme disease originally diagnosed as adult onset Still's disease. B. burgdorferi infection must be suspected in cases of arthritis suggestive of adult onset Still's disease.

Janus kinase inhibitors (JAKis) represent a relatively new class of immunomodulatory drugs with potent effects on various cytokine signalling pathways. They have revolutionized the treatment landscape for autoimmune diseases such as rheumatoid arthritis, psoriatic arthritis, and ulcerative colitis. However, their ability to modulate immune responses presents a dual-edged nature, influencing both protective immunity and pathological inflammation. This review explores the complex role of JAKis in infectious settings, highlighting both beneficial and detrimental effects. On the one hand, experime

Photodynamic therapy (PDT) employs the triple combination of photosensitizers, visible light and ambient oxygen. When PDT is used for cancer, it has been observed that both arms of the host immune system (innate and adaptive) are activated. When PDT is used for infectious disease, however, it has been assumed that the direct antimicrobial PDT effect dominates. Murine arthritis caused by methicillin-resistant Staphylococcus aureus in the knee failed to respond to PDT with intravenously injected Photofrin(®). PDT with intra-articular Photofrin produced a biphasic dose response that killed bacter

A new nuclear scanning technique was found more specific for bone, joint, and soft tissue infections than any previously described scanning technique. The leukocyte scan, whereby a patient's own cells are labeled with a radioactive tagging agent (111In oxine), can distinguish an active infectious process from other pain-inducing conditions. Ninety-seven 111In labeled autologous leukocyte scans were performed in 88 patients. The findings in 17 of 40 patients scanned for possible acute osteomyelitis, six of nine for suspected septic arthritis, and six for possible soft tissue infections, were po

Extrapulmonary tuberculosis is uncommon, and musculoskeletal involvement presenting as migratory arthritis is particularly rare in immunocompetent individuals, often leading to misdiagnosis and delayed treatment. We report the case of a 66-year-old man with diabetes mellitus who presented with recurrent migratory inflammatory arthritis affecting multiple joints over 1 year, without initial pulmonary symptoms. He was initially diagnosed with rheumatoid arthritis based on positive rheumatoid factor and elevated inflammatory markers and was treated with disease-modifying antirheumatic drugs witho

We present a case of Tissierella bacteremia in the setting of polymicrobial osteomyelitis. Tissierella is a Gram-variable bacterium that has been rarely documented as the etiologic organism in human infections such as septic arthritis or otitis media, and even more rarely reported as an organism associated with bacteremia. The patient presented with septic shock and the physical exam was notable for gangrene of bilateral feet. Infectious workup was significant for acute osteomyelitis and septic arthritis of his feet in the setting of Tissierella bacteremia, which was successfully treated with

The use of etanercept in rheumatoid arthritis patients with end-stage renal failure has not been well reported. We report here the case of 58-years-old patient who was treated with 50 mg of etanercept once a week to control the activity of her disease. The patient improved after starting the treatement. No serious infectious complication or side effects were seen. The follow-up was 18 months. Our case showed the safety and efficacity of etanercept rheumatoid arthritis in patient with end-stage renal failure.

Cyclooxygenase (Cox) is a key enzyme in the biosynthetic metabolism of prostaglandins. The inducible isoform of Cox-2 has been implicated in inflammation and its specific inhibition can be used to treat noninfectious inflammatory diseases, such as rheumatoid arthritis. Borrelia burgdorferi, the agent of Lyme disease, can induce joint inflammation. Here we show that B. burgdorferi induced the upregulation of cox-2 gene expression in murine joints at the onset of arthritis in infected mice. The level of mRNA expression correlated with the degree of inflammation. The specific inhibition of Cox-2

New treatment strategies in rheumatoid arthritis are targeted to interfere with critical mediators of inflammation. Proinflammatory cytokines like IL-1 beta and TNFalpha play a crucial role in induction and maintenance of synovitis, pannus formation and bone and cartilage destruction. Within a few years, these morphological changes may lead to joint destruction and consecutively to functional impairment. Since April 2002 a recombinant human interleukin-1 receptor antagonist (Anakinra) is available in Germany for treatment of patients with rheumatoid arthritis. Anakinra (Kineret(R)) is approved

Many musculoskeletal complaints are accompanied by classic signs and symptoms that can be readily diagnosed by the primary care physician. Others are much less obvious and present a diagnostic challenge. In the office evaluation of patients with musculoskeletal complaints, the history is the most informative element. Least helpful are laboratory tests. Although erythrocyte sedimentation rate (ESR), rheumatoid factor, and other widely available tests are sensitive to the presence of rheumatic diseases, they are not specific for any of them. In the initial office evaluation, helpful points of di

Laboratory investigations have been perhaps the most important factor in the development of our understanding and classification of rheumatic diseases. Provided they are used and interpreted appropriately, they can assist considerably in the diagnosis and management of these diseases. They are an important adjunct to clinical findings, but not a substitute for them.

Lactate concentration was studied in 383 synovial fluid specimens from patients with various arthritides. The highest concentrations of lactate occurred in non-gonococcal septic synovial fluids. High values were recorded in seropositive rheumatoid arthritis and crystal-induced arthritides, medium values in synovial fluids from seronegative rheumatoid arthritis, seronegative spondylarthritides, gonococcal arthritis and haemarthrosis, and the lowest values in aspirates from osteoarthrotic joints. There was a positive correlation between synovial pH and lactic acid concentration. These data sugge

Septic arthritis is acute onset of monoarticular inflammation of a joint due to an infectious etiology. It is usually bacterial but can be viral or fungal. Septic arthritis causes significant morbidity and mortality and requires prompt diagnosis and treatment. Risk factors include age older than 80 years, smoking, comorbid conditions (eg, diabetes, rheumatoid arthritis, skin infection, HIV infection, osteoarthritis), and other factors (eg, recent joint surgery, joint prosthesis, previous intra-articular injection). The clinical presentation of septic arthritis can overlap with those of many ot

This is a follow up study of 110 patients referred to the hospital because of mono-arthritis of unknown etiology. Patients with mechanical synovitis and infectious arthritis have been excluded from this study initially. From a total of 110 patients (100%) a diagnosis could be made in 49 patients (44.6%), namely in 24 (21.8%) by the initial very thorough work up or by the repeated examinations during the follow up period in 25 patients (22.8%). In 61 patients (55.4%) the cause of the disease remained unknown despite extensive investigations and follow up controls for many years. Of these 61 cas

Necrotizing scleritis is the most destructive and vision-threatening form of scleritis. Necrotizing scleritis can occur in systemic autoimmune disorders and systemic vasculitis, as well as following microbial infection. Rheumatoid arthritis and granulomatosis with polyangiitis remain the commonest identifiable systemic diseases associated with necrotising scleritis. Pseudomonas species is the most common organism causing infectious necrotizing scleritis, with surgery the most common risk factor. Necrotizing scleritis has the highest rates of complications and is more prone to secondary glaucom
