A study of piglets born by spontaneous parturition under uncontrolled conditions: could this be a naturalistic model for the study of intrapartum asphyxia?

In order to evaluate how spontaneously born piglets could be a suitable model for the study of intrapartum hypoxia, 230 newborn piglets were studied. Out them, 8.3% (n = 19) died intrapartum, 21.7% (n = 50) were born with moderate-to-severe intrapartum hypoxia, and 70% (n = 161) were born with mild or no evidence of intrapartum distress. Piglets born without any evidence of intrapartum asphyxia weighed approximately 240 g lower than those born with intrapartum hypoxia and intrapartum-dead piglets (P<0.0001). The viability score was approximately 3 units lower and the latency to contact the udd

Cardiac troponin I in asphyxiated neonates.

BACKGROUND: Cardiac troponins T (cTnT) and I (cTnI) are well-established markers in detecting myocardial ischemic damage in adults. Perinatal asphyxia is associated with cardiac dysfunction. OBJECTIVES: To evaluate serum concentrations of cTnI in asphyxiated neonates and to investigate whether cTnI is correlated with the traditional markers of asphyxia. METHODS: Blood samples were collected from 13 asphyxiated neonates (umbilical artery pH<7.18 and either a 1-min Apgar score<4 or a 5-min Apgar score<7) and 39 controls. Data on gestation, birth weight, sex, Apgar scores, mode of delivery, umb

[Effects of hypothermia on cardiac function in neonates with asphyxia].

OBJECTIVE: To study the effects of hypothermia on cardiac function in neonates after birth asphyxia. METHODS: Fifty term newborns with Apgar score < 5 at 5 minutes were randomly divided into no cooling group (normothermia group, NG; rectal temperature = 36.5 degrees C +/- 0.5 degrees C, n = 27) and cooling group (hypothermia group, HG; rectal temperature = 34.5 degrees C +/- 0.3 degrees C, nasopharyngeal temperature = 34.0 degrees C +/- 0.5 degrees C, n = 23). The selective head cooling was applied to maintain nasopharyngeal temperature at 34 degrees C for 72 h in hypothermia group. Systolic

[Neurological prognosis of term infants with perinatal asphyxia].

Neonatal encephalopathies following birth asphyxia are the first features of cerebral insult. They never miss when asphyxia is directly involved in cerebral impairment. Mild encephalopathies have constantly a good prognosis. Conversely, moderate and severe encephalopathies are associated with poor outcome (death or severe handicap) in 25% to 100% of cases. Prognosis of these moderate and severe encephalopathies can be assessed during the first ten days of life by 3 complementary ways: clinical exam, electrophysiology and imaging. The most information is obtained from the EEG and MRI which toge

[Cognitive deficit in perinatal asphyxia].

INTRODUCTION: During the period 1950 1970 the relation between perinatal asphyxia and cognitive alterations was studied. More recently the neuropsychological approach has been introduced to detect more subtle defects. DEVELOPMENT: With regard to intelligence, alterations in the intelligence quotient resulting from anoxia are more commonly seen in young children than in adolescents, probably because of compensation mechanisms. It is widely accepted that severe asphyxia causes motor and cognitive alterations and leads to cerebral palsy, epilepsy and intellectual retardation. The effects of mild

Prediction of neurological outcome after birth asphyxia from early continuous two-channel electroencephalography.

OBJECTIVE: To determine whether two-channel continuous electroencephalography (EEG) applied within 12 h of birth can predict the severity of neurological complications and neurodevelopmental outcome following birth asphyxia. METHODS: A continuous two-channel EEG was performed within 12 h of birth in 22 infants suspected of having suffered birth asphyxia and 11 healthy control infants (22 infants at a general and 11 at a specialist paediatric unit). Criteria to categorise normal and abnormal EEG records were defined and compared with the severity of hypoxic/ischaemic encephalopathy (HIE) and w

Acute renal failure in the newborn: incidence and outcome.

OBJECTIVE: To describe an experience from a developing country of newborn renal disease particularly those without advanced neonatal care. METHODOLOGY: Prospective evaluation from a referral hospital, North-East of Nigeria between 1 July 1990 and 30 June 1994. Babies admitted for any morbidities who were diagnosed as being in acute renal failure (ARF) during the study period (non-oliguric inclusive). Onset of ARF; day on which oliguria or anuria detected, or serum urea first exceeded 10 mmol/L. Urine output quantitated from carefully bagged urine and suprapubic bladder aspiration, and venous

Cerebral perfusion, metabolism, and outcome.

Factors contributing to brain damage and an adverse outcome are complex. Both severe hypoxia-ischemia and prolonged hypoxia of lesser severity may cause brain damage. Compromised cerebral perfusion leads to ischemia. Fetal brain damage may begin because of a redistribution rather than reduction of cerebral blood flow. Normal newborn cerebral blood flow is low; fetal asphyxia at birth causes delayed cerebral hyperperfusion in the neonate. The threshold of newborn cerebral blood flow that is associated with brain damage has not yet been established. Hypoxia-ischemia will disrupt cerebral metabol

[Polycythemia in the newborn. III: blood volume in asphyxia and meconium].

Several authors had reported high blood volumes (BV) and Low placental residual blood volumes (PRBV) in hypoxic human newborns, and also in asphyxiated experimentally animals. Those findings could be explained by and exaggerated intrauterine placental transfusion, ante or intrapartum. The authors had observed high cord blood and 24-48 hs. hematocrits in meconium-stained amniotic fluid (MSAF) and/or low 1 minute Apgar score newborns (Nb), despite early cord clamping. Sometimes, by delaying cord clamping up to 1 minute, those hematocrits had a tendency to decrease, instead of increasing. In view

[Neonatal hypoxic myocardial lesion].

Five patients suffering from transient myocardial ischemia of the newborn are discussed. The role of two-dimensional doppler-echocardiography is emphasized in establishing the diagnosis. Tricuspid insufficiency due to perinatal asphyxia can be detected easier by ultrasound than by any other cardiac examination. Cardiac failure as a result of hypoxic myocardial ischemia is usually reversible and responds well to anticongestive treatment and administration of oxygen. On the other hand some cases can be fatal, histopathologic examination on the heart shows similarity to myocardial infarction.

Hypoxic-ischemic spinal cord injury following perinatal asphyxia.

The role of spinal cord injury in the pathogenesis of abnormal motor signs (depressed tone and reflexes) following severe perinatal hypoxia-ischemia was prospectively evaluated by clinical, electrophysiological, and neuropathological examinations in 18 asphyxiated neonates. All infants had an abnormal mental status (lethargy or coma), and seizures were present in 12. Neuromuscular examinations revealed hypotonia or flaccidity and hyporeflexia or areflexia in all infants. Neuropathological examinations of the cerebrum and spinal cord were conducted in the 12 neonates who expired. Cerebral patho

Diminished respiratory sinus arrhythmia in asphyxiated term infants.

Spectral analysis techniques were used to quantitate the association between respiration and heart rate variability in eight healthy and eight asphyxiated infants born at term gestation. Respiratory sinus arrhythmia was demonstrated in all healthy infants. This arrhythmia was significantly diminished in asphyxiated newborn infants. We conclude that newborn infants with low Apgar scores have a reduced respiratory sinus arrhythmia and that this reduction could account for the loss of short-term heart rate variability commonly associated with asphyxia.

Neurological sequelae in newborn babies after perinatal asphyxia.

A total of 53 babies, 37 to 44 weeks' gestational age with a history of fetal distress in labour, and severe neurological abnormalities in the early newborn period, were followed up for between 2 and 5 years. Their progress was compared with an equal number of normal babies matched for gestational age, birthweight, sex, and social class, but without a history of fetal distress or delay in establishing spontaneous respiration at birth. In the set of babies with a history of fetal distress no perinatal condition was identified that could predict with certainty the type of neurological status in

[The campomelic syndrome (author's transl)].

Case report of a male infant who died from asphyxia on the second day of life and who exhibited typical features of the campomelic syndrome: odd facies, bowing of the long bones of the legs, skeletal deformities, cleft palate. There was also coarctatio of the aorta and probably aplasia of the fibula.
