Airway constriction by isocapnic hyperventilation of cold, dry air: comparison of magnitude and duration of protection by nedocromil sodium and sodium cromoglycate.

The protective effect of 1, 2 and 4 mg of nedocromil sodium against airway constriction induced by hyperventilation of cold, dry air was compared with 10 mg sodium cromoglycate and placebo in a double-blind randomized trial. On 5 days, twelve asthmatic subjects received one of the trial medications from a metered dose inhaler. Twenty minutes, 2.5 and 5 hr later, airway responsiveness to hyperventilation of cold, dry air was measured. At 20 min, there was significant protection by all four active medications when compared to placebo, with a trend towards a nedocromil dose relationship but there

Failure to meet extubation criteria in the setting of suspected buffalo chest physiology.

We report the case of a patient who failed to meet tracheal extubation criteria due to low tidal volumes from suspected buffalo chest, which is a single pleural space physiology. This presentation followed the resection of a large pleural mass in a 59-year-old woman with a history of exercise-induced asthma, hypertension and tumour-related chronic respiratory failure. Creation of a pleuro-pleural communication during the resection of this large, unilateral pleural mass led to bilateral pneumothoraces and contributed to patients inability to generate negative inspiratory force leading to failur

The impact of tangeretin combined with whey protein on exercise-induced bronchoconstriction in professional athletes: a placebo-controlled trial.

BACKGROUND: Exercise-induced bronchoconstriction (EIB) is highly prevalent in athletes. The objective of this study was to assess the therapeutic efficacy of daily tangeretin combined with whey protein supplementation over a period of 4 weeks in professional athletes with EIB. METHODS: Using a placebo-controlled, double-blind, paired, randomized trial design, a cohort of 30 professional athletes with EIB, consisting of 14 females and 16 males, was divided into two groups: the tangeretin combined with whey protein intervention group (TIG), and the placebo control group (PCG). Both the TIG and

Effect of sodium cromoglycate on mast cell mediators during hyperpnea in athletes.

INTRODUCTION: The role of mast cells in the airway response to exercise and the benefit of sodium cromoglycate (SCG) in athletes are unclear. PURPOSE: The purpose of this study was to clarify the role of mast cell mediators in the airway response to exercise in athletes and to investigate the effect of SCG. METHODS: Eleven athletes with exercise-induced bronchoconstriction (EIB+) and 11 without (EIB-) performed a eucapnic voluntary hyperpnea (EVH) test (a surrogate for exercise) 10 min after inhalation of a placebo or 40 mg of the mast cell stabilizing agent sodium cromoglycate. The urinary

Ascaris, atopy, and exercise-induced bronchoconstriction in rural and urban South African children.

BACKGROUND: Populations with endemic parasitosis have high levels of IgE but low levels of allergic disease. We investigated the association between infection with the parasite Ascaris allergic sensitization, and exercise-induced bronchospasm (EIB). OBJECTIVE: We sought to investigate the effect of Ascaris infection on bronchial hyperreactivity, skin testing, and specific IgE levels. METHODS: A cross-sectional prevalence survey was conducted in urban and rural South African children to measure levels of EIB. A sample of children was enrolled in a nested case-control study for further investi

Effects of disinfection by-products in swimming pool environments on the immunological mechanisms of respiratory diseases.

Swimming in pools is a popular and healthy recreational activity. However, potential adverse health effects from disinfection byproduct (DBP) exposure in pool water are concerning. This study evaluated how such DBP exposure affects the respiratory system. DBP exposure was simulated with an animal-specific pool environment model. Experimental animals were exposed to DBPs for a specified duration and frequency over 4 weeks. The wet and dry weights of murine lungs were measured, with no significant differences observed. There were no significant differences in interkeukin (IL)-2/4/10, and interfe

Respiratory manifestations in the Ehlers-Danlos syndromes.

Persons with the Ehlers-Danlos syndromes (EDS) report a wide range of respiratory symptoms, most commonly shortness of breath, exercise limitation, and cough. Also reported are noisy breathing attributed to asthma, difficulty with deep inhalation, and inspiratory thoracic pain. The literature consists of case reports and small cross-sectional and cohort studies. One case-control study estimated twofold to threefold greater respiratory disease burden among persons with EDS as compared to controls. The differential diagnosis for symptoms is broad. Structural alterations include pectus deformitie

Anaphylaxis: current state of knowledge for the modern physician.

Anaphylaxis is a severe, potentially fatal, hypersensitivity reaction of rapid onset. It may trigger life-threatening cardiopulmonary compromise, often with skin and mucosal changes such as urticaria and angioedema. The prevalence of anaphylaxis is increasing and the number of cases of fatal anaphylaxis appears to be rising. Food, insect stings, and drugs are the most common triggers. Novel triggers are increasingly seen and include delayed anaphylaxis to red meat, food-dependent exercise-induced reactions and anaphylaxis to monoclonal antibodies. Anaphylaxis is usually IgE mediated, but other

Inhibition of hypertonic saline-induced bronchoconstriction by terfenadine and flurbiprofen. Evidence for the predominant role of histamine.

We investigated the possible inhibitory effects of terfenadine, a histamine H1-receptor antagonist, and flurbiprofen, a cyclooxygenase inhibitor, on the bronchoconstrictor effect of inhaled 3.6% hypertonic saline in a randomized, double-blind study. Nine mildly asthmatic subjects with a history of exercise-induced asthma took part. This was conducted, first as a dose-response study and, second, as a time-course study. In the dose-response study, the provocative dose of saline-laden air causing a 25% fall in FEV1 was calculated (PD25). Terfenadine (180 mg) and the combination of terfenadine (18

[Interpretation of exhaled CO levels in studies on smoking].

INTRODUCTION: Exhaled carbon monoxide (ECO) concentration provides an easy, fast and non invasive means of assessing smoking status. However, some difficulties have to be considered when interpreting ECO levels. BACKGROUND: CO is a non-specific biomarker of tobacco exposure. Its half-life is short (2-6 hours). CO reflects the intensity of tobacco smoke inhalation. Its high affinity for haemoglobin, producing carboxyhaemoglobin (COHb), leads to tissue hypoxia. The correlation between ECO and COHb is strong and linear. Gender, pulmonary ventilation, physical exercise and time of day may induce

Endogenous and exogenous sex steroid hormones and asthma and wheeze in young women.

BACKGROUND: Emerging evidence suggests that both endogenous and exogenous sex steroid hormones may influence the occurrence of asthma and wheeze among women. OBJECTIVE: We investigated the associations between exogenous sex hormone (oral contraceptive [OC]) use and wheezing in young women with and without asthma history. To investigate the role of endogenous sex hormones, we examined the association between age at menarche and the development of asthma after puberty. METHODS: We conducted a study among 905 women who had undergone menarche. Subjects were between 13 and 28 years of age and had

[The effect of air pollution on the prevalence of asthma in schoolchildren from Krakow].

The aim of the study was to assess the relationship between asthma, wheezing prevalence and the rate of air pollution in Kraków. In the survey of 6696 schoolchildren aged 12-14 year, ISAAC questionnaires were performed. According to the distribution of the mean annual concentration of dust particles (PM10) and sulfur dioxide, four classes of these compounds were established. In the similar way, the lead concentration in garden soil and lettuce was categorized into three classes, which were surrogates of the automobile exhaust. Children's passive smoking was established by questionnaire. The st

The pathophysiology of asthma. Extrinsic influences other than immunologic.

Atopy is not the most common factor causing asthma in adults. Recent elucidations of pathophysiologic mechanisms involving the autonomic nervous system, cyclic nucleotide metabolism, and arachnoidonic biochemistry give us a better understanding of the similarities and unique differences among asthma events related to exercise, aspirin, emotions, occupational environments, atmospheric conditions, and respiratory tract infections. The ultimate value of this information is no greater than its potential to aid the physician in decreasing the interference of asthma in the daily lives of patients.

Glucocorticoid-induced osteoporosis: pathogenesis and management.

Glucocorticoid- (GC-) induced osteoporosis and an increased risk of fractures are one of the most serious problems for patient using long-term GC therapy, such as those with rheumatoid arthritis, autoimmune diseases, inflammatory bowel diseases, bronchial asthma, and chronic lung diseases. GCs are known to affect both bone formation and resorption. In rheumatoid arthritis, the etiology of bone loss is multifactorial, including local inflammation around joints, release of bone-absorbing cytokines, physical inactivity, and malnutrition, in addition to the use of GC. Two guidelines have been publ

House cleaning with chlorine bleach and the risks of allergic and respiratory diseases in children.

Chlorine bleach or sodium hypochlorite can inactivate common indoor allergens. In this cross-sectional study we evaluated to what extent regular house cleaning with bleach can influence the risks of respiratory and allergic diseases in children. We studied a group of 234 schoolchildren aged 10-13 yr among whom 78 children were living in a house cleaned with bleach at least once per week. Children examination included a questionnaire, an exercise-induced bronchoconstriction test and the measurement of exhaled nitric oxide (NO) and of serum total and aeroallergen-specific immunoglobulin (Ig)E, C

Interaction between hyperosmolar and IgE-mediated histamine release from basophils and mast cells.

Because both hyperosmolar and IgE-dependent stimuli may be encountered in the airway, and because hyperosmolarity causes histamine release in basophils, we examined the effects of the 2 stimuli on human lung mast cells. Mast cells prepared by enzymatic digestion of human lung were suspended in buffers made hyperosmolar with mannitol. Significant histamine release was seen above 360 mOsm/kg H2O, increasing to 11.9 +/- 1.0% at 770 mOsm/kg H2O, and release was synergistically enhanced by anti-IgE. Cells that had been rendered unresponsive to IgE-dependent stimuli by exposure to anti-IgE in the ab

Is there a role for antileukotrienes in urticaria?

In vitro and in vivo clinical and experimental data have suggested that leukotrienes play a key role in inflammatory reactions of the skin. Antileukotriene drugs, i.e. leukotriene receptor antagonists and synthesis inhibitors, are a new class of anti-inflammatory drugs that have shown clinical efficacy in the management of asthma. We searched the MedLine database and carried out a manual search on journals specializing in allergy and dermatology for the use of antileukotriene drugs in urticaria. Montelukast might be effective in chronic urticaria associated with aspirin or food additive hypers

From adrenaline to formoterol: advances in beta-agonist therapy in the treatment of asthma.

Long-acting beta2-agonists (formoterol and salmeterol) represent the latest advance in a series of improvements in beta-agonist asthma therapy since the introduction of isoprenaline. Traditional inhaled short-acting beta2-agonists (salbutamol and terbutaline) provide rapid as-needed symptom relief and short-term prophylactic protection against bronchoconstriction induced by exercise or other stimuli. Where symptoms are not adequately controlled by corticosteroids alone, inhaled selective long-acting beta2-agonists are used additionally for maintenance. Salmeterol and formoterol are well tolera
