Significant left atrial appendage activation delay complicating aggressive septal ablation during catheter ablation of persistent atrial fibrillation.

BACKGROUND: This study aims to describe significant left atrial appendage activation following ablation of persistent atrial fibrillation, and explore its relationship with aggressive septal ablation. METHODS AND RESULTS: Significant left atrial appendage activation delay was found in 23 out of 201 patients undergoing persistent atrial fibrillation ablation. Of them, 14 were found in their index procedures, of whom septal line ablation was performed in nine (odds ratio 15.2, 95% confidence interval 4.6-50.8, P < 0.001). Another nine were found during their redo procedures (including two with …

Clinical significance of fibrillatory wave amplitude. A clue to left atrial appendage function in nonrheumatic atrial fibrillation.

Seventy-eight patients with chronic nonrheumatic atrial fibrillation were studied by transesophageal echocardiography with regard to the left atrial appendage function and its relation to the coarseness of atrial fibrillation on electrocardiogram. These 78 patients (52 men and 26 women; mean age, 66 +/- 10 years; range, 40 to 94 years) were classified into two groups according to the presence of coarse (group 1, n = 46; those with the greatest amplitude of fibrillatory wave in lead V1 > or = 1 mm) or fine (group 2, n = 32; those without the coarse fibrillatory wave in lead V1) atrial fibrillat…

Echocardiography evaluation of iatrogenic atrial septal defect after combined procedure of catheter ablation and left atrial appendage closure for atrial fibrillation.

Using transthoracic echocardiography (TTE) and transesophageal echocardiography (TEE) to investigate the occurrence and related causes of iatrogenic atrial septal defect (iASD) after catheter ablation combined with left atrial appendage closure (LAAC) for atrial fibrillation (AF) and its impact on the right heart system. We retrospectively analyzed 330 patients that underwent combined procedure of catheter ablation for AF and LAAC at General Hospital of Northern Theater Command from January 2018 to March 2022. These patients were divided into iASD group and non-iASD group according to whether …

Left atrial appendage volume is an independent predictor of atrial arrhythmia recurrence following cryoballoon pulmonary vein isolation in persistent atrial fibrillation.

PURPOSE: Pulmonary vein isolation (PVI) is the cornerstone of atrial fibrillation (AF) ablation in persistent AF (persAF), and cryoballoon PVI emerged as an initial ablation strategy. Symptomatic atrial arrhythmia recurrence following successful PVI in persAF is observed more frequently than in paroxysmal AF. Predictors for arrhythmia recurrence following cryoballoon PVI for persAF are not well described, and the role of left atrial appendage (LAA) anatomy is uncertain. METHODS: Patients with symptomatic persAF and pre-procedural cardiac computed tomography angiography (CCTA) images undergoin…

Sacubitril/Valsartan Improves Left Atrial and Left Atrial Appendage Function in Patients With Atrial Fibrillation and in Pressure Overload-Induced Mice.

LCZ696 (sacubitril/valsartan) is an angiotensin receptor-neprilysin inhibitor and has shown beneficial effects in patients with heart failure. However, whether LCZ696 protects against left atrial (LA) and LA appendage (LAA) dysfunction is still unclear. The present study aimed to assess the efficacy of LCZ696 for improving the function of LA and LAA. We performed both a retrospective study comparing LCZ696 with angiotensin receptor blockers (ARBs) to assess the efficacy of LCZ696 in patients with atrial fibrillation and an animal study in a mouse model with pressure overload. LA peak systolic

[Left atrial function and left atrial appendage flow velocity in hypertrophic cardiomyopathy: comparison of patients with and without paroxysmal atrial fibrillation].

The involvement of left atrial (LA) appendage flow velocity in reduced left atrial function was investigated in 24 patients with hypertrophic cardiomyopathy, who retained sinus rhythm at the examination. Patients were divided into 11 with a history of paroxysmal atrial fibrillation [PAf(+)] and 13 without such history [PAf(-)]. Transthoracic echocardiography was performed to evaluate LA fractional shortening (LA%FS) and mean velocity of circumferential LA fiber shortening (LAmVcf), as contractile functions of the left atrium at the phase of active atrial contraction. Transesophageal echocardio

Zero-fluoroscopy approach to left atrial appendage closure using intracardiac echo guidance.

BACKGROUND: Left atrial appendage closure (LAAC) with the Watchman™ device is typically performed with the use of transesophageal echocardiography (TEE) and fluoroscopy. LAAC guided by intracardiac echo (ICE) without the use of fluoroscopy has not been previously described. OBJECTIVE: In this series, we describe the safety and feasibility of Watchman™ implantation using only ICE imaging and without the use of fluoroscopy. METHODS: 214 patients undergoing LAAC with the Watchman FLX™ or Watchman FLX™ Pro device and using a planned fluoroscopy-free approach were studied. ICE was used for echoca…

New Measurements Using Computed Tomography Predict Success of Left Atrial Appendage Closure.

BACKGROUND: Preprocedural screening using transesophageal echocardiography (TEE) and computed tomography (CT) is widely performed in patients undergoing left atrial appendage (LAA) closure (LAAC). However, established methods for accurately predicting procedural device success (DS) remain unclear. AIMS: This study aimed to evaluate whether a novel preprocedural CT-based assessment focusing on the landing zone and presence of an internal septum could improve DS prediction. METHODS: We analyzed 560 patients from a Japanese multicenter registry who underwent LAAC with Watchman FLX (Boston Scien…

Left Atrial Appendage Closure Is Underutilized in Coronary Artery Bypass Graft Surgery.

INTRODUCTION: The real-world utilization and practice patterns of surgical left atrial appendage closure (s-LAAC) during coronary artery bypass graft (CABG) surgery remains inadequately characterized. This study aims to evaluate the use, predictors, and temporal trends of s-LAAC in patients undergoing isolated CABG surgery using a nationwide database. METHODS: We performed a retrospective analysis of the National Inpatient Sample from 2016 to 2021, identifying patients with a diagnosis of atrial fibrillation who underwent isolated CABG surgery. Patients were stratified by whether they receive…

Trial emulation of percutaneous left atrial appendage occlusion versus direct oral anticoagulants.

BACKGROUND: The use of percutaneous left atrial appendage occlusion (pLAAO) has increased in the past several years, as an alternative to oral anticoagulants in patients with atrial fibrillation (AF). The pivotal trials that led to pLAAO approval had small sample size and their populations differ from real-world patients who undergo pLAAO. OBJECTIVE: To compare pLAAO to direct oral anticoagulants (DOACs) in Medicare beneficiaries with AF. DESIGN: We created a retrospective cohort to emulate a target trial, from Jan 1st, 2015 to Dec 31st, 2020. Patients were identified from a 15% nationally rep…

Sudden death from non-traumatic right atrial appendage rupture: an autopsy case.

Myocardial rupture most often involves the ventricles following acute myocardial infarction, whereas non-traumatic atrial rupture is extremely uncommon, and typically confirmed histologically. Isolated atrial infarction is rare and diagnostically challenging, requiring careful distinction from traumatic causes in forensic practice. We report a fatal case of non-traumatic rupture of the right atrial appendage (RAA). The patient was a 43-year-old woman with systemic lupus erythematosus. Postmortem computed tomography revealed high-density areas within the pericardial space. Autopsy demonstrated …

Threading a needle: percutaneous closure of an incompletely ligated left atrial appendage.

Atrial fibrillation increases the risk of ischemic stroke by increasing left atrial appendage thrombus formation. Previously surgically ligated atrial appendages can recanalize, which increases stroke risk. Percutaneous ligation of such appendages is technically challenging due to altered morphology but feasible. We present a case of a successful percutaneous closure of a previously incompletely ligated atrial appendage using a WATCHMAN FLX device and discuss our approach to similar cases with challenging anatomy. Thorough pre-procedural imaging for case planning is indispensable and dictates …

2025 SCAI/HRS clinical practice guidelines on transcatheter left atrial appendage occlusion.

BACKGROUND: Left atrial appendage occlusion (LAAO) devices reduce the risk of atrial fibrillation-associated stroke relative to no oral anticoagulation. However, uncertainty and practice variation persist in the areas of patient selection, periprocedural imaging, adjunctive antithrombotic therapy, and management of peridevice leak (PDL) and device-related thrombus. The Society for Cardiovascular Angiography & Interventions and Heart Rhythm Society developed these evidence-based guidelines to support clinicians, patients, and other stakeholders in management decisions regarding LAAO. METHODS: …

Outcomes of Left Atrial Appendage Occlusion in Patients With Prior Gastrointestinal Bleeds.

BACKGROUND: Left atrial appendage occlusion (LAAO) is increasingly used as a stroke prevention strategy in patients with atrial fibrillation who have contraindications to oral anticoagulation, including prior gastrointestinal bleeding (GIB). However, the clinical risks and benefits of LAAO may differ substantially depending on the specific indication for the procedure, and outcomes in patients with prior GIB remain incompletely understood. OBJECTIVES: The aim of this study was to evaluate in-hospital and long-term outcomes of LAAO in patients with prior GIB. METHODS: Using data from the Nati…

Impact of Immunosuppressive Drugs on Patients With Percutaneous Left Atrial Appendage Occlusion.

INTRODUCTION: Percutaneous left atrial appendage closure (LAAC) reduces the risk of thromboembolic stroke in patients with non-valvular atrial fibrillation who cannot tolerate long-term oral anticoagulation. However, outcomes after LAAC in patients requiring chronic immunosuppressive therapy (IMS) remain unknown. This study aimed to investigate the perioperative and long-term outcomes of percutaneous LAAC in patients receiving chronic IMS. METHODS AND RESULTS: Patients who underwent LAAC were retrospectively evaluated according to the presence of long-term IMS therapy. Perioperative complicat…

Left Atrial Appendage Occlusion in Patients With Anticoagulation Failure vs Anticoagulation Contraindication.

BACKGROUND: Left atrial appendage occlusion (LAAO) provides mechanical cardioembolic protection for atrial fibrillation (AF) patients who cannot use oral anticoagulation therapy (OAT). Patients with a thrombotic event despite OAT are at high risk for recurrence and may also benefit from LAAO. OBJECTIVES: This study sought to investigate the efficacy of LAAO in AF patients with a thrombotic event on OAT compared to: 1) LAAO in AF patients with a contraindication for OAT; and 2) historical data. METHODS: The international LAAO after stroke despite oral anticoagulation (STR-OAC LAAO) collaborat…
