[Atrial fibrillation provoked by Valsalva maneuver].

The connection between certain arrhythmias and the autonomic nervous system is well known. Authors report a patient, whose paroxysmal atrial fibrillation was induced by increased vagal tone.

[From paroxysmal to chronic atrial fibrillation].

Chronic atrial fibrillation is by definition always preceded by paroxysmal atrial fibrillation. The electropathophysiological mechanisms underlying paroxysmal atrial fibrillation are reviewed: atrial electrophysiological inhomogeneity, atrial ectopic activity, and cardiac autonomic dysfunction. Safe and effective interventions that prevent the progression from paroxysmal into chronic atrial fibrillation have not yet been developed. Such developments should be given high priority, as the consequences of chronic atrial fibrillation--stroke and heart failure--are unacceptable.

Atrial fibrillation: overview of therapeutic trials.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia treated by physicians, and a plethora of therapeutic trials deal with selected aspects of its management. This overview attempts to categorize and summarize the available studies. A key to management of AF is a clinical classification schema that provides a framework for application of the available treatment modalities. Such a classification is provided. Antithrombotic trials have demonstrated the remarkable efficacy of warfarin and more modest effect of acetylsalicylic acid for prevention of stroke; these data are summa…

[Bilateral homonymous hemianopsia with atrial fibrillation].

We encountered an 80-year-old man with sudden bilateral visual disturbance. When he was admitted to the hospital, his blood pressure was 138/70 mmHg, and an ECG revealed atrial fibrillation. Neurological examination showed only bilateral homonymous hemianopsia with no nystagmus or impairment of eye movement. He did not have paralysis cerebellar ataxia, or speech disturbance. Therefore, there was no evidence of obstruction of the thalamogeniculate or thalamoperforating artery. Magnetic resonance imaging of the brain showed cerebral infarctions in both occipital lobes. Perimetry showed bilateral…

Neuropsychological deficits in asymptomatic atrial fibrillation.

OBJECTIVE: To assess the preclinical effects on cognitive functions of nonrheumatic atrial fibrillation (NRAF) in patients with negative history for cerebrovascular disease. MATERIALS AND METHODS: The study included 37 consecutive patients with chronic (n=16, mean age 65.3+/-6.6 years) or paroxysmal (n=21, mean age 58.3+/-9.5 years) NRAF and an equal number of control subjects in sinus rhythm, who were matched for age, education and presence of hypertension. A comprehensive neuropsychological battery including tests of attention, memory, language and visuospatial skills was administered. RES…

Electrical remodelling of chronic atrial fibrillation.

1. It is now recognized that atrial fibrillation (AF) is not a benign condition, as it is associated with a 40% increase in mortality and a doubling of the risk of stroke. 2. The development of AF leads to mechanical, electrophysiological and cellular changes in the atria that tend to sustain AF. This process is known as atrial remodelling. 3. The three electrophysiological elements in the atria that initiate and sustain AF are: (i) shortening of the refractory period and an increase in dispersion; (ii) slowing of conduction velocity; and (iii) the presence of triggering foci. 4. As AF is a he…

Carotid stenosis associated with atrial fibrillation.

PURPOSE AND SETTING: To describe the prevalence and risk factors for carotid stenosis in TIA/stroke patients with non valvular atrial fibrillation (AF) and to compare clinical and CT characteristics of TIA/stroke in AF patients with and without carotid stenosis. SUBJECTS: 50 TIA/stroke patients with AF who had ultrasound investigation of the extracranial vessels, included in a prospective hospitalar registry. RESULTS: Twenty-two patients had some degree of carotid stenosis, but only 5 had more than 50% stenosis, including one with occlusion. Stenosis was neither more frequent nor more severe…

[Atrial fibrillation: embolic risk and prevention].

When atrial fibrillation (AF) complicates rheumatic heart disease, the risk of stroke is 17 times that of patients in sinus rhythm and full anticoagulation is mandatory. Non-rheumatic AF carries a lower risk--5% per annum, a 5-fold increase. Four major trials (SPAF, AFASAK, BAATAF, CAFA) have lately examined thromboembolic prophylaxis in this group of patients. These randomized prospective open studies showed a 56-86% reduction in stroke and systemic embolism in patients receiving full anticoagulation compared with placebo. In older people, the BAATAF trial of low-dose warfarin (INR = 1.5-2.7)…

Pathophysiology of atrial fibrillation: current aspects.

The existence of independent wandering wavelets as the basis for fibrillation was postulated by Moe and coworkers in their multiple wavelet … highest degree of inhomogeneity throughout the myocardium. The role of the second prerequisite for atrial fibrillation, the length of the excitation wave, has also recently been studied in chronic instrumented conscious dogs. Conduction velocities, refractory periods and wavelengths were measured during the administration of a variety of drugs and correlated with th …

Atrial fibrillation associated with autonomic dysreflexia.

Autonomic dysreflexia is an acute disorder of autonomic homeostasis occurring in persons with spinal cord lesions above the major splanchnic sympathetic outflow. Atrial fibrillation is a serious cardiac arrhythmia characterized by disorganized atrial depolarization and an irregular ventricular response. Previously reported complications of autonomic dysreflexia have included seizures, strokes and cardiac arrhythmias other than atrial fibrillation. We present a 60-yr-old man with C-5 incomplete quadriplegia in whom atrial fibrillation was apparently induced by an episode of autonomic dysreflexi…

Clinical approach to paroxysmal atrial fibrillation.

In addition to the role of the electrophysiological substrate, paroxysmal atrial fibrillation depends on the modulation of the atrial tissues by the autonomic nervous system. Experimentally, models of atrial fibrillation can be based on either adrenergic or vagal stimulations that provoke the arrhythmia by disturbing in a different way conduction and refractory periods of the atrium. Clinically, the role of the autonomic nervous system can be suspected from the clinical history, and paroxysmal attacks can typically be observed either at daytime or at night, at exercise or at rest. Careful atte…

Towards improved control of atrial fibrillation.

Patients with atrial fibrillation frequently show a wide variation in heart rate with digoxin therapy. We have compared the effect on heart rate variability, of doubling the digoxin dosage or adding verapamil 120 mg daily in a randomized cross-over study in 14 patients. Twenty-four hour ambulatory electrocardiographic recordings, six minute walking tests and palpitation and breathlessness scores were obtained on each regime. All patients exhibited a diurnal pattern in heart rate variability. Both treatments significantly lowered heart rate but high dose digoxin lowered minimum heart rate signi…
