BACKGROUND: Recent investigations have shown that cure of patients with symptomatic tachyarrhythmias related to an accessory atrioventricular pathway may be achieved by closed-chest electrode catheter ablation of the accessory connection. Direct current shocks have primarily been used for this purpose, but its applicability is limited because of the lack of controlled titration of electrical energy, the infliction of barotrauma, and the need for general anesthesia. Early recognition of Lyme carditis is critical to preventing unnecessary pacemaker implantation for conduction abnormalities associated with this tick-born infection. Intracardiac stimulation (IS) has been widely used as the diagnostic method for various kinds of arrhythmias. Our results as to IS are as follows: 1) Since sinus recovery time (SNRT) closely correlated with basic cycle length (BCL) in 71 normal cases (r = 0.82, p less than 0.001), upper range of SNRT in normal was expressed as SNRT = 1.4BCL + 451 msec. 2) Paroxysmal supraventricular tachycardia (PSVT) induced by IS was fairly similar to the spontaneous PSVT (107 cases). This is a study of the course of the conduction system in two cases of hypoplasia of the aortic tract complex, one with mitral stenosis and the other with mitral atresia. Radiofrequency catheter ablation has been established as a non-surgical curative treatment modality for the management of patients with paroxismal supraventricular tachycardia. Success rates for atrioventricular node reentrant tachycardia and accessory pathway-mediated macroreentrant tachycardia exceed 95%. Over a 13-year period, 18 pediatric patients received temporary transvenous pacing catheters for symptomatic bradycardia due to complete atrioventricular block or sinoatrial node dysfunction. Right ventricular wall perforation occurred in two patients. The utility of two-dimensional echocardiography in the detection and assessment of myocardial perforation by temporary pacing catheters is discussed.
