Local Pressure Application Effects on Neurological and Circulatory Function.

BACKGROUND: Pain and discomfort reported during sitting is a significant problem for aviators during prolonged missions. Previous work has d … the development of neurological and circulatory alterations due to local pressure application in an aviation specific functional position. These results may be used in the development of future interventions to mitigate the negative effects of localized pressure in military aviators.Games KE, Lakin JM, Quindry JC, Weimar WH, Sefton JM. Coronary artery disease in the military patient.

Ischaemic heart disease is the most common cause of sudden death in the UK, and the most common cardiac cause of medical discharge from the Armed Forces. This paper reviews current evidence pertaining to the diagnosis and management of coronary artery disease from a military perspective, encompassing stable angina and acute coronary syndromes. Emphasis is placed on the limitations inherent in the management of acute coronary syndromes in the deployed environment. Occupational issues affecting patients with coronary artery disease are reviewed. Consideration is also given to the potential for c

Brain Recovery after a Plane Crash: Treatment with Growth Hormone (GH) and Neurorehabilitation: A Case Report.

UNLABELLED: The aim of this study is to describe the results obtained after growth hormone (GH) treatment and neurorehabilitation in a young man that suffered a very grave traumatic brain injury (TBI) after a plane crash. METHODS: Fifteen months after the accident, the patient was treated with GH, 1 mg/day, at three-month intervals, followed by one-month resting, together with daily neurorehabilitation. Blood analysis at admission showed that no pituitary deficits existed. At admission, the patient presented: spastic tetraplegia, dysarthria, dysphagia, very severe cognitive deficits and joint

Delayed Diagnosis of Pelvic Hematoma without Fracture Due to Military Parachuting.

The U.S. military has been conducting static-line parachute jumps for nearly a century. Beginning with World War II, military forces have also employed full-scale airborne operations as a method for insertion into combat. Through the years, injuries from blunt trauma as a result of static-line parachute jumps have evolved little with the refinement of equipment, training, and tactics. Parachute jumps continue to invoke primarily musculoskeletal injuries, especially to the lower extremities, back, neck, and head. These injuries are usually straightforward in their presentation and diagnosis. We

The "flying" bile duct: avulsion of the common bile duct in a plane crash survivor.

Blunt trauma is an unusual cause of extrahepatic bile duct injury. This is a case of a 51-year-old gentleman who sustained a significant seatbelt injury in a plane crash. Laparotomy, performed due to persistent abdominal pain, revealed that the common bile duct (CBD) was completely avulsed from the duodenum. Following insertion of drains and transfer to a hepatobiliary centre, the devascularised CBD was excised and replaced with a roux-en-y hepaticojejunostomy. Necrotic tissue was debrided from the pancreatic head. A persistent bile leak developed from the sub-hepatic drain. Repeat laparotomy

Detection of orthopaedic foot and ankle implants by security screening devices.

BACKGROUND: A common question asked by patients contemplating foot and ankle surgery is whether the implants used will set off security screening devices in airports and elsewhere. Detectability of specific implants may require the orthopaedic surgeon to provide attestation regarding their presence in patients undergoing implantation of these devices. Only two studies have been published since security measures became more stringent in the post-9/11 era. None of these studies specifically focused on the large numbers of orthopaedic foot and ankle implants in use today. This study establishes e

[Human diffusion of arthropod disease vectors throughout the world].

The present distribution of animals and plants throughout the world is the result of an evolutionary process involving tectonic, climatic and biotic factors. Humans, since their appearance, have contributed to the spreading of many species including disease vectors and pests. When humans left their native African home, they brought with them ectoparasites such as lice and acarids. During the neolithic era, humans were leading domesticated animals which carried their own parasites into new areas. Dwelling commensals, flea, bugs, triatomids, flies, and cockroaches followed human migrations. In t

Cyanide in blood by gas chromatography with NP detector and acetonitrile as internal standard. Application on air accident fire victims.

An improved gas chromatographic method for HCN in blood with acetonitrile as an internal standard has been developed. The necessity to test for cyanide on samples from air accident fire victims is discussed in view of the fact that carbon monoxide measurements usually do not correlate with cyanide findings. Nine cases (out of 67), with blood CO saturation of less than 20% and cyanide concentration over 2 mg/L, are described.

Helicopter accident survivability.

Army Air Corps accident and fatality rates have now reached levels which compare favourably with data from other civilian and military sources. This improvement is the result of enhanced helicopter design and parallel progress in aircrew training. The introduction of new generations of turbine powered rotor craft has largely eliminated mechanical failure as the cause of accident. As a result 75% of Army Air Corps accidents are due to pilot error. This contribution is likely to increase in the future as the pilot's task is made more difficult by the incumberance of personal equipment. Methods w

Smoke toxicity of common aircraft carpets.

The smoke toxicity of three carpets commonly available for use in commercial aircraft was determined by ignition in a specially designed smoke apparatus. Rats were exposed for 15 min to three different fuel loads, on a weight-to-volume basis. Evaluation was by mortality, time of useful function (TUF), and unconsciousness. No deaths were noted with carpets A or C at 64 mg/l or 128 mg/l fuel load concentration; at 256 mg/l, 42% mortality resulted from carpet A and 4.5% with carpet C. Exposure to carpet B resulted in a mortality of 4.3%, 72.5%, and 100% at the three concentrations. The TUF data a

Fatal gyroplane crash.

Gyroplanes are mainly used for sports aviation. Experience is required when flying these special aircraft because in 40% of gyroplane crashes, the passengers do not survive. Herein, a fatal gyroplane crash of a tandem-seat gyroplane with two occupants is reported. The occupants, both 49-year-old males, fell from a height of approximately 20 m to 30 m. In both cases, the cause of death was polytrauma with typical findings of deceleration trauma. The pilot, who probably lost his helmet in the crash, had a ring fracture at the base of his skull. As there is not much literature on autopsy findings…

A review of X-ray explosives detection techniques for checked baggage.

In recent times, the security focus for civil aviation has shifted from hijacking in the 1980s, towards deliberate sabotage. X-ray imaging provides a major tool in checked baggage inspection, with various sensitive techniques being brought to bear in determining the form, and density of items within luggage as well as other material dependent parameters. This review first examines the various challenges to X-ray technology in securing a safe system of passenger transportation. An overview is then presented of the various conventional and less conventional approaches that are available to the a…

Adaptation and failure of adaptation to extreme natural environments.

Although the environmental stresses to which man is subjected on the ground are less than those commonly encountered in aviation or under water, they may still exceed an individual's powers of adaptation. Extremes of temperature, commonly encountered in the Arctic or the tropics, may occur in regions of normally temperate climate and lead to failure of temperature regulation, resulting in hypothermia, frostbite, heat exhaustion, or heat stroke. High mountains impose additional hazards due to high winds and lack of oxygen, and deep mines are dangerous work-places because of high temperature and…

REBOA at Role 2 Afloat: resuscitative endovascular balloon occlusion of the aorta as a bridge to damage control surgery in the military maritime setting.

Role 2 Afloat provides a damage control resuscitation and surgery facility in support of maritime, littoral and aviation operations. Resuscitative endovascular balloon occlusion of the aorta (REBOA) offers a rapid, effective solution to exsanguinating haemorrhage from pelvic and non-compressible torso haemorrhage. It should be considered when the patient presents in a peri-arrest state, if surgery is likely to be delayed, or where the single operating table is occupied by another case. This paper will outline the data in support of endovascular haemorrhage control, describe the technique and e…

Commercial Air Travel for Passengers With Cardiovascular Disease: Recommendations for Common Conditions.

The exponential growth of commercial flights has resulted in an explosion of air travelers over the last few decades, including passengers with a wide range of cardiovascular conditions. Notwithstanding the ongoing COVID-19 pandemic that had set back the aviation industry for the next 1-2 years, air travel is expected to rebound fully by 2024. Guidelines and evidence-based recommendations for safe air travel in this group vary, and physicians often encounter situations where opinions and assessments on fitness for flights are sought. This article aims to provide an updated suite of recommendat…

Atrial fibrillation and flutter: aeromedical considerations. New strategies for management and intervention.

Few cases of atrial fibrillation can be completely excused from the possibility of having some impact of potential importance in the aviation environment. The clinical pattern in paroxysmal disturbance is very wide and possible incapacitating phenomena are mainly haemodynamic. In contrast, chronic atrial fibrillation is mostly well-tolerated with only minimal haemodynamic effect, but there is a risk of thromboembolism, suggesting the need for anti-thromboembolic treatment. A subgroup with chronic atrial fibrillation with a very low risk may be defined, allowing avoidance of anti-thromboembolic…
