[Newborn screening for severe T and B lymphocyte deficiencies in Switzerland].

Severe Combined Immunodeficiency (SCID) is one of the most severe forms of Primary Immunodeficiencies (PID) and leads to a potentially fatal course of disease without early and definitive treatment. Adequate management, from the first days of life, can improve the survival and outcome of patients with SCID. This can be achieved through newborn screening (NBS) based on the measurement of T-cell receptor excision circles (TREC). Already present in many countries, this NBS test was introduced in Switzerland in January 2019 on a pilot phase. In addition to the assessment of TRECs, the measurement

The chronic lymphocytic leukemia microenvironment: Beyond the B-cell receptor.

Malignant B cells accumulate in the peripheral blood, bone marrow, and lymphoid organs of patients with chronic lymphocytic leukemia (CLL). In the tissue compartments, CLL shape a protective microenvironment by coopting normal elements. The efficacy of drugs that target these interactions further underscores their importance in the pathogenesis of CLL. While the B cell receptor (BCR) pathway clearly plays a central role in the CLL microenvironment, there is also rationale to evaluate agents that inhibit other aspects or modulate the immune cells in the microenvironment. Here we review the main

MicroRNA as biomarkers and regulators in B-cell chronic lymphocytic leukemia.

Early cancer detection and disease stratification or classification are critical to successful treatment. Accessible, reliable, and informative cancer biomarkers can be medically valuable and can provide some relevant insights into cancer biology. Recent studies have suggested improvements in detecting malignancies by the use of specific extracellular microRNAs (miRNAs) in plasma. In chronic lymphocytic leukemia (CLL), an incurable hematologic disorder, sensitive, early, and noninvasive diagnosis and better disease classification would be very useful for more effective therapies. We show here

T-cell CD38 expression in B-chronic lymphocytic leukaemia.

B-cell chronic lymphocytic leukaemia (B-CLL) is a heterogeneous disease with some patients having an indolent course never needs treatment, while others having rapidly progressive one requires intensive treatment. In recent decades, numerous prognostic markers, such as immunoglobulin variable region heavy-chain (IgVH) mutational status, ZAP-70 and the expression of CD38 on leukaemic cells were introduced to screen for patients likely to have progressive course of B-CLL bearing the potential to facilitate risk-adapted treatment strategies. In B-CLL, T cell function is shown to be dysregulated.

Association of B-chronic lymphocytic leukemia and acute myeloid leukemia.

A 62-year-old man presented with fatigue, pallor and mild weight loss. Laboratory studies showed Hb 7.6 g/dl, Hct 21.8%, WBC 108x10(9)/1, PLT 143x10(9)/1. At morphological examination, circulating cells appeared as 60% blasts and 40% lymphocytes, with smudge cells. A bone marrow aspirate showed infiltration by blasts (50%) and lymphocytes (40%); alpha-naphtyl-acetate esterase was positive in 90% of blasts, while myeloperoxidase was positive in 10%. The immunologic phenotype of blasts was characterized by the co-expression of CD13, CD33, CD14, CD4, CD15, CD64, CD117, HLA-DR, CD11b. Lymphocytes

Cutaneous sarcoidlike lesions in B-cell chronic lymphocytic leukemia.

The development of systemic sarcoidosis in patients with malignancies is a well-known phenomenon. However, cutaneous sarcoidlike lesions are a rare finding. We report 2 patients with B-cell chronic lymphocytic leukemia and hypogammaglobulinemia who presented isolated cutaneous sarcoidlike lesions 9 years after the diagnosis of the hematologic malignancy in one case and after the second course of chemotherapy regimen in the other case. The role of cytokines released from malignant cells and of the hypogammaglobulinemia facilitating an unidentified infectious agent is questionable.

Composite mycosis fungoides and B-cell chronic lymphocytic leukemia.

Concordant or composite mycosis fungoides and B-cell chronic lymphocytic leukemia (B-CLL) is exceedingly rare, with only 10 cases previously described to our knowledge. We report a case of a 64-year-old woman who developed generalized erythroderma 5 years after the diagnosis of early stage B-CLL. Over the next 6 years of her clinical course multiple sequential samples of skin, peripheral blood, and one enlarged lymph node were studied in detail by flow cytometry, immunohistochemistry, molecular diagnostics, and electron microscopy. The progressive cutaneous infiltrates were initially character

Oligoclonal B lymphocyte expansion in the synovium of a patient with Behçet's disease.

OBJECTIVE: Plasma cell infiltration is observed in recurrent arthritis associated with Behçet's disease (BD). The immune mechanism underlying B lymphocyte proliferation in the synovium is unclear. One hypothesis involves nonspecific polyclonal activation and another involves antigen-driven activation. The present study was undertaken to test both hypotheses and identify immunoglobulin genes that are clonally expanded in the synovium. METHODS: Peripheral blood lymphocytes (PBL) and synovial cells from a patient with BD and PBL from a healthy control subject were obtained. Complementarity-deter

Disturbed peripheral B lymphocyte homeostasis in systemic lupus erythematosus.

In patients with active systemic lupus erythematosus (SLE), a marked B lymphocytopenia was identified that affected CD19(+)/CD27(-) naive B cells more than CD19(+)/CD27(+) memory B cells, leading to a relative predominance of CD27-expressing peripheral B cells. CD27(high)/CD38(+)/CD19(dim)/surface Ig(low)/CD20(-)/CD138(+) plasma cells were found at high frequencies in active but not inactive SLE patients. Upon immunosuppressive therapy, CD27(high) plasma cells and naive CD27(-) B cells were markedly decreased in the peripheral blood. Mutational analysis of V gene rearrangements of individual B

B-cell chronic lymphocytic leukemia: a bird of a different feather.

PURPOSE: To review the recent major advances in the molecular and cell biology of B-cell chronic lymphocytic leukemia (B-CLL). METHODS: We analyzed the nature of malignant B-CLL B cells and their interactions with the microenvironment. RESULTS: B-CLL is a malignancy of a mantle zone-based subpopulation of anergic, self-reactive, activated CD5+ B cells devoted to the production of polyreactive natural autoantibodies. It is the quintessential example of a human malignancy that primarily involves defects in the induction of programmed cell death. An abnormal karyotype is observed in about 50% o

Morphological and immunophenotypical characterization of Japanese B-cell lymphocytic leukemia.

We have analyzed the clinical and laboratory features of 42 patients with B-cell leukemia. Based on the FAB criteria, the cases were classified in 3 groups: I) typical CLL 15, II) atypical CLL 9 which included 6 cases with large cells, and III) B-cell lymphoma in leukemic phase 18. Cases diagnosed as typical CLL (group I) had similar features to those seen in CLL patients from Western countries. The morphology and markers in cases from group III corresponded to B-cell lymphoma in leukemic phase. On the other hand, group II included 3 cases classified as atypical CLL according to FAB criteria.

T cells and CD45R expression in B-chronic lymphocytic leukemia.

A major secondary immunodeficiency exists in B cell derived chronic lymphocytic leukemia (B-CLL) which involves both humoral and cellular immunity and is largely unexplained. Several T cell subset redistributions have been noted in B-CLL. We therefore examined in more detail the immunophenotype of T cells by assessing CD45R expression using a two-colour immunofluorescence technique. The proportion of CD45R+ cells among CD3+, CD4+ and CD8+ cells in B-CLL was significantly (p less than 0.001) higher than in corresponding normal cell populations. We also sought relationships between major T cell

Mechanisms of B-lymphocyte suppression induced by acute physical exercise.

Blood mononuclear cells from 20 healthy untrained volunteers were isolated before, during, 2 hr and 24 hr after bicycle exercise at 80% of VO2 max for 1 hr. A reverse plaque forming cell assay was used to investigate B-lymphocyte function. Stimulation with pokeweed mitogen, recombinant interleukin 2 and Epstein-Barr virus resulted in significant decreases in numbers of IgG-, IgM- and IgA-secreting blood cells during as well as 2 hr after exercise, with reversal to pre-exercise values 24 hr later. During and after physical activity we found an unchanged concentration of CD20-positive B lymphocy

[B- and T-lymphocyte compartments in the spleen in idiopathic thrombocytopenic purpura].

The exstirpated spleens of 62 patients in each case with idiopathic thrombocytopenic purpura (ITP) and traumatic rupture of the spleen were comparatively examined macroscopically, histologically and morphometrically. The mean follicle diameters of the spleen were significantly greater in ITP patients than in those of controls. With 500 microns identified as an upper limit of normal follicle diameter, ITP patients could be divided into two groups of equal size below and above this limiting value. The thrombocyte values determined at the time of splenectomy and 6 months before it were significan

Normal cellular counterparts of B cell chronic lymphocytic leukemia.

In an attempt to compare B cell chronic lymphocytic leukemia (B-CLL) with its normal cellular counterpart, the cell surface phenotype of 100 cases of B-CLL was determined by using a panel of monoclonal antibodies (MoAbs) directed against B cell-restricted and -associated antigens. The majority of B-CLL cells expressed Ia, B4 (CD19), B1 (CD20), B2 (CD21), surface immunoglobulin (sIg), and T1 (CD5) but lacked C3b (CD35) receptors. In contrast, the overwhelming majority of small unstimulated B cells expressed Ia, B4, B1, B2, sIg, and C3b receptors but lacked detectable T1. Small numbers of weakly

[T- and B-lymphocyte hyperreactivity in patients with progressive systemic sclerosis].

In an attempt to characterize immunological reactions in patients with progressive systemic sclerosis (PSS) mononuclear cells were tested for their ability to secrete interleukin-2 (II-2), and for their response to the T-cell independent B-cell mitogen anti-mu sepharose. Compared with patients with systemic lupus erythematosus (SLE), patients with PSS showed a marked enhancement in their response to anti-mu. With regard to II-2 secretion mononuclear cells from PSS patients tended to show increased responses as well. Our data provide evidence for hyperreactivity at both the T- and B-cell level

Perinatal influences on in vitro B lymphocyte differentiation in human neonates.

In vitro differentiation of B lymphocytes present in cord blood mononuclear cell preparations into immunoglobulin secreting cells was studied in 126 neonates with gestational ages (GA) ranging from 20 to 44 wk. Eight infants had a GA less than 27.9 wk, 24 had GA 28-32.9 wk, 30 had GA 33-37.9 wk, 51 had GA 38-41.9 wk, and 13 had GA above 42 wk. B cell differentiation in response to pokeweed mitogen plus hydrocortisone was assessed using a plaque forming cell assay. All neonates had a measurable plaque-forming cell response in this assay. An increased plaque-forming cell response was observed in

B lymphocyte precursors in embryonic and adult W anemic mice.

Mice homozygous for mutations at the dominant spotting or W locus on chromosome 5 have been extensively used as models of severe macrocytic anemia caused by defective hemopoietic stem cells. We examined cells of the developing B lineage in adult and embryonic W anemic mice both by phenotypic analyses and by three distinctly different functional assays for B lymphocyte precursors. Adult W/Wv mice had normal numbers of B cells in the spleen and bone marrow, and normal numbers of pre-B cells and cells identified by a monoclonal antibody directed to a B lineage cell surface antigen (14.8) in the b

Role of B cells in the stimulation of syngeneic mixed lymphocyte responses.

Two different approaches were used to examine the role of B cells in the stimulation of syngeneic MLR. The relative inability of spleen and peritoneal exudate cells from B cell deficient mice, treated with anti-mu from birth, to serve as stimulator cells in SMLR was previously shown in SJL mice and confirmed in BALB/c mice in the present studies. Preincubation of cells from anti-mu treated mice with serum Ig does not enhance their ability to stimulate. Upon stimulation with normal spleen cells responses from anti-mu treated mouse T cells are not deficient. Responses of thymus cells from neonat
