Clinical and microbiologic data of 296 patients with anaerobic bacteremia were reviewed. Anaerobes were isolated with aerobic or facultative bacteremia in 23 instances. The Bacteroides fragilis group accounted for 148 (70%) of 212 isolates of Bacteroides species. B. fragilis accounted for 78% and B. thetaiotaomicron for 14%. Aspiration of the exudate through open perforation was performed in 48 children with chronic suppurative otitis media. Eighty-two aerobic and 93 anaerobic isolates were recovered. Tympanocentesis was performed in 32 pediatric patients with chronic recurrent suppurative otitis media. The aspirate was cultured aerobically and anaerobically. In the present study the susceptibility of 200 blood culture isolates of anaerobic bacteria to benzylpenicillin, clindamycin, metronidazole, imipenem and piperacillin-tazobactam was examined. Metronidazole, imipenem, and piperacillin-tazobactam showed the highest activity, with 98.5% of the isolates being fully susceptible to either agent. Anaerobic bacteremia occurred in 68 patients over the age of 60 in a university hospital. These elderly patients were more likely than younger patients to have an underlying malignancy. Anaerobes involved included Bacteroides fragilis group (64 isolates), Bacteroides melaninogenicus group (11) and Clostridium species (11). The in vitro activities of 17 antimicrobial agents were evaluated against 46 clinical isolates of formate/fumarate-requiring anaerobic gram-negative bacilli. The authors present a case of a patient with sickle-β thalassaemia (Sβ(0) Thal) who had bilateral psoas abscesses on a background of splenectomy in early childhood. The patient also turned out to have vertebral osteomyelitis and hydronephrosis on the side of the larger abscess. The only organism recovered from the patient was a Bacteroides species. The patient was managed with percutaneous drainage and intravenous antibiotics and made a full recovery.

Rubber band ligation is a commonly employed office procedure for the eradication of symptomatic internal hemorrhoids. Since 1980 increasingly frequent reports of an often fatal complication--"pelvic cellulitis"--have appeared. Death has been avoided for some of these patients by early recognition and treatment. One such survivor is reported here, in a case report that illustrates the value of early recognition of symptoms and appropriate diagnostic and therapeutic intervention.

Inflammatory pelvic disease secondary to infection with Enterobius vermicularis is rare in that pinworms are usually asymptomatic inhabitants of the terminal small bowel and cecum. When these parasites do cause symptoms, however, there is a spectrum of gastrointestinal and genitourinary complaints. A case is presented of a woman with bilateral abdominal pain that was found to be due to a combination of parasitic and bacterial (Bacteroides fragilis) infection. The parasitic fallopian tube infection may have predisposed the fallopian tubes to a microbial tuboovarian abscess.

A study was performed of 102 obstetric-gynecologic patients who were thought to have sepsis or a pelvic abscess. Fifty-three of these women received chloramphenicol and 49 received clindamycin. In addition, all patients received penicillin or a similar antibiotic and an aminoglycoside. Similar clinical results were observed with the two treatment regimens.
