Atrial Fibrillation in Patients with Hypertrophic Cardiomyopathy and Cardiac Amyloidosis: From Clinical Management to Catheter Ablation Indication.

Atrial fibrillation (AF) is the most common arrhythmia in patients affected by cardiomyopathies. Reports estimate a prevalence of 27% in patients with hypertrophic cardiomyopathy (HCM) and 40% in patients with cardiac amyloidosis (CA). The presence of AF typically results in progressive functional decline, an increased frequency of hospitalizations for heart failure, and a higher thromboembolic risk. Medical management using mainly beta-blockers or amiodarone has produced variable outcomes and a high rate of recurrence. Catheter ablation reduces symptom burden and complications despite a moder

Effects of opiate-like peptides, morphine, and naloxone in the photosensitive baboon, Papio papio.

The effects of intracerebroventricular (i.c.v.) or systemic injections of Met- or Leu-enkephalin, beta-endorphin, FK 33.824 (D-Ala2, MePhe4, Met(O5)-ol-enkephalin) and of morphine and naloxone have been studied in baboons, Papio papio, which spontaneously show photically induced epileptic responses. Animals were chronically implanted with epidural or deep recording electrodes and a cannula in one lateral ventricle, and tested whilst seated in a primate chair. In some animals the natural syndrome was enhanced by the prior administration of DL-allylglycine, 100--200 mg/kg, i.v. Met- or Leu-enkep

Pre-clinical Cushing's syndrome with body weight loss and normal circadian rhythm of blood pressure.

A 68-year-old woman was referred for evaluation of an incidentally discovered left adrenal mass. Her chief complaint was body weight loss. She showed no symptoms or signs suggestive of Cushing's syndrome. The circadian rhythm of blood pressure was also normal. Hormonal assessment revealed an abnormal diurnal variation in serum cortisol levels, suppressed baseline plasma adrenocorticotrophic hormone, and nonsuppression of serum and urinary cortisol with the dexamethasone suppression test. Adrenal scintigraphy with (131)I-6-beta-iodomethyl-norcholesterol showed uptake on the left adrenal and inh

Personalized management of angina and heart failure in clinical practice in the Middle East: a narrative review.

More interventions that better manage cardiovascular disease are urgently needed in the Middle East. To discuss this issue, we held a symposium at the Heart Masters Middle East 2023 congress (Dubai, UAE; May 2023) on personalized management of angina and heart failure (HF). This narrative review summarizes the content of our symposium. Many patients with chronic stable angina have ongoing symptoms and poor quality of life (QoL) despite beta-blocker + calcium-channel blocker therapy and revascularization. Further, angina is often under-recognized in clinical practice. Clinicians should consider

How to Monitor the Neuroimmune Biological Response in Patients Affected by Immune Alteration-Related Systemic Diseases.

The clinical management of patients affected by systemic diseases, including cancer and autoimmune diseases, is generally founded on the evaluation of the only markers related to the single disease rather than the biological immuno-inflammatory response of patients, despite the fundamental role of cytokine network in the pathogenesis of cancer and autoimmunity is well known. Cancer progression has appeared to be associated with a progressive decline in the blood levels of the main antitumor cytokines, including IL-2 and IL-12, in association with an increase in those of inflammatory cytokines,

Intravenous sotalol for the termination of supraventricular tachycardia and atrial fibrillation and flutter: a multicenter, randomized, double-blind, placebo-controlled study. Sotalol Multicenter Study Group.

Sotalol is an antiarrhythmic agent with combined beta-blocking and class III antiarrhythmic properties. This study was designed to assess the safety and efficacy of sotalol in terminating supraventricular tachycardia (SVT), atrial fibrillation (AFib), and atrial flutter (AFl). Ninety-three patients with spontaneous or induced SVT (n = 45) or AF (AFib or AFl; n = 48) with a ventricular rate of > or = 120 beats/min were studied. In the first phase, the double-blind phase, patients were randomly assigned to receive placebo or intravenous (i.v.) sotalol, 1.0 or 1.5 mg/kg. If SVT or AF did not conv

Effect of stimulation of endogenous melatonin secretion during constant light exposure on 6-sulphatoxymelatonin rhythmicity in rats.

When light is presented unexpectedly at night to rats, melatonin production and secretion is acutely inhibited and the time of onset of production on the subsequent night is altered. In a series of experiments, we examined the effects of 6-12 hr light (200 lux) at night on melatonin metabolite excretion (6-sulphatoxymelatonin, aMT.6S). During the light exposure, we administered isoproterenol to stimulate endogenous production of melatonin by the pineal gland to determine if replacement of melatonin would block any phase shifting effects of the light. Exposure to 6 hr of light either during the

Cardiovascular Disease: Other Common Arrhythmias.

Premature atrial contractions (PACs) occur in nearly all individuals. Although typically asymptomatic, they can cause palpitations. PACs previously were considered benign, but there is increasing recognition that frequent PACs are associated with developing atrial fibrillation. After potentially reversible causes (eg, electrolyte abnormalities, hyperthyroidism) are eliminated, symptomatic PACs can be treated with beta blockers; some patients are candidates for ablation. Premature ventricular contractions (PVCs) also are common, occurring in more than two-thirds of the population. They typicall

Chronic pain and oscillatory neuroplasticity: Review on impaired compensatory network responses in resting-state electroencephalography.

Chronic pain is a multidimensional condition that involves persistent alterations in sensory, cognitive, and affective processes. Owing to its high temporal resolution and capacity to measure large-scale neural communications, electroencephalography (EEG) has emerged as a promising tool for identifying objective biomarkers of chronic pain. However, the findings of existing studies remain heterogeneous, which limits their clinical translation. In this narrative review, we synthesized recent resting-state EEG functional connectivity studies across a range of chronic pain conditions, highlighting

Excessive gamma and beta oscillations in manic states across mood and psychotic disorders.

The clinical manifestations of mood and psychotic disorders encompass phasic and transdiagnostic features. The complexity of these symptoms may hamper the development of biomarkers for these diagnostic categories. In the present study, machine learning was employed to cluster their transdiagnostic clinical manifestations as a means of developing state-dependent biomarkers with electroencephalography (EEG). This data-driven clustering classified the items of multiple symptom scales into three symptom domains, which subsequently stratified patients with major depressive, bipolar, and schizophren

Motion processing after sight restoration: No competition between visual recovery and auditory compensation.

The present study tested whether or not functional adaptations following congenital blindness are maintained in humans after sight-restoration and whether they interfere with visual recovery. In permanently congenital blind individuals both intramodal plasticity (e.g. changes in auditory cortex) as well as crossmodal plasticity (e.g. an activation of visual cortex by auditory stimuli) have been observed. Both phenomena were hypothesized to contribute to improved auditory functions. For example, it has been shown that early permanently blind individuals outperform sighted controls in auditory m

EEG theta/beta ratio as a potential biomarker for attentional control and resilience against deleterious effects of stress on attention.

Anxious stress compromises cognitive executive performance. This occurs, for instance, in cognitive performance anxiety (CPA), in which anxiety about one's cognitive performance causes that performance to actually deteriorate (e.g., test anxiety). This is thought to result from a prefrontal cortically (PFC) mediated failure of top-down attentional control over stress-induced automatic processing of threat-related information. In addition, stress-induced increased catecholamine influx into the PFC may directly compromise attentional function. Previous research has suggested that the ratio betwe

Functional heterogeneity of beta bursts in childhood reveals a dimensional neural signature of motor skill.

Activity in the beta frequency band appears to play a critical role in sensorimotor processing, with transient "bursts" exhibiting distinct waveform motifs that may reflect different neural computations. However, how these dynamics relate to motor development and neurodevelopmental conditions remains unclear. Here, we used EEG to investigate sensorimotor beta bursts in children with and without developmental coordination disorder (DCD). We characterized beta bursts during action execution and observation of gross and fine motor actions. Conventional spectral features and overall burst rates di

Noninvasive Temporal Interference Stimulation of the Subthalamic Nucleus in Parkinson's Disease Reduces Beta Activity.

BACKGROUND: Temporal interference stimulation (TIS) is a novel noninvasive electrical stimulation technique to focally modulate deep brain regions; a minimum of two high-frequency signals (f1 and f2 > 1 kHz) interfere to create an envelope-modulated signal at a deep brain target with the frequency of modulation equal to the difference frequency: Δf = |f2 - f1|. OBJECTIVE: The goals of this study were to verify the capability of TIS to modulate the subthalamic nucleus (STN) with Δf and to compare the effect of TIS and conventional deep brain stimulation (DBS) on the STN beta oscillations in pa

Electrophysiological changes in the acute phase after deep brain stimulation surgery.

BACKGROUND: With the introduction of sensing-enabled deep brain stimulation devices, characterization of long-term biomarker dynamics is of growing importance for treatment optimization. The microlesion effect is a well-known phenomenon of transient clinical improvement in the acute post-operative phase. While beta band activity is confirmed as a reliable biomarker for bradykinesia using chronic recordings, little is known about the ideal time point for initial electrophysiology-based programming. OBJECTIVE: To investigate the microlesion effect impact in chronic biomarker recordings. METHOD
