Parachute mitral valve accompanied by bicuspid aortic valve on three-dimensional transesophageal echocardiography.

We report the findings of three-dimensional (3D) transesophageal echocardiography (TEE) in a patient with a parachute mitral valve (MV) acco … limit in diastole with an MV area of 1.6 cm. Moreover, the left ventricle increased in size and the bicuspid AV was malformed. Continuous wave Doppler angiograph showed that the flow rate increased to 398 cm/seconds at the AV orifice area. A 3D form of the MV structure was observed from the left ventricle using 3D-TEE inspection. The anterolateral papillar …

Altered microRNAs in bicuspid aortic valve: a comparison between stenotic and insufficient valves.

BACKGROUND AND AIM OF THE STUDY: Bicuspid aortic valve (BAV), the most common form of congenital heart disease, is a leading cause of aortic stenosis (AS) and aortic insufficiency (AI). AS is typically caused by calcific valve disease. Recently, microRNAs (miRNAs) have been shown to modulate gene expression. The study aim was to examine the miRNAs that were altered in the aortic valve leaflets of patients with AS compared to those in patients with AI. In-vitro experiments were also carried out to determine if these miRNAs could modulate calcification-related genes. METHODS: Aortic valve sampl…

The presence of bicuspid aortic valve does not predict ventricular septal defect type.

Previous studies have identified an increased incidence of bicuspid aortic valve (BAV) in patients with ventricular septal defect (VSD). Because endocardial cushion remodeling contributes to both the formation of semilunar valves and ventricular septation, we hypothesized that examination of humans with BAV and VSD would identify a specific VSD type. We evaluated VSD type in pediatric patients diagnosed with BAV and VSD (n=82) and compared findings to patients diagnosed with VSD and normal aortic valve morphology (n=429). VSD type was described as conoventricular, muscular, inlet or conoseptal…

Bicuspid aortic valve: clinical approach and scientific review of a common clinical entity.

Bicuspid aortic valve (BAV) disease is becoming increasingly respected by clinicians. This enhanced appreciation is based on recognition of how common and virulent this disease really is. This disease is now known to be the most common congenital lesion affecting the human heart (with potential competition in frequency posed by mitral valve prolapse). It is estimated that the BAV lesion alone accounts for more morbidity and mortality than all the other congenital cardiac lesions combined. Many authorities feel that this disease is so virulent that every individual with a BAV will, given enough…

Successful valve repair for congenital bicuspid aortic valve and degenerated prolapsed mitral valve.

A 54-year-old man with congenital bicuspid aortic valve underwent simultaneous valve repair for aortic and mitral regurgitation. Surgical technique consisted of plication of redundant aortic valve repair and mitral annuloplasty with chordal replacement. One-year follow-up transthoracic echocardiography showed no valve regurgitation. Valve repair for both bicuspid aortic valve and mitral valve regurgitation should be the first option in this subset of patients.

The congenitally bicuspid aortic valve: how does it function? Why does it fail?

BACKGROUND: The study was intended to investigate the reason why congenitally bicuspid valves (CBAVs), which may function hemodynamically and clinically well, have a high early failure rate. METHODS: Observations were made on cryopreserved, then thawed human aortic roots containing CBAVs. Valvular function was studied in the left heart simulator using conventional and 500-frames/second cinematography, intravascular ultrasound, by preparation of silicone molds, and by computerized digital modeling. RESULTS: The function of the clinically "normal" CBAVs is characterized by (a). excessive foldi…

Valve-sparing operation for aortic root aneurysm in patients with bicuspid aortic valve.

Aortic root aneurysms are not rare in patients with bicuspid aortic valve, but valve-sparing operations to treat them remain challenging. We describe our surgical method of reconstructing only two commissures, which is extremely simple and easy to reproduce without aortic stenosis.

Left ventricular septal aneurysm in association with bicuspid aortic valve--a case report.

Diverticula of the left ventricle are rare cardiac anomalies. Most cases arise from the apex of the left ventricle and are usually found in children. Only a few cases have been documented in adults. The authors report a case of a 38-year-old woman who presented with dyspnea and chest pain. She was found to have a septal left ventricular diverticulum associated with bicuspid aortic valve, aortic stenosis, and aortic regurgitation. The aortic valve was replaced with the resection of the diverticulum. Pathologic examination confirmed the diagnosis of fibrous diverticulum.

[A case of acute dissecting aortic aneurysm associated with congenital bicuspid aortic valve].

The congenital bicuspid aortic valve is recognized as a cause of aortic dissection. But in Japan, the case of aortic dissection associated with bicuspid aortic valve is rare. A 66-year-old man was referred to our hospital with a diagnosis of acute Stanford A type dissecting aneurysm associated with stenotic bicuspid aortic valve. The chest computed tomography showed thrombosis of the pseudo lumen. An operation consisting of aortic valve replacement and graft replacement of the aneurysm was performed on emergency basis because of remarkable dilation of the ascending aorta. He has been doing wel…

[Echocardiographic assessment of aortic regurgitation and aortic root dilatation in bicuspid aortic valve].

Aortic regurgitation (AR) and aortic root dilatation in 29 consecutive patients with bicuspid aortic valves but without aortic root disease (20 males, and 9 females: aged 27-85 years) were studied using two-dimensional echocardiography. The normal ranges of aortic root dimensions were calculated from values of 185 normal subjects, as 95% confidence intervals. AR was observed in 17 patients by color flow mapping. In 12 of the 17 AR patients, no significant lesion of the aortic cusp was detected by two-dimensional echocardiography. These 12 AR patients were compared with 12 patients without AR. …

Contrasting histoarchitecture of calcified leaflets from stenotic bicuspid versus stenotic tricuspid aortic valves.

Preliminary findings from clinical trials of percutaneous balloon aortic valvuloplasty and intraoperative debridement of calcific deposits in patients with aortic stenosis have suggested that calcified, congenitally bicuspid aortic valves may be less amenable to these techniques than are calcified tricuspid aortic valves. Accordingly, we evaluated the histoarchitecture of calcific deposits in 30 operatively excised aortic valves. Light microscopic sections taken through the calcified aortic valve leaflets disclosed two principal types of histoarchitecture. In 11 aortic valves nodular calcific …

Allograft aortic valve replacement for bicuspid aortic valves with 180-degree coronary ostia.

Congenital bicuspid aortic stenosis is often an indication for aortic valve replacement in young adults and children. However, when the coronary ostia are exactly 180 degrees opposite each other, placing a trileaflet human allograft with freehand technique can be difficult. The described technique enlarges the left ventricular outflow tract beginning at the annulus level, using only allograft tissue. In addition to allowing placement of a larger allograft, this method rotates the coronary ostia toward each other, converting the aortic root to three-sinus anatomy.

An exceptional form of congenitally bicuspid aortic valve resulting in pure aortic regurgitation.

A case with an exceptional form of congenitally bicuspid aortic valve is described. A fibrous strand connected to the supra-aortic ridge supported the conjoined cusp, and severe pure aortic regurgitation was present although the fibrous strand remained intact. Aortic valve replacement was successfully performed.

Clinical Performance and Reproducibility of the LIRA Method for TAVR in Bicuspid Anatomy: The SUBLIME Study.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) in bicuspid aortic valve (BAV) anatomy remains challenging, with heterogeneous outcomes and no standardized sizing strategy. The LIRA (level of implantation at the raphe) method is a bicuspid-dedicated approach that complements standard annular sizing with a single perimeter measurement at the raphe level. A previous single-center experience suggested favorable results. OBJECTIVES: The aim of this study was to evaluate clinical performance and reproducibility of the LIRA method for TAVR sizing in a large BAV multicenter cohort. METHOD…

Effect of Bicuspid Versus Tricuspid Aortic Valve Morphology on the Fate of the Ascending Aorta.

BACKGROUND: Bicuspid aortic valves are associated with ascending aortic pathology, but their impact on long-term outcomes, including aortic growth and adverse events, remains unclear. METHODS AND RESULTS: This prospective cohort-study included adult patients undergoing aortic valve surgery or ascending aortic surgery at a single center (2007-2013). The primary outcome was aortic diameter growth; secondary outcomes included all-cause mortality and adverse aortic events. Inverse probability of treatment weighting was used to adjust for baseline differences. Among 570 patients, 204 underwent ech…

Association of the bovine aortic arch and bicuspid aortic valve with thoracic aortic disease.

BACKGROUND: Both bicuspid aortic valve (BAV) and bovine aortic arch (BA) are considered as markers of thoracic aortic disease (TAD). But the association between them is not yet clear. This study aimed to explore the potential association of BAV and BA with TAD. METHODS: The study involved 449 participants who underwent their first aortic valve replacement in Fuwai Hospital from June 2017 to March 2018. All patients underwent multidetector computed tomography and echocardiography before surgery. The clinical characteristics were recorded to analyze the association between BAV, BA, and TAD. The…
