We recently observed numerous microspheroliths consisting of microscopic organized crystalline structures of varying shapes, sizes, and colors in a bile specimen from a 65-yr-old woman obtained directly from the gallbladder during a surgical procedure for cholecystectomy. Detection of microspheroliths could be very useful in the diagnostic approach to patients with recurrent pain in the biliary region or with acute pancreatitis of unknown origin. To date, we are unaware of any cytologic reports describing microspheroliths in bile.

While variations of the common bile duct are relatively common, true duplication of the common bile duct is very rare. Such deviation from normal anatomy, if unrecognized, may cause problems during surgical intervention. Here we present a case of a unique variant of an accessory bile duct which arises from the confluence of the right and left hepatic ducts and runs down to join the pancreatic duct to form a long, common channel before opening into the papilla with the main duct.

Relation between duodenal anatomy and function was studied in cannulated sheep. It was concluded that during the interperistaltic periods, gravitational backflow of bile and admixed pancreatic juice towards the pylorus may provide a means for desorption of enterokinase from the mucosal surface of the cranial duodenum. The biliary back and forth ("bile shuttle") may contribute to the utilization of duodenal enterokinase.

Miniaturization of ultrasound probes (6 F, 20 MHz) enables to perform intraluminal sonography of the bile duct in patients in whom a percutaneous transhepatic access has been achieved for diagnostic or therapeutic reasons. Initial experiences in 13 patients with malignant bile duct obstruction and in 8 patients with benign bile duct disease are described. In the submitted paper the authors present an account on 88 patients with reconstructions of the biliary pathways, most frequently after iatrogenic damage. Check-up examinations revealed that the results in patients who survive for prolonged periods and were reoperated in time, unless they developed irreversible liver damage, are very satisfactory. 81.8% of the patients have no subjective complaints and the objective finding (laboratory examination, HIDA) is consistent with the subjective report.
