Early and late fracture following extensive limb lengthening in patients with achondroplasia and hypochondroplasia.

Two types of fracture, early and late, have been reported following limb lengthening in patients with achondroplasia (ACH) and hypochondroplasia (HCH). We reviewed 25 patients with these conditions who underwent 72 segmental limb lengthening procedures involving the femur and/or tibia, between 2003 and 2011. Gender, age at surgery, lengthened segment, body mass index, the shape of the callus, the amount and percentage of lengthening and the healing index were evaluated to determine predictive factors for the occurrence of early (within three weeks after removal of the fixation pins) and late f

A Novel Method for Lengthening Iliac Crest Grafts for Mandibular Reconstruction.

The objective of this article is to propose a technique for mandibular reconstructions of up to 17 cm, in order to obtain anterior iliac crest grafts smaller than those required, and then lengthen them using corticotomies. This technique has been performed in 5 patients with an indication for reconstruction after partial mandibulectomy due to pathology. Reconstruction of the body of the mandible, including the angle in some cases, was performed using nonvascularized anterior iliac crest grafts. These were elongated using corticotomies and fixed to cover the defect using reconstruction plates a

Limb salvage following snakebite using acute limb shortening and secondary lengthening.

A case of 16-year-old boy from the remote tribal population of Uttarakhand is described, who sustained a viper snakebite. The patient after various interventions and referrals developed locoregional and systemic complications. He not only had an open tibiofibular fracture but a large bimalleolar defect over his lower limb. The wound infection with underlying osteomyelitis progressed to septic shock and failure of the conventional cross-leg flap. Computed tomography scan of the limb revealed a single patent vessel, eliminating the option of microvascular flap. Limb amputation was considered for

Neuromuscular injury during limb lengthening: a longitudinal follow-up by rabbit tibial model.

Limb lengthening has been used for many years to correct congenital anomalies and unequal leg length. Some surgeons focus on bone genesis and anatomical alignment, and pay less attention to potential damage to muscles, nerves, and vessels during the gradual stretching. In an attempt to identify the presence and extent of neuromuscular abnormalities during leg elongation, a rabbit tibial osteotomy model was used for 17 male rabbits, and fixation-distraction apparatuses were applied at a rate of 1 mm lengthening per day. Electrophysiological test was used to investigate both hindlegs every 2 wee

Demineralized bone matrix: basic science and clinical applications.

Bone grafting with demineralized bone matrix (DBM) is useful in reconstructive surgery to ultimately provide anatomic alignment, restore function, or augment/change the biomechanics of the foot and ankle. DBM should be used in conjunction with a transplant or implant displaying mechanical strength. DBM can augment cortical grafts used for bridging gaps or defects and lengthening procedures by increasing the connectivity of the structural graft with the host bone. Another useful application is providing a biologic boost to patients who have less-than-ideal physiology. Because DBM has higher con

Is there an Increase in Valgus Deviation in Tibial Distraction Using the Lengthening Over Nail Technique?

BACKGROUND: During tibial lengthening, the soft tissues of the posterolateral compartment produce distraction-resisting forces causing valgus angulation. Although this occurs with the classic Ilizarov method, whether a valgus deformity develops with the lengthening over nail (LON) technique is questioned, because the intramedullary nail is thought to resist deforming forces and adequately maintain alignment of the distracted bone. QUESTIONS/PURPOSES: The purposes of this study were to (1) determine the amount of valgus deviation during tibial lengthening with the LON technique; and (2) analyz

Limb lengthening in achondroplasia: a cost-effectiveness analysis using a Markov model.

BACKGROUND: Disproportionate short stature is one of the most characteristic clinical traits of achondroplasia, a condition with a significant impact on patients' autonomy that results in a series of limitations leading to poorer health-related quality of life. Limb lengthening surgery is an effective therapeutic alternative for increasing the stature of persons with achondroplasia. The purpose of this study was to evaluate the cost-effectiveness of bone lengthening in patients with achondroplasia and to estimate the economic impact of the procedure on the Spanish health system by means of a M…

[Progressive correction of an ulnar clubhand secondary to post-traumatic epiphysiodesis of the distal ulna].

The authors report the correction of an ulnar club-hand in a 16 year-old boy who complained of recurrent wrist pain after a fracture of both bones of his left forearm treated by internal fixation at the age of nine years. Correction was achieved by progressive ulnar lengthening, using Ilizarov's method, without radius osteotomy or bone grafting. Union was achieved 2 months post-operatively. Functional outcome and cosmetic appearance were satisfying.

Angulation occurring during the distraction lengthening of digits.

Distraction lengthening of congenitally short digits is an accepted reconstructive procedure in hand surgery. A previously unreported complication of angulation occurring during distraction was treated by open reduction and insertion of a longitudinally placed, smooth Kirschner wire. The wire functioned as a spindle along which the distraction of the osteotomized bone fragments was continued. A satisfactory result was achieved.

Management of post-traumatic femoral defects with a monorail external fixator over an intramedullary nail.

PURPOSE: The management of limb-length discrepancy secondary to traumatic femoral bone loss poses a unique challenge for surgeons. The Ilizarov technique is popular, but is associated with long external fixator time and many complications. This retrospective study assessed outcomes of post-traumatic femoral defects managed by monorail external fixation over an intramedullary nail. METHODS: Eight patients were included from October 2015 to May 2019 with post-traumatic femoral defects that underwent treatment with monorail fixator-assisted intramedullary nailing. Primary outcome was time to bon

Labial repositioning using polymethylmethracylate (PMMA)-based cement for esthetic smile rehabilitation-A case report.

INTRODUCTION: One of the most common esthetic complaints among dental patients is a gingival smile, which may be of multifactorial etiology, e.g. gingival hyperplasia, skeletal deformities featuring overgrowth of the anterior maxilla, altered passive eruption, maxillary alveolar tooth extrusion, fine lip and hypermobility of lip elevator muscles, which must be diagnosed prior to treatment so that the appropriate management approach can be selected. Maxillary overgrowth may give rise to subnasal skeletal depression where the upper lip retracts to during smiling, causing gingival exposure. The o

Infection rates and risk factors with magnetic intramedullary lengthening nails.

INTRODUCTION: Surgical site infection (SSI) related to magnetic intramedullary lengthening nails (MILNs) can lead to delayed consolidation or loss of limb function, resulting in deleterious effects to a patient's quality of life. With the rise of MILNs, we sought to determine the incidence rate and risk factors for infection during limb lengthening with MILNs. METHODS: We reviewed a consecutive series of patients who underwent femoral and/or tibial lengthening with an MILN at a single institution between 2012 and 2020 (n = 420). SSI was defined according to CDC-NHSN criteria (including superf

Correction of lower-limb deformities by intramedullary nailing.

BACKGROUND: Osteotomy to correct lower-limb deformity may be necessary when residual growth precludes asymmetric epiphysiodesis. Interlocking intramedullary nailing (IMN) provides minimally invasive stable fixation, spanning fragile segments, and compatible with bone lengthening, but requires precise preoperative planning and meticulous operative technique, as the room for postoperative adjustment is very limited. Our aim here is to present practical indications, planning principles, instrumentation, operative steps and postoperative management for safe and effective correction of femoral and …

Vertical lengthening genioplasty: A new osteotomy technique.

Vertical insufficiency of the chin imbalances the orofacial and aesthetic parameters of the lower third of the face. We here propose a new osteotomy technique for chin lengthening. Our procedure is based on a single osteotomy and it does not require any interposition of a bone graft. In our experience with seven patients, we report good aesthetic and morphometric outcomes at one year.

Meeting report from the 3rd ISCBH-ERN BOND Achondroplasia Workshop on Long Bone Pathology in Children with Achondroplasia, Salzburg, Austria 22nd June 2024.

A pre-meeting workshop on Long Bone Pathology in Children with Achondroplasia was held in Salzburg, Austria at the 11th International Conference on Children's Bone Health (ICCBH) 22-25 June 2024. There remains poor understanding and awareness amongst physicians managing achondroplasia of the underlying pathophysiology, radiology, natural history and orthopaedic procedures available for long bone deformities and restrictions. The structure of the workshop consisted of presentation of the results of a multinational patient survey on views of leg lengthening in achondroplasia, lectures, a debate

Defining Postoperative Weight Trends in Pediatric Patients Undergoing Limb Reconstruction Surgery by Distraction Osteogenesis.

BACKGROUND: Distraction osteogenesis (DO) is widely used to treat limb length discrepancy, deformity, and segmental defects. During recovery, body weight may change due to surgical stress, increased osteogenic metabolic demand, pain, reduced mobility, appetite changes, and disuse-related muscle loss. Because early weight change may affect wound healing, infection risk, and regeneration quality, defining postoperative weight trajectories is clinically relevant. We aimed to characterize early weight change after DO-based limb reconstruction and determine when deviation from baseline is greatest.
