Percutaneous cerclage wiring followed by intramedullary nailing for subtrochanteric femoral fractures: a technical note with clinical results.

BACKGROUND: Although intramedullary nailing is an ideal treatment for subtrochanteric femoral fractures, it is technically challenging in fractures extending into the nail entry area and/or involving the lesser trochanter. Although the application of circumferential wire may facilitate reduction in these situations, its use remains controversial due to possible blood supply disturbances to underlying bone. In the present study, we evaluated complex subtrochanteric fractures treated by percutaneous cerclage wiring followed by intramedullary (IM) nailing for anatomical fracture reduction and uni

[Free vascularized bone transfer en bout de chaîne].

The vascularized bone proved its capacity to be healed, without resorption even in sclerotic or infected tissue. Reconstruction of the distal part of the finger is often difficult for a pedicle transplant and a conventional bone transplant disappears. So the free vascularized bone transfer (FVBT) is indicated in distal post-traumatic finger amputations. According to the loss of substance to be reconstructed, the vascularised bone is harvested with some pulp, the nail complex or the cartilage of growth for a child. We report our experience of the technique of wrap around modified, of partial tr

Massive Osteolysis of the Femur (Gorham-Stout Disease) Requiring Total Femur Replacement: A Case Report.

CASE: Gorham-Stout disease (GSD) is a rare disorder marked by massive bone destruction. A 56-year-old woman with GSD presented to our hospital with a proximal femur fracture. She was initially diagnosed with a simple fracture and underwent open reduction and internal fixation using an intramedullary nail. However, the delayed diagnosis of GSD resulted in postponed appropriate treatment. Although the intramedullary nail helped prevent mechanical bone failure, extensive osteolysis continued, ultimately necessitating total femur replacement. CONCLUSION: This case highlights the importance of con

The locked flexible intramedullary humerus nail in pediatric femur and tibia shaft fractures: a feasibility study.

There are several options for the treatment of long bone fractures in skeletally immature patients. Surgeon experience, type of fracture, and the possibility of damage to the physeal area dictate individual fracture management patterns. Notably, nail devices have not gained popularity in this patient group. Intramedullary locking nails have become the standard of care in adult patients due to decreased morbidity and mortality. A novel nail has been developed for humeral shaft fractures that uses a lateral starting position to avoid damage to the rotator cuff in humeral fracture fixation. This

Management of comminuted proximal ulna fracture-dislocations using a multiplanar locking intramedullary nail.

Intramedullary nails have been used for the fixation of olecranon fractures in an attempt to reduce the soft tissue irritation and resulting need for hardware removal seen with plating and tension banding. Further benefits include preservation of vascular supply, and increase stability and improved compression over some alternative techniques. Most intramedullary nails have been limited to simple olecranon fractures or osteotomies. One novel multiplanar, locking intramedullary nail, however, is indicated to stabilize all fracture patterns of the proximal ulna, including the coronoid. This part

Hypoplastic-hypocalcified enamel of teeth and dysplastic nails: an undescribed ectodermal dysplasia syndrome.

Hypoplastic-hypocalcified enamel of all permanent teeth and dysplasia of finger- and toe-nails were found in a 17-year-old Japanese male. Physical examination revealed no remarkable changes in skin, hair, sweat glands, bones, etc. Family history revealed the same abnormalities of teeth and nails in his mother's brother. A review of the literature concerning ectodermal dysplasia syndromes failed to reveal a combination of hypoplastic-hypocalcified enamel and dysplastic nails without changes in any other ectodermal tissues.

Intramedullary nailing of (impending) pathologic fractures.

Forty-one (impending) fractures were reviewed in 39 patients with metastatic bone disease. Thirty-one lesions were located in the femur, and the remaining ten lesions were located in the humerus. In 22 cases there were multiple lesions in the affected bone. All patients were treated with intramedullary nailing, 21 times because of a pathologic fracture and 20 times because of an impending pathologic fracture. There was no mortality related to the surgical procedures. In nine patients the postoperative course was complicated (four technical and five systemic complications). Pain relief was achi

Femoral head avascular necrosis after interlocking nail of a femoral shaft fracture in a male adult: a case report.

Reamed interlocking nail through the piriformis fossa remains the golden standard for treatment of femoral shaft fracture. Fracture healing rates are 95-99%, and infection rates less than 1% (Clawson et al. in J Bone Joint Surg (Am) 53:681-692, 1971; Winquist et al. in J Bone Joint Surg (Am) 66:529-539, 1984; Brumback et al. in J Bone Joint Surg (Am) 70:1453-1462, 1988). Previous reports recognize avascular necrosis of the femoral head as a complication of antegrade interlocking nail in the adolescent (Beaty et al. in J Pediatr Orthop 14:178-183, 1994; Mileski et al. in J Bone Joint Surg (Am)

Biomechanical behavior of retrograde intramedullary nails in distal femoral fractures.

Fractures of the distal femur affect three different groups of individuals: younger people suffering high-energy trauma, elderly people with fragile bones and people with periprosthetic fractures around previous total knee arthroplasty. Main indications of intramedullary nailing are for supracondylar fractures type A or type C of the AO classification. The main objective of the present work is to analyze, by means of FE simulation, the influence of retrograde nail length, considering different blocking configurations and fracture gaps, on the biomechanical behavior of supracondylar fractures o

Outcomes of periprosthetic distal femur fractures following total knee arthroplasty: Intramedullary nailing versus plating.

BACKGROUND: Periprosthetic distal femur fractures (PPDFFs) present a challenge in terms of optimizing fixation in patients with poor bone quality and limited bone stock. The main treatment options include laterally based plating and intramedullary nailing. We hypothesized that treatment of PPDFFs with intramedullary nails would result in improved union rate, fewer complications, and an equivalent rate of malalignment compared to plating. MATERIALS AND METHODS: Cases of PPDFFs were identified through a query of our institutional trauma database between 2011-2018. Adult patients (>18 years) wer

Posterior spinal fusion using a pedicle nail system with polymethylmethacrylate in a paraplegic patient after vertebral collapse caused by osteoporosis.

BACKGROUND CONTEXT: Because posterior decompression and fusion for vertebral collapse in an osteoporotic spine sometimes results in a pullout of the pedicle screw. Several authors reported that fixation of pedicle screws in severely osteoporotic bone could be improved by inserting polymethylmethacrylate (PMMA) into the hole before inserting the screw. However, pedicle screws were not designed to be used with PMMA. PURPOSE: To report a patient with vertebral collapse who were treated using a novel-concept, pedicle nail using with PMMA. STUDY DESIGN: Case report. METHODS: The patient was a 74

Comparative study of two retrograde locked intramedullary nail designs for ankle arthrodesis: A finite element analysis.

Ankle arthrodesis is the gold standard for treatment of end-stage arthritis. The goal of ankle arthrodesis is to obtain bony union between the tibia and the talus. Retrograde intramedullary nailing is typically reserved for ankle and subtalar joints arthrodesis. The purpose of this study is to evaluate the effect of two different materials, two locking pin configurations and two nail designs of a retrograde locked intramedullary nail used for ankle arthrodesis. Using the finite element analysis, a numerical study of ankle arthrodesis was developed to evaluate the effect of materials: TI-6Al-4V

[Atypical presentation of lung cancer].

The authors present the clinical case of a 61-year-old male admitted to the emergency department, with arthritis of his right wrist and left knee as well as left leg cellulitis, digital clubbing and 'hourglass' nails. Subsequent clinical work-up led to the final diagnosis of stage IV adenocarcinoma of the lung, with bone metastasis. The authors discuss the association between this type of lung neoplasm and osteoarticular manifestations as well as its first presentation as bone metastasis.

Ipsilateral floating hip and knee, or floating lower limb - A rare case report.

INTRODUCTION: Ipsilateral floating hip and knee injuries at the same time are very rare injuries, occurring due to high velocity trauma. With a small number of such case reports, the treatment guidelines regarding surgical fixation are yet to be formulated. CASE REPORT: We report an interesting case of hip dislocation, comminuted acetabular fracture, intertrochanteric and shaft of femur fracture and both bones leg fracture ipsilaterally. Open reduction of hip, antegrade femoral nailing with recon nail and closed tibial nailing was done in a single setting. Acetabular fracture was treated non-

Problems encountered in leg shortening.

We reviewed 46 leg-shortening operations (37 femoral and nine tibial), performed by different methods, to assess the incidence of complications and permanent disability. Shortening of as much as 7.5 cm in the femur and 5 cm in the tibia was achieved in men of normal height without any loss of function. Complications were seen with all surgical techniques despite the use of modern implants. Most problems arose from inadequate stabilisation of the osteotomy. The most reliable method of femoral shortening was open subtrochanteric osteotomy with preservation of the isthmus, and fixation with an in

A retrospective study of squamous cell carcinoma of the nail unit diagnosed in a Belgian general hospital over a 15-year period.

BACKGROUND: Squamous cell carcinoma (SCC) is the most common malignant tumor at the nail unit. It mainly affects middle-aged men, with a peak incidence between 50 and 69 years of age. Diagnosis is often delayed because of the slow evolution of the lesion and multiple clinical features. OBJECTIVE: We sought to characterize the different clinical and histopathological patterns of SCC of the nail unit and evaluate their therapeutic outcome. METHODS: Records for 58 patients were retrieved from our department's dermatopathology database over a period of 15 years (1995-2011) and the patients recon
