[Mechanisms of Neurodegeneration in Multiple Sclerosis].

Multiple sclerosis (MS) is a chronic inflammatory disease of the central nervous system, which results in the formation of primary demyelinating lesions in the white and gray matter, as well as diffuse axonal and neuronal damage. Although there has been substantial progress in drug research in relapsing-remitting MS, treatment of progressive forms of the disease, can be challenging. Diffuse and compartmentalized lymphocyte and macrophage infiltration of the CNS tissue inhibits the differentiation of myelinating mature oligodendrocytes, disrupting the process of remyelination. Chronic inflammat

Resolution of peritumoral brain edema following excision of meningioma.

We studied the resolution of peritumoral brain edema after meningioma excision. In twenty-nine patients with meningioma, the total volume of tumor and the peritumoral edema were measured planimetrically by serial CT scans and MRI with or without contrast enhancement. Four different patterns of postoperative resolution of hypodense volume on CT were observed: Group A: a large hypodensity rapidly decreased and disappeared, which may be related to the clearance of the real peritumoral edema in meningioma. Group B: a small hypodensity gradually disappeared. Group C: the hypodensity remained unchan

Blood-brain barrier damage in experimental African trypanosomiasis.

African sleeping sickness is characterized by progressive central nervous system (CNS) involvement, leading to the so-called secondary or late stage in which there are widespread inflammatory changes with lymphoplasmocytic infiltration. A study was made of blood-brain barrier (BBB) integrity in the late stages of a rodent model by assessing the uptake of the fluorescent fluid-phase marker sulphorhodamine B into the brain tissue. Brain oedema was estimated from brain weight, density and electrolyte concentrations. Trypanosome distribution was studied by light and electron microscopy. At 35 days

Neuroimaging in posterior reversible encephalopathy syndrome.

The terms posterior reversible leukoencephalopathy, reversible posterior cerebral edema syndrome, and posterior reversible encephalopathy syndrome (PRES) all refer to a clinicoradiologic entity characterized by headaches, confusion, visual disturbances, seizures, and posterior transient changes on neuroimaging. Clinical findings are not sufficiently specific to readily establish the diagnosis; in contrast, magnetic resonance imaging pattern is often characteristic and represents an essential component of the diagnosis of PRES. Typical lesions predominate in the posterior white matter, with som

MRI findings in patients with symptomatic localization-related epilepsies beginning in infancy and early childhood.

We studied MRI findings in patients with symptomatic localization-related epilepsy (S-LRE) beginning in infancy and early childhood and their relation to some clinical features. MRI findings were investigated in 183 patients with S-LRE beginning before 6 years of age. S-LRE was defined as an epileptic syndrome manifesting with only partial seizures at least for a certain period during the clinical course, having specific neuroimaging abnormalities considered to be related to epileptic seizures, moderate or severe mental retardation, or cerebral palsy. The timing of injury was divided into the

Motor dysfunction profiles in traumatic brain injury and postconcussion syndrome.

Motor measures are sensitive to central lesions, but they are also affected by peripheral injury and motivation. The motor skills profiles of proven brain injury clients were compared with the profiles of healthy postconcussion patients. The chief result was a double dissociation: The traumatic brain injury (TBI) group produced a motor dysfunction gradient consistent with upper motor neuron disease, while the compensation-seeking postconcussion group produced a nonphysiologic pattern. Objective measures of behavioral pain and emotional distress did not correlate with the findings. Motor skill

Transient elevation of the intracranial pressure increases the infarct size and perifocal edema after subsequent middle cerebral artery occlusion in the rat.

The objective of this study was to mimic in a simple experiment the two major brain insults sustained by the patient with a subarachnoid hemorrhage, that is, the ictus and the subsequent delayed reduction of focal cerebral blood flow caused by vasospasm without the interference of subarachnoid blood, to test the hypothesis that ictal events not related to the presence of blood in the subarachnoid space per se may be important for the development of ischemic deficits and cerebral infarction when vasospasm develops. Groups of rats were subjected to a sudden transient elevation of the intracrania

Comparison of patients with and without emotional/behavioral deterioration during the first year after traumatic brain injury.

The records of 34 patients who showed evidence of emotional deterioration 6 months or more following traumatic brain injury were compared with a group of patients matched for severity of initial neuropsychiatric impairment who did not show deterioration. The deterioration group was more likely to have been involved in assaults and less likely to have been involved in a motor vehicle accident than the improvement group. The deterioration group was also more likely to have a prior history of alcohol abuse and to have sustained a skull fracture with left parietal lobe injury than the improvement

Diabetes insipidus in severely brain damaged children.

Nineteen children aged four months to 15 years with diabetes insipidus (DI) secondary to severe brain insults were studied. The main primary brain insult was severe head injury in 12, anoxic ischemic brain damage in four, encephalitis in two, Reye's syndrome in one. Sixteen children died, and three survived. The time from insult to onset of polyuria varied from several hours to one month, and was significantly shorter in head trauma patients, 1.8 +/- 0.9 vs. 9.3 +/- 1.9 days for patients with anoxic ischemic brain damage (p less than 0.03). Twelve of the 19 patients met the criteria for brain

Pharmacological targeting of secondary brain damage following ischemic or hemorrhagic stroke, traumatic brain injury, and bacterial meningitis - a systematic review and meta-analysis.

BACKGROUND: The effectiveness of pharmacological strategies exclusively targeting secondary brain damage (SBD) following ischemic stroke, aneurysmal subarachnoid hemorrhage, aSAH, intracerebral hemorrhage (ICH), traumatic brain injury (TBI) and bacterial meningitis is unclear. This meta-analysis studied the effect of SBD targeted treatment on clinical outcome across the pathological entities. METHODS: Randomized, controlled, double-blinded trials on aforementioned entities with 'death' as endpoint were identified. Effect sizes were analyzed and expressed as pooled risk ratio (RR) estimates wi

Cognitive development following early brain injury: evidence for neural adaptation.

Over the past few decades a large body of work from developmental neurobiology has shown that mammalian brain development is the product of dynamic and adaptive processes operating within highly constrained, but continually changing, biological and environmental contexts. The recent study of children with prenatal focal brain injury supports this dynamic view of development for humans. Children's injuries often affect substantial portions of one cerebral hemisphere, resulting in damage that would compromise cognitive ability in adults. However, longitudinal behavioral studies of this populatio

[Perinatally-induced hypoxic-ischemic encephalopathy? Possibilities of retrospective evaluation from the neuropediatric viewpoint].

This paper analyzes the feasibility of determining retrospectively, from a neuropediatric standpoint, whether an existing neurological disorder can be traced back to a perinatal hypoxic-ischemic encephalopathy. In principal, we can assume that there is a correlation between this time and pathogenesis of the brain injury on the one hand and the neurological symptoms on the other if these conditions are fulfilled 1. In the first days of live term infants revealed cerebral symptoms (like abnormality of muscle tone and consciousness and seizures) which manifest themselves in a typical sequence and

[Transcranial Doppler sonography with evaluation of CO2 reactivity in craniocerebral trauma].

17 patients with severe head injury were examined by transcranial Doppler sonography. Resting values and CO2 reactivity were determined. The resting measurements had no prognostic value. There was no significant side difference in patients with unilateral intracranial lesions. CO2 reactivity was 2.8 +/- 2.3%/mmHg pCO2 in patients with good outcome, whereas in patients with bad outcome CO2 reactivity was 0.6 +/- 5.1%/mmHg pCO2. However, in the acute situation after the trauma there was no difference between the two. In patients with a unilateral lesion we found a significantly lower CO2 reactiv

White-matter lesions on CT in Alzheimer patients: relation to clinical symptomatology and vascular factors.

Fifty-four patients with Alzheimer's disease (AD) were examined for white-matter lesions (WMLs) using computerized tomography. WMLs were more frequent in late-onset AD (LAD) (26/34-76%) than in early-onset AD (EAD) (5/20-25%) (p less than 0.0001), in AD without parietal predominance (10/11-91%) (p less than 0.005) than in AD with parietal predominance (5/15-33%), and in AD with confusional symptoms (11/12-92%) than in AD without confusional symptoms (4/14-29%) (p less than 0.001). The supine systolic blood pressure was higher in AD with WMLs (151 +/- 20) than in AD without WML (139 +/- 22) (p

How strong is the evidence that brain serotonin neurons are damaged in human users of ecstasy?

Understanding the diverse functions of serotonin in the human brain can be obtained through examination of subjects having a lower than normal number of brain serotonin neurons. Behavioral abnormalities consistent with brain serotonergic damage have been reported in some polydrug users who also use the neurotoxin ecstasy (methylenedioxymethamphetamine, MDMA). This review evaluates the evidence from neuroimaging studies that brain serotonergic damage is a feature of human users of ecstasy. To date, neuroimaging studies designed to establish whether levels of brain serotonin neurons are lower th

Docosahexaenoic acid (DHA) prevents binge ethanol-dependent aquaporin-4 elevations while inhibiting neurodegeneration: experiments in rat adult-age entorhino-hippocampal slice cultures.

Repetitive binge intoxication with ethanol (alcohol) in adult rats, mimicking chronic ethanol abuse in alcoholics, causes trauma-like brain edema and relatively selective neurodegeneration of hippocampal dentate granule cells and pyramidal neurons in the temporal cortex (especially entorhinal cortex). We have now modeled the aspects of this type of acquired brain damage in vitro with rat entorhino-hippocampal slice cultures of adult brain age (62 ± 3 days). When sequentially treated (four 16-h overnight exposures) with 100 mM ethanol, the slices display elevated levels of aquaporin-4 (AQP4) wa

Quantitative medial temporal lobe brain morphology and hypothalamic-pituitary-adrenal axis function in cocaine dependence: a preliminary report.

Preclinical and clinical studies have shown that cocaine increases plasma adrenocorticotropin hormone (ACTH) and cortisol. Chronic elevation of plasma cortisol exerts direct toxic effects upon hippocampal neurons and exacerbates hippocampal damage resulting from ischemia and seizures. The authors tested for evidence of hippocampal damage in patients with chronic cocaine dependence. Medial temporal lobe and total brain volumes were quantified using magnetic resonance imaging (MRI) in 27 patients with cocaine dependence and 16 healthy subjects. Basal and ovine corticotropin releasing hormone (oC

Clinical importance of the anterior choroidal artery: a review of the literature.

The anterior choroidal artery (AChA) is a critical artery in brain physiology and function. The AChA is involved in many diseases, including aneurysm, brain infarct, Moyamoya disease (MMD), brain tumor, arteriovenous malformation (AVM), etc. The AChA is vulnerable to damage during the treatment of these diseases and is thus a very important vessel. However, a comprehensive systematic review of the importance of the AChA is currently lacking. In this study, we used the PUBMED database to perform a literature review of the AChA to increase our understanding of its role in neurophysiology. Althou
