Ethyl pyruvate protects against hypoxic-ischemic brain injury via anti-cell death and anti-inflammatory mechanisms.

Ethyl pyruvate (EP) is protective in experimental models of many illnesses. This study investigates whether EP can protect against neonatal hypoxic-ischemic (H-I) brain injury. Pre-treatment with EP significantly reduced brain damage at 7 days post-H-I, with 50 mg/kg EP achieving over 50% recovery in tissue loss compared to vehicle-treated animals. Delayed treatment with EP until 30 min after H-I was still neuroprotective. EP-afforded brain protection, together with neurological function improvement, was observed up to 2 months after H-I. We further demonstrated an inhibitory effect of EP on c

Brain death and donor management in the intensive care unit: experiences over the last 3 years.

We sought to examine the frequency of brain death in the ICU, the donation rate, and the problems encountered during donor management between the years of January 2000, the date we began treating patients with neurologic pathologies and traumatic brain injury, and March 2003. Between January 2000 and March 2003, 134 patients diagnosed with brain death in our ICU were studied prospectively for the reason of brain death, the time between admission to ICU, and the diagnosis of brain death, the frequency of diabetes insipidus, the inotrope requirement, hypothermia, electrolyte imbalance, arrhythmi…

Metabolic impairment elicits brain cell type-selective changes in oxidative stress and cell death in culture.

Abnormalities in oxidative metabolism and inflammation accompany many neurodegenerative diseases. Thiamine deficiency (TD) is an animal model in which chronic oxidative stress and inflammation lead to selective neuronal death, whereas other cell types show an inflammatory response. Therefore, the current studies determined the response of different brain cell types to TD and/or inflammation in vitro and tested whether their responses reflect inherent properties of the cells. The cells that have been implicated in TD-induced neurotoxicity, including neurons, microglia, astrocytes, and brain end

Heme oxygenase-2 acts to prevent neuronal death in brain cultures and following transient cerebral ischemia.

Heme oxygenase (HO) cleaves the heme ring to form biliverdin, which is rapidly reduced to bilirubin, carbon monoxide, and iron. HO1, the first form of the enzyme discovered, is an inducible protein, concentrated in tissues that are exposed to degrading red blood cells and stimulated by hemolysis and numerous other toxic perturbations to eliminate potentially toxic heme. By contrast, HO2 is constitutive and most highly concentrated in neural tissues. Carbon monoxide, formed from HO2, is a putative neurotransmitter in the brain and peripheral autonomic nervous system. HO1 regulates the efflux of

Unveiling the Role of Pain Catastrophising as a Predictor of Fear of Personal Death amongst Brain Tumour Patients.

OBJECTIVES: According to the National Health Portal, brain cancer is witnessing an upward curve in India. The reports by medical professionals suggest that brain tumours will become the second most common cancer by 2030. Any life-threatening illness not only impacts the physical well-being of the patient but also affects the mental well-being of the patient. As an interdisciplinary field, psychology aims to understand the psychosocial components of an illness that goes beyond the realm of medical treatments. Hence, the present study focused on two psychological constructs, namely Pain Catastro

Fatal pneumopericardium in a road traffic accident: An uncommon contributor to death in the presence of traumatic brain injury.

A pneumopericardium is a condition characterized by the presence of air or gas within the pericardial sac. Sixty percent of pneumopericardium in adults is caused by traumatic events, specifically penetrating chest trauma. Clinical monitoring is sufficient for patients with no hemodynamic instability while waiting for air absorption. In contrast, if pericardial tamponade develops, pericardiocentesis or immediate surgery to seal the channel between air and pericardium is required. However, whenever there are multiple injuries are present, the clinician's attention can divert to a more severe for

The use of apnea test and brain death determination in patients on extracorporeal membrane oxygenation: A systematic review.

OBJECTIVE: To review practices of brain death (BD) determination in patients on extracorporeal membrane oxygenation (ECMO). METHODS: A systematic search was applied to PubMed and 6 electronic databases from inception to May 22, 2019. Studies reporting methods of BD assessment in adult patients (>18 years old) while on ECMO were included, after which data regarding BD assessment were extracted. RESULTS: Twenty-two studies (n = 177 patients) met the inclusion criteria. Eighty-eight patients (50%) in 19 studies underwent the apnea test (AT); most commonly through decreasing the ECMO sweep flow …

A case of suspected brain death within 24 hours after ingestion of tolfenpyrad and an organophosphorus insecticide.

A 55-year-old man attempted suicide by ingesting tolfenpyrad, emulsion formulation insecticide, and organophosphorus pesticide emulsion, each three gulps. He was found lying on the floor and his family called an ambulance an hour later from ingesting. On arrival at our hospital, his Glasgow Coma Scale score was 14 (E3V5M6), his vital signs were stable, and he was able to converse. Activated charcoal and laxatives were injected through a gastric tube, and continuous administration of pralidoxime iodide was started. After hospitalization, he complained of nausea and his consciousness level decre

Technical aids in the diagnosis of brain death: a comparison of SEP, AEP, EEG, TCD and CT angiography.

BACKGROUND: The use of technical aids to confirm brain death is a controversial matter. Angiography with the intra-arterial administration of contrast medium is the international gold standard, but it is not allowed in Germany except in cases where it provides a potential mode of treatment. The currently approved tests in Germany are recordings of somatosensory evoked potentials (SSEP), brain perfusion scintigraphy, transcranial Doppler ultrasonography (TCD), and electroencephalography (EEG). CT angiography (CTA), a promising new alternative, is being increasingly used as well. METHODS: In a …

Prevention of the death of the rat axotomized hypoglossal nerve and promotion of its regeneration by bovine brain gangliosides.

We have examined the time course of the neuronal death and regeneration of rat axotomized hypoglossal nerve with various conditions of the nerve resection, and established a useful system to measure neurotrophic activities of bioactive substances. In this system, neuronal death can be evaluated by counting surviving neurons in the nucleus of hypoglossal neuron at the brain stem, and the degree of the regeneration can be measured by counting horseradish peroxidase-positive cells at the same region after injection of horseradish peroxidase into tongue. Using this system, the effects of brain gan

Comparative Assessment of Transcranial Doppler and MRI Perfusion Imaging in a Japanese Child With Probable Brain Death.

The determination of brain death differs internationally, although in Japan, it is legally recognized only when organ donation is intended. Otherwise, the condition is termed probable brain death. Ancillary tests, including transcranial Doppler (TCD) ultrasonography, are not routinely incorporated into the clinical framework for pediatric brain death evaluation. In this report, we describe the case of a Japanese child with probable brain death, in whom both TCD and MRI revealed an absence of cerebral perfusion, providing valuable comparative data. A four-year-old girl with influenza A who deve…

[Results of surveys on brain-death determination: Report from the Neuroemergency section of the Japanese Society of Neurology].

There are very few organ transplants from brain-dead donors (BD transplants) in Japan compared with other countries. The neuroemergency section of the Japanese Society of Neurology (JSN) conducted a survey on the participation of neurologists in BD determination. The target of the survey was educational and quasi-educational institutions of the JSN. It was found that neurologists served as doctors to legally declare BD in 38% of institutions. Many institutions had neurological staffs with the skills required for BD determination, such as EEG reading (97%), neurological evaluations of BD (93%),

Optimal duration of the apnea test for determining brain death: Benefit of the short-term apnea test.

BACKGROUND: The criteria for brain death determination have not been unified globally, and there is no global consensus on the apnea test, which is essential for determining brain death. Since the apnea test is associated with many complications, we aimed to determine an optimal duration of the apnea test. METHODS: We analyzed the results of the apnea test performed for brain death determination between August 2013 and February 2021 at a single institution in South Korea. Elevations in the partial pressure of carbon dioxide and mean arterial blood pressure fluctuations over time in the apnea …

Positive fluid balance is associated with death and severity of brain injury in neonates with hypoxic-ischemic encephalopathy.

OBJECTIVE: To investigate the association between fluid balance during therapeutic hypothermia (TH) and severity of brain injury on magnetic resonance imaging (MRI) in neonates with hypoxic-ischemic encephalopathy (HIE). STUDY DESIGN: This is a secondary analysis of data from a prospective observational study in neonates with HIE. Daily net positive fluid balance during TH was investigated for association with the adverse primary outcome of death or moderate-to-severe brain injury on MRI using multivariable logistic regression. RESULTS: Of the 150 neonates included, 50 suffered adverse outco
